The Syndrome of Catatonia.

Catatonia is a psychomotor syndrome which has historically been associated with schizophrenia. Many clinicians have thought that the prevalence of this condition has been decreasing over the past few decades. This review reminds clinicians that catatonia is not exclusively associated with schizophrenia, and is still common in clinical practice. Many cases are related to affective disorders or are of an idiopathic nature. The illusion of reduced prevalence has been due to evolving diagnostic systems that failed to capture catatonic syndromes. This systemic error has remained unchallenged, and p

[Catatonia in the elderly].

Catatonia is a syndrome characterized by mutism, stupor, immobility, negativism, posturing, stereotypy, and echophenomena. Not only patients with schizophrenia, but also patients with general medical disease, mood disorder, and substance-related disorder exhibit catatonia. In the patients with catatonia, it is recommended to examine whether they have a general medical disease. We present two catatonic elder patients. Case 1 exhibited catatonia with vascular dementia, and was revealed to have anti-phospholipid antibody syndrome. Case 2 exhibited catatonia with dementia with Lewy bodies, and was

Catatonia: a review.

BACKGROUND: To write an up-to-date review paper on catatonia using published literature. METHODS: This review involved a search using the terms "catatonia," "stupor," "catatonic schizophrenia" and "catalepsy" in the Cochrane Database of Systematic Reviews, the Medline database and EMBASE and PsychINFO. Additional use was made of these databases in searching for randomized controlled trials, meta-analyses, cohort studies, case-control studies, case series, case reports and reviews. RESULTS: Available evidence indicates that catatonia is a common neuropsychiatric syndrome characterized by the

Hyponatraemia-associated catatonia.

This is a case report highlighting the impact of low sodium levels on one's mental status. For the first time in the literature, we see two discrete episodes of catatonia associated with hyponatraemia occurring in the same patient. On both occasions, his serum sodium levels were depleted; he became profoundly psychotic with catatonic symptoms. Recovery, with complete resolution of all symptoms, was associated with normalisation of these levels. This case exemplifies the profound effect that electrolyte imbalances, in particular sodium depletion can have on a patient's mental status and is for

Subtyping chronic catatonia: Clinical and neuropsychological characteristics of progressive periodic catatonia and chronic system catatonias vs. non-catatonic schizophrenia.

Catatonia has been defined by ICD-11 as a nosologically unspecific syndrome. Previous neuropsychiatric conceptions of catatonia such as Wernicke-Kleist-Leonhard's (WKL) one, have isolated chronic catatonic entities, such as progressive periodic catatonia (PPC) and chronic system catatonias (CSC). This study aimed at comparing the clinical and neuropsychological features of PPC, CSC and non-catatonic patients, all diagnosed with a schizophrenia spectrum disorder (SSD). The clinical and cognitive measures were compared among 53 SSD patients, first by separating catatonic (C-SSD, n = 27) and non-

Catatonia in ICD-11.

In the International Statistical Classification of Diseases and Related Health Problems Version 11 (ICD-11), the diagnostic criteria for catatonia have been extensively revised. We provide context for these changes beginning with a brief history of how catatonia has been conceptualized and a description of how the criteria for catatonia have changed across ICD versions. We also compare ICD-11 with the criteria in the latest Diagnostic and Statistical Manual of Mental Disorders (i.e., DSM-5-TR), consider clinical implications of the changes in ICD-11, and highlight conceptual areas in need of f

Catatonia in Preadolescent Children.

OBJECTIVE: The aim of the study is to describe prepubescent catatonia in very young children, which is poorly documented in the current literature and, as a result, overlooked in medical settings. METHODS: We examined a convenience sample of 10 patients at an academic center who were younger than 12 years and met criteria for catatonia. After institutional review board approval, we extracted from the electronic medical records demographic and diagnostic information, comorbidity, developmental history, and laboratory testing. Catatonia is not schizophrenia and it is treatable.

