Pure post-stroke cerebellar cognitive affective syndrome: a case report.

Cerebellar pathology commonly shows important motor signs and less evident cognitive dysfunction. The 'cerebellar cognitive affective syndrome' is characterised by impairment on executive function, spatial cognition, language and behaviour. We report the case of a man with acute onset of transitory motor features and severe mental disorders. Cranial CT and brain MRI revealed extended cerebellar lesions. Neuropsychological assessment disclosed deficits of attention, executive function and memory. Auditory event-related potentials showed abnormal P300. These data suggest a pure "cerebellar cogni

The cerebellar cognitive affective syndrome scale in early multiple sclerosis.

BACKGROUND: Cerebellar posterior lobes regulate various cognitive functions, and their damage can result in Cerebellar Cognitive Affective Syndrome (CCAS). The validity of the CCAS Scale (CCAS-S), a reliable tool to diagnose CCAS, remains unexplored in multiple sclerosis (MS). OBJECTIVES: To evaluate the performance of CCAS-S in patients with MS (pwMS) at clinical onset, and to assess MRI characteristics of pwMS with CCAS (CCAS+) compared with healthy controls (HC) and with pwMS classified by standard cognitive assessments as cognitively impaired (CI+) or cognitively normal (CI- CCAS-). [The Hungarian validation of the Cerebellar Cognitive-Affective Syndrome Scale].

Introduction: Cerebellar cognitive-affective syndrome can manifest in not only motor executive dysfunction, but also memory, shift inhibition, conceptualization, emotion, and behavioral regulation. A dedicated Hungarian-language instrument to measure the syndrome has not yet been available to Hungarian professionals. Objective: Validating the Cerebellar Cognitive-Affective Scale (CCAS-H) in a Hungarian sample; evaluating its diagnostic validity and publishing the test for the professionals. Method: Patients with cerebellar involvement and controls were administered tests in a multicenter resea

The Cerebellar Cognitive Affective/Schmahmann Syndrome Scale in Spinocerebellar Ataxias.

The Cerebellar Cognitive Affective/Schmahmann Syndrome (CCAS) manifests as impaired executive control, linguistic processing, visual spatial function, and affect regulation. The CCAS has been described in the spinocerebellar ataxias (SCAs), but its prevalence is unknown. We analyzed results of the CCAS/Schmahmann Scale (CCAS-S), developed to detect and quantify CCAS, in two natural history studies of 309 individuals Symptomatic for SCA1, SCA2, SCA3, SCA6, SCA7, or SCA8, 26 individuals Pre-symptomatic for SCA1 or SCA3, and 37 Controls. We compared total raw scores, domain scores, and total fail

Hungarian adaptation of the cerebellar cognitive affective/Schmahmann Syndrome Scale.

Recent studies have reported that cerebellar lesions can cause cognitive, behavioral, and affective symptoms. This constellation is called the cerebellar cognitive affective syndrome (CCAS). A bedside instrument, the CCAS-Scale, has been developed to screen for this clinical presentation. The aim of this study is to adapt the CCAS-Scale to Hungarian according to international cross-cultural guidelines. In cooperation with the senior author of the original CCAS-Scale, we defined a five-step adaptation protocol (license number 6758-1/2021). Step 1: translation of the scale from English to Hungar

The Cerebellar Cognitive Affective/Schmahmann Syndrome: a Task Force Paper.

Sporadically advocated over the last two centuries, a cerebellar role in cognition and affect has been rigorously established in the past few decades. In the clinical domain, such progress is epitomized by the "cerebellar cognitive affective syndrome" ("CCAS") or "Schmahmann syndrome." Introduced in the late 1990s, CCAS reflects a constellation of cerebellar-induced sequelae, comprising deficits in executive function, visuospatial cognition, emotion-affect, and language, over and above speech. The CCAS thus offers excellent grounds to investigate the functional topography of the cerebellum, an

Cognitive impairment in essential tremor assessed by the cerebellar cognitive affective syndrome scale.

BACKGROUND: Essential tremor (ET) is a movement disorder characterized by cerebellar neurodegenerative changes. ET is also associated with non-motor symptoms including cognitive impairment. The neuropsychologic profile of a patient with ET could relate to cerebellar cognitive affective syndrome (CCAS). OBJECTIVE: This study aimed to assess the prevalence of cognitive impairment in patients with ET and identify whether the cognitive impairment in ET corresponds to a CCAS. METHODS: Cognitive functions were evaluated with the CCAS-Scale (CCAS-S) in 20 patients with ET and 20 controls matched fo

Remote Online-Based Application of the Cerebellar Cognitive Affective Syndrome Scale.

The Cerebellar Cognitive Affective Syndrome Scale (CCAS-S) is increasingly used in clinical trials to assess non-motor function in cerebellar disorders, yet its suitability for remote administration has not been evaluated. Using a matched-pairs design, we compared remote, video-based administration of the German CCAS-S (version A) in 22 patients with cerebellar disease and 22 matched controls with at site -testing from a larger, previously tested cohort. Group matching included age, level of education, sex, occupational experience, and ataxia severity. Uncorrected as well as age, education and

Cerebellar cognitive affective syndrome in patients with spinocerebellar ataxia type 10.

