[Acute appendicitis after colonoscopy: causality or coincidence?].

Complications due to diagnostic colonoscopy are very rare. Perforation is the most frequent colon-related event. So far only ten cases of acute appendicitis after colonoscopy have been reported in the literature. We present the case of a 79-year-old man who developed acute appendicitis after diagnostic colonoscopy. Diagnosis was aggravated by the fact that in the patient's history an appendectomy had been performed 65 years earlier. Because of the rarity of this event it is doubtful whether the endoscopic examination was causal or coincidental. However, in cases of pain after colonoscopy, appe

Generative formalism of causality quantifiers for processes.

The concept of dynamical causal effect (DCE) is generalized and equipped with a formalism which allows one to formulate in a unified manner and interrelate a variety of causality quantifiers used in time series analysis. An elementary DCE from a subsystem Y to a subsystem X is defined within the stochastic dynamical systems framework as a response of a future X state to an appropriate variation of an initial (X,Y)-state distribution or a certain parameter of Y or of the coupling element Y→X; this response is quantified in a probabilistic sense via a certain distinction functional; elementary D

Hypothyroidism and Hyponatremia: Rather Coincidence Than Causality.

BACKGROUND: Hypothyroidism is referred to be a rare but possible cause of hyponatremia. However, there is only poor evidence supporting this association. Since hyponatremia and hypothyroidism are both common conditions themselves, co-occurrence does not have to be causal. METHODS: To address a potential relationship, a retrospective analysis of data from the Division of Endocrinology of the Medical University of Vienna from April 2004 to February 2016 was performed. A total of 8053 hypothyroid patients (48 ± 18 years of age; 71% female) with thyrotropin >4.0 μIU/mL and available blood tests f

Canonical Granger causality between regions of interest.

Estimating and modeling functional connectivity in the brain is a challenging problem with potential applications in the understanding of brain organization and various neurological and neuropsychological conditions. An important objective in connectivity analysis is to determine the connections between regions of interest in the brain. However, traditional functional connectivity analyses have frequently focused on modeling interactions between time series recordings at individual sensors, voxels, or vertices despite the fact that a single region of interest will often include multiple such r

Fabry's disease and psychosis: causality or coincidence?

A 21-year-old female with Fabry's disease (FD) presented acute psychotic symptoms such as delusions, auditory hallucinations and formal thought disorders. Since the age of 14, she had suffered from various psychiatric symptoms increasing in frequency and intensity. We considered the differential diagnoses of prodromal symptoms of schizophrenia and organic schizophrenia-like disorder. Routine examinations including cognitive testing, electroencephalography and structural magnetic resonance imaging revealed no pathological findings. Additional structural and functional imaging demonstrated a min

Neuroleptic malignant syndrome: mechanisms, interactions, and causality.

This review focuses on new data from recent publications concerning how compounding interactions between different thermoregulatory pathways influence the development of hyperthermia and/or neuroleptic malignant syndrome (NMS), and the fundamental issue of the presumed causal role of antipsychotic drugs. The formal criteria for substantiating cause-effect relationships in medical science, established by Hill, are applied to NMS and, for comparison, also to malignant hyperthermia and serotonin toxicity. The risk of morbidities related to hyperthermia is reviewed from human and experimental data

Pentosan polysulfate maculopathy: clinical considerations, pathobiology, and causality.

Pentosan polysulfate (PPS) maculopathy is a progressive, vision-threatening retinal disorder linked to prolonged use of PPS, a heparin-like sulfated polysaccharide prescribed for interstitial cystitis/bladder pain syndrome. Affected individuals often experience impaired dark adaptation and progressive central vision loss. Fundus imaging commonly reveals hyperpigmented macular clumps at the level of the retinal pigment epithelium (RPE), and a distinctive pattern of autofluorescence abnormality in the posterior fundus. This degenerative maculopathy may continue to progress even after drug cessat

Bidirectional causality of physical exercise in retinal neuroprotection.

Physical exercise is recognized as an effective intervention to improve mood, physical performance, and general well-being. It achieves these benefits through cellular and molecular mechanisms that promote the release of neuroprotective factors. Interestingly, reduced levels of physical exercise have been implicated in several central nervous system diseases, including ocular disorders. Emerging evidence has suggested that physical exercise levels are significantly lower in individuals with ocular diseases such as glaucoma, age-related macular degeneration, retinitis pigmentosa, and diabetic r

Ventricular Tachycardia Burden and Mortality: Association or Causality?

