Traumatic central cord syndrome in a patient with Os odontoideum.

We report the case of a 33-year-old man who presented to the emergency department the morning after a motor vehicle accident with physical findings consistent with central cord syndrome. Radiographs of the cervical spine revealed os odontoideum. This abnormality of the odontoid process has not previously been described in association with central cord syndrome.

Bilateral upper-extremity deep vein thrombosis following central cord syndrome.

UNLABELLED: Deep vein thrombosis (DVT) is a common complication following spinal cord injury (SCI). Although DVT of the upper extremity is much less common than DVT of the lower extremities, the risk of pulmonary embolism following upper-extremity DVT should not be disregarded. METHOD: Case report. FINDINGS: A bilateral upper-extremity DVT developed in a 51-year-old woman with SCI (central cord syndrome) being followed in our rehabilitation clinic. Medical treatment resulted in improvement in the clinical status of the patient as well as the regression in the thrombus. CONCLUSION: In patien…

Central cord syndrome after total hip arthroplasty: a patient report.

STUDY DESIGN: Case report. OBJECTIVE: To present a patient with central cord syndrome injury after total hip arthroplasty performed under general endotracheal anesthesia. SUMMARY OF BACKGROUND DATA: Central cord syndrome, a common injury usually sustained as a result of an extension injury to the cervical spine, often occurs in geriatric patients with underlying spondylotic changes. The injury results in weakness and sensory changes, which are more pronounced in the upper than in the lower extremities. Patients with this syndrome experience variable return of function, but some degree of res…

The syndrome of acute central cervical spinal cord injury revisited.

Seven patients with acute central cervical spinal cord injury were operated upon during the subacute stages following injury, when their condition had improved and become stable for seven to ten days. Myelography in each case showed significant anterior compression of the spinal cord. The rapid improvement in function following the operation in these patients was gratifying and emphasizes the pathophysiological importance of spinal compression in this condition.

Comparative Validation of Scoring Systems in Acute Traumatic Central Cord Syndrome: Acute Traumatic Central Cord Syndrome Score, Central Cord Score, and Subaxial Cervical AO Spine Injury Score for Surgical Decision Making, Recovery, and Timing of Surgery.

Acute traumatic central cord syndrome (ATCCS) is the most common form of incomplete spinal cord injury. Treatment recommendations for ATCCS patients are largely from North America, and their applicability to Asian populations remains uncertain. Scoring systems such as the Acute Traumatic Central Cord Syndrome Score (ATCCSS), Central Cord Score (CCScore), and Subaxial Cervical AO Spine Injury Score (Subaxial AOSIS) can guide treatment, standardize practice, and improve outcomes. We aimed to validate and compare the predictive capabilities of ATCCSS, CCScore, and Subaxial AOSIS in a Southeast As…

[Risk factors associated with lethality on patients with central cord syndrome].

BACKGROUND: Central Cord Syndrome is the most common incomplete spinal cord injury, and it represents 9% of all spinal injuries of the adult. OBJECTIVE: to determine the risk factors associated with lethality or/and mechanical ventilation (MV) in patients with Central Cord Syndrome (CCS). MATERIAL AND METHODS: upon a retrospective cohort with patients with posttraumatic CCS we evaluated at the time of emergency admission and until the hospital discharge. The dependent variable was dead or MV. We calculated incidence, relative risk (RR) with CI95% and a multivariate model for the association …

Comparison of outcomes between people with and without central cord syndrome.

STUDY DESIGN: Retrospective analysis of prospectively collected data. OBJECTIVE: Central cord syndrome (CCS) is reported to have better outcomes than other cervical lesions, especially for ambulation and bladder recovery. However, a formal comparison between patients with CCS and other incomplete cervical spinal cord injuries (iCSCI) is lacking. Aim of the study is to investigate the neurological and functional outcomes in patients with or without CCS. SETTING: European Multicenter Study. METHODS: Data following SCI were derived from the European Multicenter Study about Spinal Cord Injury D…

A review of surgical intervention in the setting of traumatic central cord syndrome.

BACKGROUND CONTEXT: Surgical treatment in the setting of central cord syndrome (CCS) has become safer since Schneider's original description. It is generally accepted that a decompressive surgical intervention is a valid treatment option in a patient with CCS and radiographic evidence of spinal cord compression. The optimal timing of surgical intervention for CCS remains controversial. PURPOSE: To review a single institution's experience managing CCS, with particular emphasis on surgical versus medical management, timing of surgery, neurologic outcomes, hospital length of stay, and complicati…

Central cord syndrome in a high school wrestler: a case report.

