Significance of proximal margin involvement in low-grade appendiceal mucinous neoplasms.

CONTEXT: Appendiceal adenomas and low-grade appendiceal mucinous neoplasms (LAMNs) confined to the appendix are cured by appendectomy. However, involvement of the proximal margin raises concern for residual disease. Some patients with a positive margin at appendectomy undergo cecal resection to eliminate a perceived risk for tumor recurrence or dissemination, although that likelihood is assumed rather than demonstrated. OBJECTIVE: To determine whether involvement of the proximal appendiceal resection margin by adenoma or LAMN is a risk factor for local development of recurrence or pseudomyxom

Intussusception in the Setting of Tuberous Sclerosis Complex.

A 53-year-old man with tuberous sclerosis complex presented with severe, acute, right lower quadrant pain. The patient was found to have ileocolonic intussusception and a cecal mass on imaging. Diagnostic colonoscopy revealed a 3-cm cecal lesion which was grossly equivocal for hamartoma, adenoma, or other neoplasm, biopsies of which revealed villous adenoma. Given the lesion was believed to represent the trigger point for intussusception, the patient was referred for surgical resection, the histopathology from which the lesion was identified to be invasive adenocarcinoma. The patient underwent

Further observations on spontaneous neoplasms in the Mongolian gerbil, Meriones unguiculatus.

One hundred sixty Mongolian gerbils, Meriones unguiculatus, were examined for the presence of naturally occurring lesions. The first recognized cases of cecal adenocarcinoma, testicular teratoma, and sebaceous gland pad carcinoma were found. Neoplasms previously reported from the gerbil and also seen in the present study included ovarian theca lutein and granulosa lutein cell tumors, sebaceous gland pad adenoma, cutaneous squamous cell carcinoma, malignant melanoma, and renal and splenic hemangiomas. Calcinosis cutis was also observed in two male gerbils.

Primary dermoid cysts of the cecum: A systematic review and case report.

RATIONALE: Cecal dermoid cysts, mature cystic teratomas, are exceedingly rare. They may present with nonspecific abdominal pain and mimic common surgical conditions. We report a pediatric case of cecal dermoid cyst presenting with acute appendicitis and provide a systematic review of all published cecal dermoid cases. PATIENT CONCERNS: A 14-year-old female presented with 1 day of right lower abdominal pain, nausea, vomiting and anorexia; she had prior vague right-sided pain with an ultrasound 5 years earlier that identified a right iliac fossa cyst, which was not followed up. DIAGNOSES: Cont

Diagnostic limitations of 10 mm thickness single-slice computed tomography for patients with suspected appendicitis.

PURPOSE: The aim of this retrospective analysis was to evaluate the accuracy of 10 mm thickness single helical computed tomography (CT) examination for confirming the diagnosis of appendicitis or providing a diagnosis other than appendicitis, including underlying periappendical neoplasms. MATERIALS AND METHODS: From April 1, 2001 to March 30, 2005, a total of 272 patients with suspected appendicitis underwent CT examinations. Of the 272 patients, 106 (39%) underwent surgery. Seven CT examinations for seven patients were excluded because of inconsistency of the CT protocol. We therefore review

[Inflammatory tumors of the cecum in emergencies: surgical considerations and case report].

A cecal mass of uncertain etiology encountered during surgery for presumed appendicitis is a dilemma for the surgeon. The differential diagnosis of an unsuspected ileocecal mass must include neoplasm, diverticular disease, inflammatory bowel disease and severe appendicitis involving the ileocecal region. Right hemicolectomy is the gold standard for the treatment of the unsuspected ileocecal mass. The Authors report a case of inflammatory tumour of caecum secondary to appendicitis in a young adult.

Primary Basidiobolomycosis of the Cecum in Immunocompetent Patients: Two Case Reports.

