[Postoperative treatment of phantom pain and causalgias with calcitonin].

Results of IV calcitonin treatment in patients suffering from postoperative phantom limb pain (n = 12) or causalgia following peripheral nerve lesions (n = 4) are reported. All patients were complained of severe pain after a traumatic event or amputation, with disturbed sleep in many cases. After only 1-2 infusions 10 patients with phantom limb pain (83%) were discharged from hospital pain-free. Pain was effectively reduced by up to 5 infusions in 2 patients (17%). A follow-up for maximally 24 months showed a recurrence of pain in only 4 patients with obvious stump problems or reamputations. T

Phenoxybenzamine in the treatment of causalgia. Report of 40 cases.

Forty consecutive cases of causalgia treated during a 7-year period are presented. The patients ranged in age between 17 and 55 years, and all patients were males who received their nerve injuries from missile or shrapnel wounds. The greater occipital nerve was involved in two cases, median nerve in 10, sciatic nerve in 12, brachial plexus in seven, cauda equina in five, and multiple nerves in four cases. Each patient was treated with phenoxybenzamine, a postsynaptic alpha 1-blocker and presynaptic alpha 2-blocking agent. The drug was given orally in gradually increasing increments until a max

A personal overview of causalgia and other reflex dystrophies.

This is a personal assessment of true major causalgia and the other reflex dystrophies, related but distinctly separate entities. The clinical picture of causalgia differs only in minor respects from that described by Mitchell over 120 years ago. Its management has, however, been clarified, largely through the extensive experiences of World War II. It is readily recognized and can be treated effectively by sympathetic blocks or sympathectomy together with active exercise. The other reflex dystrophies are far less understood. They appear to have a similar pattern in their early phase and to res

Historical perspective of causalgia. Management of sympathetically maintained pain.

The plethora of terms included in the broad category of sympathetic dystrophies, and causalgia in particular, has made specific disorders, with unique clinical characteristics, very difficult to isolate into discrete clinical entities. Rather, the sympathetic dystrophies currently are regarded as existing along a continuum of varying severity and as having one basic pathophysiological mechanism, with considerable overlap of terms. The purposes of this article are 1) to review the theories of physiological mechanisms of causalgia and other forms of sympathetically maintained pain, 2) to describ

Peripheral nerve injury and causalgia secondary to routine venipuncture.

I examined 11 patients with upper-extremity causalgia secondary to peripheral nerve injury occurring during routine venipuncture. The nerves affected were the medial (n = 5) and lateral (n = 2) antebrachial cutaneous in the antecubital fossa, the superficial radial at the wrist (n = 2), and the dorsal sensory branches in the hand (n = 2). Anatomically, nerves lie on a plane just beneath and in close proximity to veins, making them vulnerable to injury during the procedure.

Sympathectomy for causalgia. Patient selection and long-term results.

Thirty-four sympathectomies were performed for causalgic pain. Overt extremity trauma was the precipitating event in only 26%. In 48%, nerve compression requiring surgical relief preceded the onset of the pain; most common lumbar disk surgery (37%). In the remainder (26%), miscellaneous vascular conditions contributed. Satisfactory immediate relief was obtained in 97% and 61% were completely relieved of pain initially. There were no deaths, 10% wound complication rate, and one instance of Horner's syndrome. Postsympathectomy neuralgia occurred in close to 40%, lasted a little over a month on t

The diagnosis and management of post-traumatic pain syndromes (causalgia).

There are two major categories of post-traumatic pain syndroms: (1) causalgia; and (2) mimocausalgia states or reflex sympathetic dystrophy. Vasoconstriction is usually present. Because of the pain, limitation of motion of the extremity occurs, and may result in permanent disability. There is often a great disparity between the apparent trauma and the severity of the pain. Sympathetic blocks and sympathectomy are definitive modes of therapy. In a series of 147 patients, 56% required surgical sympathectomy. The rest were treated by sympathetic blocks, physical therapy, and other medical measure

The rehabilitation of causalgia (complex regional pain syndrome-type II).

CRPS-type I or causalgia is a challenging pain syndrome and its pathogenesis remains controversial. Although its incidence is relatively low, the pain and suffering it causes can be severe and functionally debilitating. Early, accurate diagnosis permits initiation of appropriate therapeutic interventions and enhances the potential for successful treatment.

Nerve resection and re-location may relieve causalgia: a case report.

This article reports the relief of severe causalgia of the right infra-orbital nerve by nerve section and re-location in a 14-year-old boy who had worsening neuropathic pain (NP) and was housebound and refractory to all analgesics for 14 months. His infra-orbital nerve was sectioned and re-located into his buccal fat pad. Severe steady burning, electric shock-like pain and allodynia disappeared and he was able to return to school and an increasingly normal life at one year post-operatively and is pain-free at 3 years and 6 months of follow-up. With NP further deafferentation can cause a worsen

Complex regional pain syndrome (type I, RSD; type II, causalgia): controversies.

