LocaliType: Development of locality-based social determinants of health archetypes using census tract data from the UCSF Health Atlas.

Locality archetypes (denoted as LocaliTypes) - neighborhood, district, or nearby region patterns shaped by multiple social determinants of health (SDOH) - offer a novel approach to understanding place-based inequalities while preserving data confidentiality in the era of big data. Using 2020 U.S. Census shaped census tract-level data from the UCSF Health Atlas, we developed LocaliTypes and conducted exploratory validation analyses. We grouped 107 variables spanning race/ethnicity, socioeconomic conditions, environmental exposures, and disaster risk into five domains using large language model-

Census Tract-Level Social Vulnerability and Gonorrhea Rates in Central Ohio: Results From the STD Surveillance Network, 2020-2021.

BACKGROUND: Gonorrhea remains a significant burden in central Ohio, particularly in socially disadvantaged communities. This study evaluates gonorrhea case rates and changes from 2020 to 2021, focusing on geographic patterns associated with social vulnerability. METHODS: We analyzed gonorrhea case data from the Columbus STD Surveillance Network for 2020 and 2021. Cases were linked to census tracts and categorized into low (0-0.66) or high (0.67-1.0) vulnerability groups based on the Centers for Disease Control and Prevention/Agency for Toxic Substances and Disease Registry Social Vulnerabilit

The Relationship Between Zoning Policies, Racialized Economic Segregation and Life Expectancy Across Census Tracts in Chicago, 2010-2015.

INTRODUCTION: Racial segregation is a recognized cause of racial health disparities. One factor that has contributed to racial segregation is municipal zoning policies that exclude people of color and those of low income from advantaged residential neighborhoods. Few studies have explicitly explored the relationship between zoning policies and health. Using data on zoning policies in Chicago in 1923 and 2012, we examined inequities in life expectancy across census tracts in relation to the proportion of land in each tract zoned for specific purposes. METHODS: For each Chicago census tract, we

Geographic disparities in access to Medication for Opioid Use Disorder across US census tracts based on treatment utilization behavior.

Drug overdose is the leading cause of accidental death in the U.S. with deaths from opioid overdose occurring at a higher rate in rural areas. The gaps in the provision of healthcare services have been exacerbated by the opioid crisis leaving vulnerable populations without access to preventative care and education, harm reduction, both chronic and acute treatment of the symptoms of opioid use disorder (OUD), and long-term psychological support for those with OUD and their families. There has been a call in the literature -and a federal mandate-for increased access to opioid treatment facilitie

Selective screening of hospitalized patients for the presence of asymptomatic hepatitis B virus carriage: predictability based on census tract data.

Review of all hepatitis B surface antigen-positive patients reported to the county health department over the past 3 years showed that 60% of patients had been diagnosed at our hospital, which serves an inner city, predominantly black, indigent population. Sera from 524 adult patients admitted to the hospital were prospectively collected and tested for hepatitis B surface antigen, eight (1.5%) of which were antigen-positive. Census tract data allowed us to perform selective screening of sera from 95 other patients who were subsequently hospitalized and deemed "high risk" for hepatitis B infect

Relationship Between Census Tract-Level Poverty and Domestically Acquired Salmonella Incidence: Analysis of Foodborne Diseases Active Surveillance Network Data, 2010-2016.

BACKGROUND: The relationships between socioeconomic status and domestically acquired salmonellosis and leading Salmonella serotypes are poorly understood. METHODS: We analyzed surveillance data from laboratory-confirmed cases of salmonellosis from 2010-2016 for all 10 Foodborne Disease Active Surveillance Network (FoodNet) sites, having a catchment population of 47.9 million. Case residential data were geocoded, linked to census tract poverty level, and then categorized into 4 groups according to census tract poverty level. Patient Portal Activation Disparities by Ward, Census Tract, and Zip Code in an Urban Landscape Among Neurology Patients: Cross-Sectional Multi-Scale Analysis.

BACKGROUND: Patient portals are essential infrastructure, reinforced by the 21st Century Cures Act, yet adoption remains inequitable. The COVID-19 pandemic accelerated portal adoption as telehealth and remote result delivery made electronic access integral to care, but racial and ethnic disparities persisted. Understanding activation determinants is critical for addressing digital health disparities, particularly among neurology patients, for whom cognitive, speech, and mobility impairments can complicate portal use. OBJECTIVE: We examined the demographic, geographic, and neighborhood-level f

Analyzing pedestrian fatalities in the United States: A comparative study of pedestrian and census tract level factors across pre-, during, and late-pandemic periods.

INTRODUCTION: The increasing number of pedestrian fatalities has emerged as a significant public health concern, a situation that has been exacerbated by the disruptions resulting from the COVID-19 pandemic. While existing literature provides insights into general trends and factors affecting pedestrian safety, a gap remains in understanding how these dynamics shifted during the pandemic and late-pandemic periods. This study aims to conduct a comprehensive assessment of the factors that have differential impacts on pedestrian fatalities across three periods: pre-pandemic (2018-2019), pandemic

A census tract-level examination of social determinants of health among black/African American men with diagnosed HIV infection, 2005-2009--17 US areas.