BACKGROUND: Spinocerebellar ataxia type 10 (SCA10) is an autosomal dominant cerebellar ataxia, characterized by epilepsy, ataxic symptoms, and cognitive impairments linked to Cerebellar Cognitive Affective Syndrome (CCAS). The Cerebellar Cognitive Affective Syndrome Scale (CCAS-S) has been developed to identify CCAS across various cerebellar pathologies. OBJECTIVE: To determine whether patients with SCA10 exhibit CCAS using the CCAS-S, and to compare its effectiveness with the Montreal Cognitive Assessment (MoCA). A secondary objective was to evaluate the effect of demographic and clinical da

Cerebellar Cognitive-Affective Syndrome: A Case Report and Review of Literature.

INTRODUCTION: The cerebellum has traditionally been found to be the only coordinator of motor functions, however, recent studies have shown that the cerebellum also has a role in cognitive and affective processing. CCAS is characterized by impaired executive functions, visual-spatial disorganization, emotional dysregulation, changes in personality and language deficits. The cognitive and affective symptoms that make up the CCAS have been attributed to disorders of the cerebral hemispheres, specifically in association areas and paralimbic regions. CASE REPORT: A 26-year-old woman is presented,

Validation of the cerebellar cognitive affective syndrome (CCAS) scale in CCAS patients and cerebellar controls.

OBJECTIVE: The cerebellar cognitive affective syndrome (CCAS) scale has been developed to screen for possible cognitive and affective impairments in cerebellar patients, but previous studies stressed concerns regarding insufficient specificity of the scale. Also, direct comparisons of CCAS scale performance between cerebellar patients with and without CCAS are currently lacking. The aim of this study was to evaluate the validity of the CCAS scale in cerebellar patients. METHOD: In this study, cerebellar patients with CCAS (n = 49), without CCAS (n = 30), and healthy controls (n = 32) were inc

Affective and Cognitive Symptoms in the Elderly: Review and Case Report of Co-Occurring Klein-Levin Syndrome and Cerebellar Cognitive Affective Syndrome.

Illnesses with both cognitive and affective symptoms are common in older adults and have a broad neuropsychiatric differential diagnosis. We present a case of a 66-year-old female with fluctuating affective symptoms on a background of chronic dysexecutive cognitive symptoms, posing an intriguing diagnostic challenge and ultimately considered to be probable Kleine-Levin Syndrome (KLS) and cerebellar cognitive affective syndrome (CCAS). To our knowledge, this is the first published instance of both diagnoses not only co-occurring, but having a contemporaneous onset. This is followed by an overvi

Ferdinando Rossi Lecture: the Cerebellar Cognitive Affective Syndrome-Implications and Future Directions.

The notion that the cerebellum is devoted exclusively to motor control has been replaced by a more sophisticated understanding of its role in neurological function, one that includes cognition and emotion. Early clinical reports, as well as physiological and behavioral studies in animal models, raised the possibility of a nonmotor role for the cerebellum. Anatomical studies demonstrate cerebellar connectivity with the distributed neural circuits linked with autonomic, sensorimotor, vestibular, associative and limbic/paralimbic brain areas. Identification of the cerebellar cognitive affective s

Cerebellar cognitive affective syndrome without global mental retardation in two relatives with Gillespie syndrome.

Although previous studies of Gillespie syndrome have systematically reported a generalized delay of cognitive development (mental retardation or oligophrenia), psychometric data to substantiate this view are strikingly absent. In the present study two first degree relatives (mother and daughter) with Gillespie syndrome were neuropsychologically investigated. Aside from a marked asymmetry in the Wechsler-IQ profile, consisting of significantly better results on the verbal [Verbal IQ (VIQ)] than on the nonverbal part [Performance IQ (PIQ)] of the test, cognitive and behavioral assessments reveal

Prevalence of Cerebellar Cognitive Affective Syndrome in Patients with Cerebellar Disorders: A Systematic Review and Meta-Analysis.

Cerebellar cognitive affective syndrome (CCAS) is increasingly recognized across cerebellar disorders, but its prevalence remains uncertain because study populations and assessment methods vary. We systematically evaluated CCAS prevalence across cerebellar disorders and synthesized estimates according to assessment method. MEDLINE, Embase, Scopus, CINAHL, and two trial registries were searched from inception to November 5, 2025, without language restrictions. Two reviewers independently selected studies, extracted data, and assessed risk of bias using the Joanna Briggs Institute checklist. Stu

CLINICAL NOTE: CEREBELLAR COGNITIVE AFFECTIVE SYNDROME IMPROVEMENT WITH SELECTIVE INHIBITOR OF SEROTONIN RECAPTATION.

Cerebellar cognitive affective syndrome (CCAS) is characterized by alterations at the cognitive level (dysexecutive syndrome, visuospatial deficit, language ...), associated with affective / emotional changes. Its pathophysiology is not well known and there is currently no specific treatment. We describe a 64-year-old man with a rare condition of cognitive-behavioral disorder after an infarction in the left middle cerebral artery, dominated by executive dysfunctions, predominantly oral apraxia, interrupted divided attention, disturbed visuospatial organization and affective abnormalities with

Understanding cognitive features of cervical dystonia: application of the cerebellar cognitive affective syndrome scale (CCAS-S).

Cervical Dystonia (CD) is now recognized to encompass a constellation of non-motor symptoms, including sensory, cognitive, and psychiatric manifestations, which significantly affect patients' quality of life. Cerebellar dysfunction may affect cognitive and affective processing in CD, mirroring cognitive and affective patterns observed in Cerebellar Cognitive and Affective Syndrome (CCAS). To investigate impairments in cerebellar-dependent cognitive and affective domains in CD patients using the Cerebellar Cognitive and Affective Syndrome scale (CCAS-S), and to analyze the potential relationshi
