Despite the widespread use of non-invasive imaging modalities, right heart catheterization remains the gold standard for the hemodynamic assessment of various cardiopulmonary disorders. A Swan-Ganz catheter allows the direct measurement of venous, intracardiac, and pulmonary arterial pressures, as well as cardiac output. Its execution and interpretation require technical precision, given the numerous potential sources of error. This review provides an overview of best practices, common pitfalls, and current clinical indications for right heart catheterization.

Background/Objectives:: Right ventricular (RV) pressure assessment is crucial in both the diagnosis and follow-up of patients with chronic thromboembolic pulmonary hypertension (CTEPH). While right heart catheterization (RHC) and pulmonary angiography (PA) are gold-standard invasive methods, transthoracic echocardiography (TTE) offers a safer and more accessible alternative. This study aimed to evaluate the reliability of echocardiographic RV pressure estimation compared to invasive techniques and to identify clinical predictors of response to balloon pulmonary angioplasty (BPA). Methods: In t

In patients with anasarca, the relative importance of cardiac, pulmonary and/or hepatic dysfunction is often difficult to determine. Conventional use of the Swan-Ganz catheter helps to separate the contributions of right and left heart disease, but it is seldom used to evaluate liver dysfunction. This report describes passage of a Swan-Ganz catheter into the hepatic vein prior to pulmonary artery placement in 11 patients. Hepatic vein catheterization permitted wedged hepatic venography using contrast media and measurement of the wedged and free hepatic venous pressures. All 11 patients had pul

A case of an accidental placement of the introducer sheath of a Swan Ganz catheter into the right lymphatic duct during right internal jugular vein catheterization is presented. This rare complication has to be reported, because the choice of the right internal jugular vein for central venous catheterization has been strongly recommended, with special concern being paid to the avoidance of any thoracic duct injury. No deleterious complications were observed either in the immediate or in the later postanaesthetic period.

Based on a review of the literature, hemodynamic signs and measurements in severe preeclampsia are described. Advantages and disadvantages of various hemodynamic methods and resulting indications for therapy are discussed. We found that in respect of hemodynamic monitoring in severe preeclampsia there are no indications for central venous pressure and conclude that at present there is no alternative to right heart catheterization (Swan-Ganz catheter).

Thirty-six hearts of patients who died with indwelling Swan-Ganz catheters in place were prospectively examined to assess the incidence and extent of localized lesions in the right side of the heart. Bland mural thrombosis in the superior vena cava, the right atrium and the pulmonary artery was found in 22 of 36 cases (61%). Patients with catheter periods greater than 2 days had a greater incidence and extent of bland mural thrombosis (79%) than patients with short-term catheterization (41%, p less than 0.01). Anticoagulation had no influence on bland mural thrombosis. Valvular hemorrhage occu

INTRODUCTION: The role of pulmonary artery catheterization (PAC) in cardiogenic shock (CS) remains debated: prior intensive care unit trials showed no benefit, but contemporary observational registries suggest improved outcomes. However, the overall evidence remains fragmented, with inconsistent phenotypic and outcome reporting. Our objective was to assess the association between PAC use and clinical outcomes in CS.
METHODS: We systematically searched MEDLINE, Cochrane, and Scopus (2015-2025) for observational and registry studies in adult CS. Outcomes included in-hospital/30-day mortality, m

A report is made on the experience gained in more than 600 cardiac catheterizations using the Swan-Ganz-Intracardiac floating catheter. This procedure is an addition to the diagnostic possibilities and provides answers to cardiological and pulmonological questioning which, so far, had not been an indication for cardiac catheterism. Its advantage is the safety of application and little inconvenience to the patient with a high information content for the physician. If the investigations are carried out under ergometer load, new aspects of functional diagnostics arise with respect to problems req

Right heart catheterization(RHC) is a technique of inserting a catheter into the right heart and pulmonary artery via a peripheral vein, to evaluate hemodynamics and oxygen dynamics. When Swan-Ganz catheter is used, it is called pulmonary artery catheterization (PAC). PAC plays an important role in the diagnosis and evaluation of pulmonary hypertension (PH), and hemodynamic assessment of critically ill patients. We developed this consensus to improve the overall quality of RHC operation, especially the PAC standardization in the diagnosis and evaluation of PH, thereby to promote the diagnosis