Colonic basidiobolomycosis is a rare fungal infection caused by Basidiobolus ranarum. Primary cecal basidiobolomycosis is an exceptionally rare condition. The study describes two cases of primary basidiobolomycosis of the cecum in immunocompetent male and female patients (one each). The patients presented with fever, abdominal pain, weight loss, eosinophilia, and high erythrocyte sedimentation rates. Computed tomography revealed wall thickening and mass lesions involving the cecum, suggesting malignancy. Right hemicolectomies were performed to relieve the intestinal obstruction. On microscopy,

Benign multicystic mesothelioma of peritoneum complicating acute appendicitis in a man: a case report.

BACKGROUND: Benign multicystic mesothelioma is a rare pathology. Few cases are reported in the medical literature and acute presentation is extremely uncommon. CASE PRESENTATION: We describe an acute clinical presentation of the neoplasm that revealed itself with signs and symptoms attributable to acute appendicitis in a 41-year-old white man. Abdominal echography and computed tomography scans demonstrated the presence of a mass in direct contiguity with cecal fundus, but diagnosis remained unclear. Our patient underwent surgery and complete removal of the neoplasm. Only a definitive histolog

Cytomorphology of metastatic colonic MXD4::NUTM1-rearranged sarcoma.

This report describes the cytologic features of a recently described MXD4::NUTM1-rearranged colonic sarcoma metastatic to the midclavicular soft tissue. Thirteen years ago, a 65-year-old woman presented with a cecal mass and subsequent liver mass. The cecal mass was diagnosed as malignant undifferentiated spindled and epithelioid neoplasm based on morphology and lack of tumor immunoreactivity with a panel of epithelial, smooth muscle, skeletal, melanoma, hematologic, and GIST markers. The liver mass showed morphologic and immunophenotypic similarity to the epithelioid component of the patient'

Spatial clustering of multiple hyperplastic, adenomatous, and malignant colonic polyps in individual patients.

Analysis of relative polyp locations in 426 consecutive patients with multiple colonic polyps found on colonoscopy showed novel findings. First, synchronous and metachronous neoplastic polyps showed spatial clustering in individual patients. For example, patients with their largest neoplasm in the cecum or proximal ascending colon, had 34.3 percent +/- 4.6 percent (standard error) of their other colonic neoplasms in the same location. Second, hyperplastic polyps showed spatial clustering in individuals that was statistically significantly greater than expected from the increased hyperplastic p

Low-grade mucinous neoplasia in a cecal diverticulum: A case report.

INTRODUCTION: Low-grade mucinous neoplasia is an uncommon benign tumor that develops in the appendix. The development of mucocele disease has never been reported in a colonic diverticulum. We present a case developing low-grade mucinous neoplasia in a cecal diverticulum. PRESENTATION OF CASE: A tumor in the ileocecal region was found during a medical examination of a 66-year-old woman. Three months later, the tumor was still present and the patient developed abdominal pain. Laparoscopic ileocecal resection with D2 lymph node dissection was performed. Histopathological examination revealed a l

Cecal schwannoma: A case report and literature review.

Schwannomas are slow-growing tumors originating from Schwann cells and are rarely found in the gastrointestinal tract, particularly in the cecum. We report the case of a 59-year-old man who had recurrent diarrhea and was found to have a mass in the cecum during endoscopic examination. Contrast-enhanced abdominal computed tomography revealed a slightly hypodense, homogeneous mass in the cecum with moderate enhancement. The tumor was laparoscopically resected. Histopathological examination indicated spindle-shaped tumor cells arranged in a fascicular or palisading pattern, with minimal cellular

Appendiceal mucinous neoplasm in adolescence: Diagnosis, management, and surveillance.

This case report describes a 17-year-old patient with a low-grade appendiceal mucinous neoplasm. The patient presented with non-bloody diarrhea, abdominal pain, and weight loss. A colonoscopy revealed a cecal polypoid mass that required laparoscopic surgery. The residual appendix was dilated with myxoglobulosis and histopathology confirmed the diagnosis of a low-grade appendiceal mucinous neoplasm staged pT3Nx. The potential risk of pseudomyxoma peritonei is a serious complication of these tumors. Surveillance plans include computed tomography abdomen and pelvis, and tumor markers every 6 mont

Cecal Adenoma Presenting as a Large Bowel Intussusception in an Adult.