Introduction of the term complex regional pain syndromes (CRPS) as a replacement of the older terminology, reflex sympathetic dystrophy (RSD) and causalgia, has achieved two goals: it has focused attention on the diagnosis and treatment, and sent basic scientists back to their laboratories. The relation of sympathetically maintained pain and sympatholysis is examined, particularly as a neuropathic process that is found in many conditions, including CRPS. This review also focuses on recent observations proposing a pathologic basis in support of diagnosis and treatment of these disorders.

Comparison of sympathetic neurons in orofacial and upper extremity nerves: implications for causalgia.

This study compared nerves of the orofacial region with nerves frequently associated with causalgia to determine if there is a significant difference in the proportion of sympathetic neurons within these nerves, which may account for the lower incidence of orofacial causalgia. Three orofacial and two upper extremity nerves were examined. Each nerve was transected and labeled with horseradish peroxidase to identify the cell bodies of neurons contributing axons to the nerve. The study included two trials per nerve, for a total of 10 trials in eight cats. The trigeminal and dorsal root ganglia, c

Do nerve resection, amputation, and immunotherapy relieve causalgia in the long term?

Reported here is the unpublished long follow-up (22 years and 14 years) of 2 patients with longstanding causalgia initially successfully treated by nerve resection and reported in detail previously in this journal. There are very little such long-term data for causalgia of this nature available. This is a rare opportunity to reevaluate these 2 patients who began their stories not too differently but went on to follow very different paths. One of these patients (CH) had causalgia after a traumatic right infraorbital nerve injury. This pain was relieved by nerve resection with no recurrence of f

Sympathetic maintained pain (causalgia) associated with a demonstrable peripheral-nerve lesion. Operative treatment.

Nine patients who had sympathetic maintained pain (causalgia) and a total of ten identifiable lesions involving peripheral nerves were managed with a continuous sympathetic block; repair, reconstruction, or lysis of the involved nerve, or a combination of these procedures; and rotation of a muscle flap over the nerve in an attempt to enhance the blood supply in the area and to reduce scarring in the region surrounding the nerve. The lesions were located in the median nerve at the wrist in five of the patients; in both the ulnar nerve at the elbow and the median nerve at the wrist in one; and i

Abnormal and collateral innervations of sympathetic and peripheral sensory fields associated with a case of causalgia.

A 41-year-old female developed spontaneous burning pain (causalgia) and stimulus-induced dysesthesia (allodynia) of the dorso-lateral part of her right foot following trauma. An L3 and L4 sympathectomy eliminated the spontaneous burning pain for only 1 year, but did not affect the stimulus-induced dysesthesia. We evaluated her two years post-sympathectomy with grouped sequential anesthetic blocks and sensory testing. Sympathetic blocks at L1 and L2 eliminated the burning pain and normalized heat perception from baseline hyperalgesia, indicating that the causalgia had been reactivated via more

Surgical relief of causalgia with an artificial nerve guide tube: Successful surgical treatment of causalgia (Complex Regional Pain Syndrome Type II) by in situ tissue engineering with a polyglycolic acid-collagen tube.

Two patients with causalgia associated with allodynia and finger contracture were treated surgically with a bioresorbable nerve guide tube made from polygycolic acid and collagen: the injured segment of the digital nerve was resected and the resulting gap (25 and 36mm) was bridged with the tube. In both cases, a neuroma was found on the injured nerve and many sprouting branches were. After reconstruction, the causalgia and allodynia disappeared and movement of the fingers recovered during the following 6 months. Functional recovery was objectively identified for 1 year and 9 months. Both patie

Causalgia: A Review of Nerve Resection, Amputation, Immunotherapy, and Amputated Limb CRPS II Pathology.

BACKGROUND: Causalgia and complex regional pain syndrome (CRPS) type II with nerve injury can be difficult to treat. Surgical peripheral nerve denervation for causalgia has been largely abandoned by pain clinicians because of a perception that this may aggravate a central component (anesthesia dolorosa). METHODS: We selectively searched Pubmed, Cochrane, MEDLINE, EMBASE, CINAHL Plus, and Scopus from 1947 for articles, books, and book chapters for evidence of surgical treatments (nerve resection and amputation) and treatment related to autoimmunity and immune deficiency with CRPS. RESULTS: Re

Infraorbital Nerve Decompression for Infraorbital Neuralgia/Causalgia following Blowout Orbital Fractures: A Case Series.

The purpose of this study was to present the management of a series of patients referred with infraorbital nerve paraesthesia that developed after insignificant orbital floor fracture without diplopia or exophthalmos, and that did not require initial surgical repair. This is a retrospective interventional case series. The main outcome and measures were assessment of preoperative symptoms including neuralgia and sensory symptoms; review of periorbital computed tomography (CT) scans; and assessment of postoperative effects of surgery for infraorbital nerve decompression. Nine patients were ident