Pulmonary artery perforation is one of the most serious complications of pulmonary artery catheterization. We report a case of a pulmonary artery perforation which occurred during the placement of a Swan-Ganz catheter in a patient with mitral valve regurgitation. During the insertion of the catheter a deep negative pressure wave was seen in the pulmonary artery tracing, with concomitant haemoptysis. The postoperative chest radiograph revealed an infiltration distal to the tip of the pulmonary artery catheter. This type of unusual capillary wedge pressure tracing during pulmonary artery cathete

Doppler ultrasound was used to assess the response to serial drug testing in a patient with primary pulmonary hypertension. There was a close correlation between the pressure estimated by Doppler and the pulmonary arterial systolic pressure measured invasively (r = 0.98). Continuous-wave Doppler ultrasound, although not a substitute for initial Swan-Ganz catheterization, may obviate the need for repeat invasive procedures and permit closer monitoring of patients during treatment.

We describe an unusual complication during flotation of a pulmonary artery catheter through a preexisting percutaneous introducer sheath. A malfunctioning pulmonary artery catheter, which was placed through an introducer sheath in the right internal jugular vein, was removed. Attempts at repositioning a second pulmonary artery catheter met with resistance, and we were unable to either advance or withdraw it. Chest radiograph showed a bent introducer sheath going from the right internal jugular vein into the right subclavian vein and a pulmonary artery catheter loop. Under continuous fluoroscop

Purulent pericarditis is a serious but uncommon disorder which rarely complicates acute myocardial infarction. We have described a patient who had fatal purulent pericarditis subsequent to Swan-Ganz catheterization, which was done to facilitate the management of left ventricular failure complicating acute myocardial infarction. Although rare, purulent pericarditis should be considered in the differential diagnosis of otherwise unexplained sepsis associated with myocardial infarction. The presence of a pericardial effusion may rapidly be confirmed by echocardiography, and diagnostic pericardioc

One hundred eighty-nine Swan-Ganz catheters were inserted in 165 critically ill patients with different techniques. Percutaneous catheterization through the subclavian, internal jugular, and antecubital veins and antecubital venous cutdown with or without fluoroscopy were compared. The antecubital approach, percutaneously or through a venous cutdown, was associated with most complications. We conclude that the method of choice is the percutaneous insertion of the balloon-tipped catheter through the subclavian or internal jugular veins and that fluoroscopy is unnecessary.

Pulmonary artery catheterization is a useful tool for the diagnosis and management of lung or cardiac disease. This procedure is considered safe and associated with a low incidence of major complications. However, pulmonary artery rupture during right heart catheterization, albeit rare, remains a severe complication. Despite modern management with metal-coil embolization, selective intubation and deployment of bronchial blocker, the mortality rate may be as high as 50%. In this case, we report a new approach to deal with a Swan-Ganz-induced pulmonary artery rupture based on stent graft implant

A new technique for right heart catheterization using a Mullins' sheath is described. This device allows a Swan-Ganz catheter to reach pulmonary artery position easily and permits simultaneous pressure recordings in right heart chambers, thus avoiding a double venous puncture and two catheters. This new technique, its indications, and our experience in 29 patients are described. It is most useful in patients with severe pulmonary hypertension and in those conditions in which accurate right heart pressure measurements are needed.

A case of maternal pulmonary edema is presented as a late complication after intravenous administration of ritodrine in combination with dexamethasone for the suppression of premature uterine contraction and for the stimulation of fetal lung maturity. Swan-Ganz catheterization showed normal pulmonary capillary wedge pressures. Two-dimensional echocardiography disclosed good ventricular performance. The patient recovered very soon in 2 days post termination of pregnancy. Relative fluid overload might probably be responsible for the development of pulmonary edema in this case.

The pulmonary artery was catheterized using a Swan-Ganz catheter (7F) in 16 patients with adult respiratory distress syndrome. Hydrostatic pressure in the pulmonary capillaries was determined by analyzing changes in pulmonary artery pressure during its occlusion with a balloon and by the formula suggested by Gaar. The results have been compared. A high and significant correlation between the parameters obtained by the two above techniques has been established. The determination of pressure in the pulmonary capillaries makes it possible to calculate precapillary and postcapillary resistance.
