Management update of acute bacterial rhinosinusitis and the use of cefdinir.

The pathogenesis, bacteriology, diagnosis, and antimicrobial treatment of acute bacterial rhinosinusitis (ABRS) is reviewed. Most cases of ABRS arise as complications of the rhinosinusitis of colds and other acute viral respiratory infections. Nose blowing during colds may be a risk factor for ABRS by propelling bacteria-laden nasal fluid into the sinus cavity. The bacterial causes of ABRS continue to be S pneumoniae. H influenzae, other streptococcal species, M catarrhalis, anaerobes, and S aureus. Clinical diagnosis of ABRS is based on obtaining a history of a cold or influenza-like illness

Cefdinir reprograms Gram-positive bacteria to synergize with lysozyme against superbugs.

Multidrug-resistant (MDR) bacteria pose a critical global health threat, urgently requiring solutions. Cefdinir, the highest-selling third-generation cephalosporin but one now facing clinical obsolescence due to escalating resistance, is conventionally classified as a nascent cell wall synthesis inhibitor. This study challenges this understanding by demonstrating that cefdinir renders existing cell walls negatively charged, thereby synergizing with ubiquitous lysozyme and simultaneously restoring both agents' efficacy against a broad spectrum of MDR Gram-positive superbugs without driving resi

Recurrent Pyelonephritis in Quadriplegia: Rethinking Oral Cefdinir in the Presence of Nephrolithiasis.

Patients with cerebral palsy who develop functional quadriplegia are at an increased risk for recurrent urinary tract infections and kidney stones. Impaired bladder emptying, chronic constipation, and reduced physical mobility collectively promote urinary stasis and bacterial growth. Despite the frequency of these complications, there is little guidance in the currently available literature on how to select appropriate empiric oral antibiotics for pyelonephritis in this patient population, especially when renal stones are also present. This case highlights the importance of careful antibiotic

Comparison of Cefdinir and Cephalexin as Step-Down Therapy in Pyelonephritis or Urosepsis.

Background: Cephalosporins are increasingly used to treat severe urinary tract infections (sUTI) due to rising resistance with standard antibioics. When switching from intravenous to oral cephalosporins there is limited clinical outcome data to guide selection. Objective: To compare the failure rate between cephalexin and cefdinir when used as oral step-down therapy in sUTI. Methods: In this retrospective study, we assessed patients admitted to 3 hospitals for sUTI over a 1-year period. Cases were identified using ICD-10 codes N10, N39.0, and A41.9. Patients who received intravenous antibiotic

Efficacy of penicillin versus cefdinir in eradication of group A streptococci and tonsillar flora.

Core tonsillar cultures were obtained from 40 children with recurrent tonsillitis treated with either penicillin or cefdinir. Group A beta-hemolytic streptococci were isolated from 11 penicillin- and 3 cefdinir-treated (P < 0.001) patients. beta-Lactamase producers were recovered from 17 penicillin- and 3 cefdinir-treated (P < 0.01) patients. Inhibiting alpha-hemolytic streptococci were isolated less often from penicillin-treated patients than from cefdinir-treated patients.

Cefdinir-Ornidazole for Bowel Preparation Before Colorectal Surgery: A Retrospective Cohort Study.

INTRODUCTION: Mechanical bowel preparation (MBP) combined with oral antibiotics (OAB) before colorectal surgery was significantly associated with reduced surgical site infection (SSI) rates. However, aminoglycoside oral preparations are not available in Chinese medical institutions; it is necessary to find an alternative. This study aimed to investigate whether MBP combined with oral cefdinir and ornidazole was associated with lower SSI rates. METHODS: This retrospective cohort study included patients who underwent elective colorectal surgery between June 2019 and May 2021. Patients were stra

Cefdinir vs cephalexin for the treatment of urinary tract infections: A retrospective evaluation.

PURPOSE: Cefdinir and cephalexin are cephalosporin antibiotics commonly used in the treatment of urinary tract infections (UTIs). Their efficacy depends on achieving sufficient time with concentrations exceeding the minimum inhibitory concentration (MIC). Despite being frequently prescribed for UTIs, cefdinir has markedly lower urine penetration compared to cephalexin. It is possible that differences in pharmacokinetics could result in dissimilar efficacy between these agents; however, comparative studies of cephalosporins in UTIs are lacking. METHODS: This was a retrospective comparative stu

Cefdinir: a review of its use in the management of mild-to-moderate bacterial infections.

Cefdinir (Omnicef) is an oral third-generation cephalosporin with good in vitro activity against many pathogens commonly causative in community-acquired infections. The drug provides good coverage against Haemophilus influenzae, Moraxella catarrhalis and penicillin-susceptible Streptococcus pneumoniae, the most common respiratory tract pathogens. Cefdinir is stable to hydrolysis by commonly occurring plasmid-mediated beta-lactamases and retains good activity against beta-lactamase-producing strains of H. influenzae and M. catarrhalis. The drug distributes into various tissues (e.g. sinus and t

Cefdinir activity against contemporary North American isolates from community-acquired urinary tract infections.

Cefdinir is an oral cephalosporin approved by the US Food and Drug Administration in 1997 for the treatment of community-acquired (CA) respiratory tract and uncomplicated skin and soft tissue infections. The objective of the present study was to evaluate the in vitro activity of cefdinir against recent clinical isolates collected from CA-urinary tract infections (UTIs), a possible expanded indication. A total of 456 isolates from CA-UTI were collected from medical centres in North America (NA; United States and Canada) in 2003 and susceptibility tested by NCCLS reference broth microdilution me

Successful steroid therapy for cefdinir-induced acute tubulointerstitial nephritis with progressive renal failure.

A 58-year-old woman was admitted to our hospital because of renal dysfunction that continued to progress even after withdrawal of cefdinir, the presumed cause of acute renal failure. Renal histologic findings included interstitial fibrosis accompanied by moderate lymphocytic infiltration, and tubular atrophy with reduced numbers of epithelial cells. Mesangial cells and glomerular basement membranes were nearly normal. Scintigraphy with 67gallium disclosed diffuse abnormal accumulation in both kidneys. A lymphocyte stimulation test with cefdinir was positive. The patient was diagnosed with acut