Catatonia is a cluster of motor features that appears in many recognized psychiatric illnesses, that according to the DSM-5 it is not linked as a subtype to schizophrenia anymore. The classic signs are mutism, a rigid posture, fixed staring, stereotypic movements, and stupor, which are all part of a broad psychopathology that may be found in affective, thought, neurological, toxic, metabolic and immunological disorders. Despite the many etiologies, catatonia may be a life-threatening condition with a specific treatment. Benzodiazepines are the first line therapeutic option for catatonia, being

Catatonia in DSM-5.

Although catatonia has historically been associated with schizophrenia and is listed as a subtype of the disorder, it can occur in patients with a primary mood disorder and in association with neurological diseases and other general medical conditions. Consequently, catatonia secondary to a general medical condition was included as a new condition and catatonia was added as an episode specifier of major mood disorders in DSM-IV. Different sets of criteria are utilized to diagnose catatonia in schizophrenia and primary mood disorders versus neurological/medical conditions in DSM-IV, however, an

Catatonia in the emergency department.

Disturbances of the level of awareness are a frequent motive of attendance to emergency departments where the initial assessment and management will determine the direction of their outcome. The syndrome of catatonia must be taken into consideration and although it is normally associated with psychiatric diagnoses, it is also very often found in a great variety of neurological and medical conditions. Due to the clinical complexity of catatonia, there are still difficulties in its correct identification and initial management, something that leads to diagnostic delays and increased morbidity an

Catatonia recognition and treatment.

BACKGROUND: Catatonic syndrome is no longer considered a subtype of schizophrenia but is more frequently associated with mood disorders (mania, melancholia, and psychotic depression) as well as general medical conditions (neurological disorders, drug-induced and toxic-induced conditions, metabolic conditions). A case of catatonia is reported which occurred after alcohol withdrawal. CASE REPORT: Catatonia occurred 48 hours after alcohol withdrawal in a 54-year-old woman who had stopped all her treatment (in the context of a type II bipolar disorder). A dramatic improvement of symptoms was obse

Organic catatonia: a review.

Catatonia is a clinical syndrome associated with a wide variety of psychiatric, medical and neurological disorders. Despite several reports in the literature of a wide range of medical and neurological diseases causing catatonia, there has been a tendency to consider catatonia as purely psychiatric disorder. The review attempts to look at the concept of organic catatonia from a historical viewpoint, including its place in the psychiatric classification, discusses the various etiological causes of organic catatonia, and them goes through some important management issues in organic catatonia. Catatonia, autism, and ECT.

We describe the clinical presentation, course, and treatment response of a 14-year-old boy with catatonic stupor. This patient, with a preexisting diagnosis of autism, displayed mutism, akinesia, and an extreme level of rigidity, waxy flexibility, posturing, including the psychological pillow, facial grimacing, and other involuntary movements of his upper extremities. In addition he had symptoms suggestive of a depressive disorder as well as some non-specific psychotic symptoms. Intravenous injection of sodium amytal failed to resolve any motor symptoms, although he showed a good response to t

Carbamazepine resolved within 6-24 hours the catatonic stupor in two patients, one with a schizophreniform and one with a schizoaffective disorder. Reports of frequent EEG abnormalities and the successful use of diazepam in catatonia prompted these trials with another anticonvulsant.

Catatonia: diagnosis and management.

The number of cases of catatonic schizophrenia has decreased in the past 50 years, but a few patients with the syndrome are still seen. The author describes the excited and withdrawn types of catatonia, reviews some organic and psychiatric conditions that resemble them, and gives suggestions for making the differential diagnosis. He discusses the use of the barbiturate interview, ECT, and phenothiazines in treatment, using case examples.

ECT for Catatonia in Schizencephaly.

Schizencephaly is a rare congenital brain malformation in which individuals have abnormal unilateral or bilateral clefts in the cerebral cortex. Medical comorbidities include hemiparesis, developmental delay, seizures, and psychiatric conditions such as bipolar disorder and schizophrenia. Here, we describe the case of a 51-year-old female with schizencephaly who developed catatonia and was successfully treated with electroconvulsive therapy (ECT). This case report is seemingly the first of its kind in detailing the use of ECT to treat catatonia in a patient with schizencephaly. Considerations