An adult intussusception is a relatively rare entity and is more commonly confined to the small bowel when it is found. The majority of the colonic intussusceptions in adults are caused by malignant neoplasms. Here we present the case of a 65-year-old female with a cecal adenoma as the lead point causing intussusception all the way to the splenic flexure. Controversy still exists regarding optimal treatment strategies, specifically the question of if it is safe or not to perform preoperative reduction prior to surgical resection in adult large bowel intussusceptions.

Bilio-Cecal Stent Migration Presenting as Massive Rectal Bleeding.

Endoscopic biliary stenting is a well-established intervention for the treatment of biliary, hepatic, and pancreatic disorders. The common indications include strictures, neoplasms, stones, infections, and bile leaks. Stents can be occluded, predisposing patients to ascending cholangitis and biliary sepsis. Distal stent migration is another known complication of endoscopic stenting and is usually spontaneous. Bowel perforation, abscesses, bleeding, and pancreatitis are rare complications of distal stent migration and are usually limited to the duodenum. Herein, we describe an extremely rare ca

CT in the diagnosis of enterovesical fistulae.

Enterovesical fistulae are difficult to demonstrate by conventional radiographic methods. Computed tomography (CT), a sensitive, noninvasive method of documenting the presence of such fistulae, is unique in its ability to outline the extravesical component of the primary disease process. Twenty enterovesical fistulae identified by CT were caused by diverticulitis (nine), carcinoma of the rectosigmoid (two), Crohn disease (three), gynecologic tumors (two), bladder cancer (one), cecal carcinoma (one), prostatic neoplasia (one), and appendiceal abscess (one). The CT findings included intravesical

The use of silicone expander and cecal transposition after pelvic exenteration.

AIMS: Describe a new approach for pelvic floor treatment employing a temporary mechanical support device with silicone expander, with or without association to cecal transposition. METHODS: From January 2000 to June 2006, 106 patients were submitted to pelvic exenteration. A retrospective evaluation was done of the last 30 patients previously submitted to total pelvic exenteration without neither urinary nor faecal sphincter preservation who latter were submitted to a pelvic floor treatment with silicone expander with or without association to cecal rotation. RESULTS: Twenty-six patients wer

Collision tumor of the appendix with concomitant cecal adenocarcinoma: A case report and literature review.

INTRODUCTION: Collision tumors of the appendix are rare tumors consisting of two distinct pathologies arising from different cell lines simultaneously. The most common type is the coexistence of a neuroendocrine tumor (NET) with a low-grade appendiceal mucinous neoplasm (LAMN). We report a unique case of appendiceal collision tumor with synchronous cecal intra-mucosal carcinoma and pulmonary sarcoidosis. CASE PRESENTATION: A 57-year-old man presented to the clinic with vague abdominal pain. Upon investigation, a colonoscopy showed a large cecal mass, and a computed tomography scan showed a po

A neoplasm of globule leukocytes in a cat.

A neoplasm involving the ileo-cecal-colic junction, thymus, and tracheobronchial lymph nodes of a 7-year-old domestic cat was composed of dense sheets of round to oval mononuclear cells with oval to indented nuclei, moderate amounts of cytoplasm, and variable numbers of round eosinophilic granules. These granules are brown to black in phosphotungstic acid hematoxylin-stained sections and stain variably with the periodic acid-Schiff stain. They are 0.8 to 1.5 micron in diameter, limited by a single unit membrane, and have variable electron density. Light microscopic cellular morphology and stai

Cecal Leiomyosarcoma Management: A Case Report and Review of Literature.

Leiomyosarcomas are very rare soft tissue sarcomas originating from smooth muscle cells. This neoplasm can develop in various places around the body, including but not limited to the uterus, retroperitoneum, colon, blood vessels, and bladder. This case report details the discovery of a cecal leiomyosarcoma in a patient who presented to the emergency department after a year of abdominal pain, fatigue, and unexpected weight loss. The report will discuss the available literature on leiomyosarcoma, the surgical approach for management in this patient, and post-operative management.