Ventricular tachycardia (VT) is a potentially fatal cardiac rhythm disorder. Implantable cardioverter defibrillators (ICDs) are the primary management strategy for VT and have been shown to reduce the incidence of death but, ICDs do not reduce VT recurrences. Further, mounting evidence indicates that high VT burden, defined as the cumulative number of recurrent VTs or ICD shocks, is associated with an elevated risk of death; however, it is unclear if high VT burden is a cause of death or a marker of severe heart disease. Proposed mechanisms for a causal pathway suggest that multiple VT episode

Two-stage hemoglobin prediction based on prior causality.

INTRODUCTION: Perioperative hemoglobin (Hb) levels can influence tissue metabolism. For clinical physicians, precise Hb concentration greatly contributes to intraoperative blood transfusion. The reduction in Hb during an operation weakens blood's oxygen-carrying capacity and poses threats to multiple systems and organs of the whole body. Patients can die from perioperative anemia. Thus, a timely and accurate non-invasive prediction for patients' Hb content is of enormous significance. METHOD: In this study, targeted toward the palpebral conjunctiva images in perioperative patients, a non-inva

Air flight barotrauma and meningitis: causality seems real.

BACKGROUND: Barotrauma to the middle-ear cavity and paranasal sinuses is a relatively common flight-related health problem. Occasionally, it may result in severe mechanical or infectious intracranial complications; these have been rarely reported to date. OBJECTIVE: Four cases of acute bacterial meningitis following air travel are presented, and its pathogenesis is briefly described. CONCLUSION: Neurological symptoms occurring after air flight should prompt proper investigation. Otitis media and sinusitis are common primary focuses of bacterial meningitis. Severe complications of air flight

Difficult Causality Relationship between Colorectal Cancer and Schistosomiasis.

The purpose of the study was to determine the nature of the association between colorectal cancer and schistosomiasis infection. This was a retrospective and descriptive study of colorectal cancers and to identify cases associated with schistosomiasis. A total of 814 cases of rectal colorectal cancer were collected and 3 cases were associated with schistosomiasis. These are 2 cases of Liberkunian adenocarcinoma and one case of malt lymphoma. The patients were all farmers and from rural areas. A co-infection with HIV was found in his 3 patients. Our data show an extreme rarity of the associatio

Eosinophilic esophagitis and esophageal atresia: coincidence or causality?

Eosinophilic esophagitis is an immune-mediated chronic disease of the esophagus characterized by symptoms related to esophageal dysfunction and tissue eosinophilia. In the endoscopy, the esophageal mucosa may appear normal or show exudates, rings, edema, furrows, and strictures. Its management is based on elimination diet, topical corticosteroids and/or esophageal dilation. Atresia is the most common congenital alteration of the esophagus; it requires surgical repair and poses potential complications, such as gastroesophageal reflux, strictures, and esophageal dysmotility. Up to 2015, 48 cases

Reflux and Voice Disorders: Have We Established Causality?

A trend of attributing abnormal voice changes to reflux has gained momentum among medical professionals over the last few decades. Evidence supporting the connection between reflux and voice and the use of anti-reflux medication in patients with dysphonia is conflicting and deserves careful examination. In the current health care environment, it is important that medical decisions be based on science rather than anecdote and practice patterns. The goal of this review is to investigate the evidence linking reflux and voice changes. Specifically, this association will be examined in the context

Science, rhetoric and the causality of adverse events.

This paper outlines aspects of the interface between law and science in the domain of treatment induced injury, using examples drawn from litigation on SSRIs. In the face of claims that randomized controlled trials provide uniquely appropriate evidence and that there should be a statistically significant doubling of the risks on treatment, the examples used demonstrate that the methods of assessing the possibility of causal links between treatment and injuries must necessarily be tailored to both the injury and the treatment.

Viruses and human cancer: from detection to causality.

The study of cancer is incomplete without taking into consideration of tumorigenic viruses. Initially, searches for human cancer viruses were fruitless despite an expansion of our knowledge in the same period concerning acute-transforming retroviruses in animals. However, over the last 40 years, we have witnessed rapid progress in the tumor virology field. Currently, acknowledged human cancer viruses include Epstein-Barr virus, hepatitis B virus, hepatitis C virus, high-risk human papilloma viruses, human T-cell lymphotropic virus type 1 and Kaposi's sarcoma-associated herpesvirus. Extensive e