OBJECTIVE: To alert athletic trainers to the importance of recognizing the signs and symptoms of central cord syndrome. BACKGROUND: A 15-year-old high school wrestler was found lying supine on the mat after sustaining a hyperextension injury to his neck while drilling during practice, complaining of numbness, tingling, and a burning sensation in all 4 extremities. Touching the extremities elicited an extreme burning sensation. After in-line stabilization of the cervical spine was performed, palpation of the spinous processes elicited tenderness and an increase in pain. Six weeks before the in…

A new orthosis for central cord syndrome and brachial plexus injuries.

Proximal upper extremity weakness may develop secondary to central cord syndrome due to spinal cord injury or brachial plexus injury. Functional deficits, pain, decreased upper extremity arm swing during gait, and shoulder subluxation are common sequelae of these injuries. This report describes a new orthotic design that can be easily fabricated in two to four hours from readily available materials to compensate for these deficits. This orthosis allows for early participation in activities of daily living for patients with greater proximal than distal upper extremity weakness. The orthosis con…

Central cord syndrome in Ireland: the effect of age on clinical outcome.

Central cord syndrome is an incomplete spinal cord injury first described by Schneider et al. (J Neurosurg 11:546-547, 1954). The typical neurological deficit is symmetrical incomplete quadriplegia affecting the upper more than the lower limbs; sensory impairment is variable and urinary retention is common. Records of all patients with acute traumatic central cord syndrome admitted to the National Spinal Injuries Unit from 1999 to 2007 were compiled from a prospectively collected computerized spinal database. Data recorded included patient demographic profile, mechanism of injury, initial neur…

Traumatic central cord Syndrome: An integrated neurosurgical and neurocritical care perspective.

Traumatic Central Cord Syndrome (TCCS) presents complex challenges in the management of spinal cord injury. Characterized by disproportionate upper limb weakness, TCCS is the most common clinical spinal cord syndrome, typically affecting males in a bimodal age distribution. Mechanisms include hyperextension injuries in older adults with degenerative cervical spine disease and high-energy trauma in younger individuals. Diagnosis is based on neurological assessment, with the American Spinal Injury Association (ASIA) Impairment Scale used for severity classification. Management strategies, includ…

Acute traumatic central cord syndrome: magnetic resonance imaging and clinical observations.

OBJECT: Acute traumatic central cord syndrome has been classically thought to be caused by a hemorrhage that primarily affects the central part of the spinal cord and that destroys the axons of the inner part of the corticospinal tract devoted to the motor control of the hands. Some authors, however, have proposed that its pathogenesis is based on the destruction of the motor neurons supplying the muscles of the hand. To test the validity of these two theories, the authors retrospectively studied the magnetic resonance (MR) images obtained in 18 cases of acute traumatic central cord syndrome (…

[Acute Traumatic Central Cord Syndrome: Etiology, Pathophysiology, Clinical Manifestation, and Treatment].

Acute Traumatic Central Cord Syndrome: Etiology, Pathophysiology, Clinical Manifestation, and Treatment Abstract. The acute traumatic central cord syndrome (ATCCS) represents an injury to the spinal cord with disproportionately greater motor impairment of the upper than the lower extremities, with bladder dysfunction and with varying degrees of sensory loss below the level of the respective lesion. The mechanism of ATCCS is most commonly a traumatic hyperextension injury of the cervical spine at the base of an underlying spondylosis and spinal stenosis. The mean age is 53 years, and segments C…

Acute traumatic central cord syndrome: analysis of clinical and radiological correlations.

AIM: In patients with traumatic spinal cord injury, several studies correlate neurological impairment and radiological findings. However, little information is available about this correspondence in the particular group of acute traumatic central cord syndrome. The object of the present work was to describe the clinical and radiological features of a series of patients presenting with acute traumatic central cord syndrome and to analyze clinical and radiological correlations on admission and at last follow-up. METHODS: Retrospective review of 15 patients diagnosed of acute traumatic central c…

Cervical central cord syndrome involving the spinal trigeminal nucleus: a case report.

BACKGROUND: Central cord syndrome is a commonly encountered disease entity. Its underlying pathophysiology, however, is not well understood. METHODS: A 20-year-old patient with cervical injury with an atypical symptom of burning sensation in the face and neck area is reported. RESULTS: The initial burning sensation in the face retreated from the center towards the periphery according to the "onionskin" segmentation of facial dermatomes, which suggested some pathologic condition residing in the spinal trigeminal nucleus. CONCLUSIONS: The findings of this case suggest that the main locus of p…
