Injuries to the carpometacarpal joints are uncommon and are sometimes overlooked on initial radiographs. Their recognition depends on a careful physical and radiographic examination that may require trispiral tomograms and CT scans. Most injuries can be treated successfully by closed reduction and percutaneous fixation of the joint(s) using Kirschner wires. Chronic joint injuries can also be effectively treated, and if surgery is required, an arthrodesis will restore stability and eliminate discomfort.

We report a case of a rare injury, lateral fracture dislocation of the second and third carpometacarpal joints, which presented late and was managed by fusion of the involved joints.

Four acute cases of volar carpometacarpal dislocations of the medial four metacarpals were treated in our hospital's emergency room. Our initial diagnosis after clinical examination was confirmed by routine x-rays of the hand. Closed reduction was followed by percutaneous fixation of the carpometacarpal joints with Kirschner wires. Satisfactory function of the hand was obtained in all cases.

Injuries to all five carpometacarpal joints are rare. Only four cases have been reported in the English literature. A case of fracture-dislocations of all five carpometacarpal joints is presented. An excellent result has been obtained 2 years after open reduction and internal fixation. This case is the only one in the English literature so treated and with prolonged follow-up.

A 57-year-old right-hand-dominant woman was involved in a motor vehicle collision. Upon examination, her right hand was markedly deformed and swollen, with limited range of movement. Plain radiographs revealed dorsal dislocations of the index, long, ring, and small finger carpometacarpal joints and an avulsion fracture of the dorsal aspect of the capitate. Closed reduction was unsuccessful. Closed reduction under general anesthesia was successful on the carpometacarpal joint of the ring and small fingers, however, the long and index fingers remained irreducible. An open approach revealed that

A variety of injuries can occur to the medial four CMC joints. Chronic problems are best avoided by a combination of attentive diagnostic evaluation, appreciation of unstable injury patterns, and prompt fixation either by percutaneous pinning or open reduction and internal fixation.

The thumb carpometacarpal (CMC) joint is the most critical of the 4 articulations comprising the basal joints of the thumb. Peculiar anatomy, inherent instability, and function predispose this joint to repetitive microtrauma and eventual osteoarthritis (OA), often resulting in debilitating disease. Clinical examination and radiographs are usually sufficient in evaluating this joint while MR imaging may play an important role in the evaluation of more complex pathologies and postoperative complications. This article summarizes normal MR imaging anatomy as well as degenerative, traumatic, and va

BACKGROUND: A divergent carpometacarpal dislocation is a rare, high-energy, multijoint injury where the metacarpals are dislocated in opposite directions. This differs from the more common, lower energy, single-joint carpometacarpal dislocations that feature unidirectional displacement. We report the case of a 55-year-old patient who sustained this injury and provide a systematic review of its presentation, management, and outcomes.
METHODS: Clinical details for the case report were obtained from hospital records. A comprehensive literature search in English was conducted across PubMed/MEDLIN

Symptomatic post-traumatic arthritis affecting the finger CMC joints is less common than might otherwise be expected based on the frequency of injury, especially to the fifth CMC joint. For the fifth CMC joint, the shallow concavoconvex articulation combined with the typical fracture location may provide a protective effect. Non-operative measures are typically successful, except in cases of missed fracture/dislocations and symptomatic joint instability. In these instances, re-construction emphasizes stability first, with an eye toward mobility for the ulnar column. It is common to detect a bo

We describe the diagnosis, treatment, and follow-up of a group of 13 patients with hand pain traced to pathologic conditions of the second or third carpometacarpal joints. Missed diagnosis was universal. With suspicion raised by history of injury or repeated stress and point tenderness on examination, diagnosis was confirmed by complete pain relief after injection of 1% lidocaine locally. In management of patients with occult pain in the hand, attention should be directed to the second or third carpometacarpal joints. Arthrodesis with use of an inverted triangular graft from the base of the me

An important challenge in the management of thumb carpometacarpal (CMC) joint arthritis is determining the most appropriate surgical approach for any given clinical scenario, once non-operative treatment has been exhausted. The variety of surgical procedures available highlights the fact that there is no general consensus on the optimal surgical treatment of this condition. With the advancement in wrist arthroscopy, the use of arthroscopic-assisted treatment for basilar joint arthritis has become more popular and aims to meet some of the shortcomings of open procedures. We will review arthrosc

Background: Isolated dislocation of the carpometacarpal (CMC) joints is a rare injury that accounts for less than 1% of hand injuries. Few cases of isolated volar dislocations of the fifth CMC joint have been reported, making such injuries worthy of reporting. Given the rarity of these injuries, they are easily overlooked in the emergency setting and thus require a high index of clinical suspicion. Case Report: A 57-year-old female sustained an isolated volar dislocation of the fifth CMC joint when she fell onto her outstretched right hand. Physical examination revealed an inability to move th

Although there are many surgical options to treat thumb carpometacarpal (CMC) arthritis, a precise etiology for this common disorder remains obscure. To better understand the physiology of the thumb CMC joint and treat pathology, it is helpful to examine the biomechanics, hormonal influences, and available surgical treatment options, along with the evolutionary roots of the thumb; its form and function, its functional demands; and the role of supporting ligaments based on their location, stability, and ultrastructure. It is important to appreciate the micromotion of a saddle joint and the role

Arthroscopy of the first carpometacarpal joint has been developed as a reliable surgical procedure offering an opportunity for direct evaluation of the joint's intra-articular status, with minimal morbidity to the joint capsule. The procedure uses a 1.9-mm short barrel arthroscope with standard supportive electronic equipment. After adequate anesthesia is established, approximately 5 lb. of longitudinal traction is applied to the thumb. Arthroscopic portals are located on the radial side of the abductor pollicis longus tendon (1-R portal) and the ulnar side of the extensor pollicis brevis tend

Isolated dislocation of the carpometacarpal joint of the index finger is a rare injury. This paper reports a case of an isolated dorsal dislocation of the second carpometacarpal joint not reducible by closed means. At open reduction, the extensor carpi radialis brevis was found to be interposed in the joint, blocking reduction.