BACKGROUND: HIV disproportionately affects black men in the United States: most diagnoses are for black gay, bisexual, and other men who have sex with men (collectively referred to as MSM). A better understanding of the social conditions in which black men live and work may better explain why HIV incidence and diagnosis rates are higher than expected in this population. METHODS: Using data from the National HIV Surveillance System and the US Census Bureau's American Community Survey, we examined the relationships of HIV diagnosis rates and 5 census tract-level social determinants of health va

The obesity-related chronic disease index (ORCDi), a novel composite metric to quantify ORCD burden: an ecological study across U.S. census tracts.

BACKGROUND: Obesity contributes substantially to chronic disease multimorbidity in the U.S., yet no composite metric exists to quantify the cumulative burden of obesity-related chronic diseases at the community level. We aimed to develop and validate the obesity-related chronic disease index (ORCDi) and examine its spatial distribution and associations with social vulnerability domains. METHODS: We used Centers for Disease Control and Prevention (CDC) PLACES 2024 estimates for nine obesity-related conditions across 83,522 U.S. census-tracts. Principal component analysis was applied to derive

A Census Tract-Level Examination of Differences in Social Determinants of Health Among People With HIV, by Race/Ethnicity and Geography, United States and Puerto Rico, 2017.

OBJECTIVE: Social and structural factors, referred to as social determinants of health (SDH), create pathways or barriers to equitable sexual health, and information on these factors can provide critical insight into rates of diseases such as HIV. Our objectives were to describe and identify differences, by race/ethnicity and geography, in SDH among adults with HIV. METHODS: We conducted an ecological study to explore SDH among people with HIV diagnosed in 2017, by race/ethnicity and geography, at the census-tract level in the United States and Puerto Rico. We defined the least favorable SDH

Spatial Distribution of Hospitalizations for Ischemic Heart Diseases in the Central Region of Asturias, Spain.

Hospitalizations for ischemic heart disease have an uneven distribution throughout Spain, in which Asturias is the community with the highest rates of acute myocardial infarction (AMI) and angina pectoris (AP). Cardiovascular diseases are related to environmental, socioeconomic and previous medical conditions, which result in geographical differences in the incidence of hospital admissions and mortality. The goal of this study was to describe the spatial distribution of hospital admissions in the central area of Asturias and explore the existence of spatial patterns or clusters. Urgent hospita

Spatial clusters of latent tuberculous infection, Connecticut, 2010-2014.

SETTING: In the United States, tuberculosis (TB) control is increasingly focusing on the identification of persons with latent tuberculous infection (LTBI). OBJECTIVE: To characterize the local epidemiology of LTBI in Connecticut, USA. METHODS: We used spatial analyses 1) to identify census tract-level clusters of reported LTBI and TB disease in Connecticut, 2) to compare persons and populations in clusters with those not in clusters, and 3) to compare persons with LTBI to those with TB disease. RESULTS: Significant census tract-level spatial clusters of LTBI and TB disease were identified.

Comparison of Driving Times to Opioid Treatment Programs and Pharmacies in the US.

IMPORTANCE: Methadone maintenance is an effective treatment of opioid use disorder, but federal regulations in the US restrict methadone dispensing to opioid treatment programs (OTPs). In Australia, Canada, and the UK, patients can obtain methadone maintenance from community pharmacies. OBJECTIVE: To compare driving access to methadone maintenance treatment between OTP and pharmacy dispensing models. DESIGN, SETTING, AND PARTICIPANTS: This descriptive cross-sectional study assessed driving times from census tract mean centers of population to OTPs and pharmacies. Census tracts from the 50 US

Urban-rural differences in tobacco product availability in food retailers, United States, 2017.

PURPOSE: Tobacco use prevalence is higher in rural compared to urban settings, possibly due to differences in tobacco availability, including the option to purchase food and other essential items in stores that do not sell tobacco (tobacco-free food retailers). The goal of this research is to determine whether tobacco-free food retailer availability varies by urbanicity/rurality. METHODS: Using the 2017 National Establishment Time-Series database, we identified food retailers across all census tracts containing food retailers in the United States (n = 66,053). We used multivariable logistic a

Pharmacy-based methadone dispensing and drive time to methadone treatment in five states within the United States: A cross-sectional study.

BACKGROUND: Within the United States, there is a shortage of opioid treatment programs (OTPs), facilities which dispense methadone for opioid use disorder. It is unknown how pharmacy-based methadone dispensing, as available internationally, could affect methadone access. We aimed to compare drive times to the nearest OTP with drive times to the nearest chain pharmacy in urban and rural census tracts. METHODS: Cross-sectional geospatial analysis of 2018 OTP location data and 2017 pharmacy location data. We included census tracts with non-zero population in Indiana, Kentucky, Ohio, Virginia, an
