Aqueductal flow of cerebrospinal fluid (CSF) and anatomical configuration of the cerebral aqueduct (AC) in patients with communicating hydrocephalus--the trumpet sign.

PURPOSE: We explore the relationship of aqueductal flow of cerebrospinal fluid (CSF) and the changes of the anatomical configuration of the cerebral aqueduct (AC) in patients with communicating hydrocephalus (CH) in a routine MRI setting. METHODS/PATIENTS: We performed a retrospective evaluation of different anatomical configurations of the AC on midsaggital MRI images in 43 patients (medial age 67 years, median 68 years, range from 14 to 85, 25 women) with suspected communicating hydrocephalus and compared the anatomical form of the AC on the sagittal sequences with MRI CSF flow data. Comprehensive review of the cerebral aqueduct: anatomy, embryology, imaging, genetics, molecular, and pathology.

The cerebral aqueduct (CA), or aqueduct of Sylvius, is the narrowest portion of the ventricular system, connecting the third and fourth ventricles and providing an essential conduit for cerebrospinal fluid (CSF) circulation. Despite its small caliber, the CA transmits over 500 ml of CSF daily, and its vulnerability to obstruction makes it central to the pathophysiology of hydrocephalus. This review synthesizes current knowledge of the CA across developmental, anatomical, and clinical dimensions. We examine its embryological formation and the contribution of structures such as the subcommissura

Using deep learning convolutional neural networks to automatically perform cerebral aqueduct CSF flow analysis.

Since the development of phase-contrast magnetic resonance imaging (PC-MRI), quantification of cerebrospinal fluid (CSF) flow across the cerebral aqueduct has been utilized for diagnosis of conditions such as normal pressure hydrocephalus (NPH). This study aims to develop an automated method of aqueduct CSF flow analysis using convolution neural networks (CNNs), which can replace the current standard involving manual segmentation of aqueduct region of interest (ROI). Retrospective analysis was performed on 333 patients who underwent PC-MRI, totaling 353 imaging studies. Aqueduct flow measureme

Enlarged Vestibular Aqueduct Syndrome: Sudden Hearing Loss in a Child with a Cerebral Shunt.

Enlarged vestibular aqueduct syndrome (EVAS) is the most common congenital ear anomaly that causes sensorineural hearing loss in children and may predispose a child to sudden hearing loss from sudden pressure changes or minor head trauma. We report a case of a 4-year-old boy with a history of a ventriculoperitoneal shunt and migraines who presented to the emergency department with parental and child care provider reports of acute hearing loss, without a history of trauma, infection, or hardware malfunction, who was diagnosed with bilateral EVAS. Diagnosis of EVAS occurs with specific temporal

Iatrogenic obstructive hydrocephalus resulting from Gelfoam accumulation within the cerebral aqueduct: a case report.

The authors present the case of a previously healthy 12-year-old male with intractable seizures localized to a right frontal area of encephalomalacia and porencephalic cyst who underwent resection of the seizure focus. The surgical resection cavity extended into the right lateral ventricle, and due to encountered hemorrhage, Gelfoam was used for optimal hemostasis. The patient did well following the procedure, but presented 5 months later with headaches and emesis and was discovered to have obstructive hydrocephalus on imaging studies. Endoscopic third ventriculostomy (ETV) was performed, wher

Microsurgical anatomy of the cerebral aqueduct and periaqueductal region: implications for microscopic and neuroendoscopic approaches.

PURPOSE: The cerebral aqueduct (CA) is the narrowest part of the ventricular system, connecting the third and fourth ventricles. This structure is encircled by important neuroanatomical features, and with the increasing application of neuroendoscopic techniques, a comprehensive understanding of the anatomy of the CA is essential. This study aims to provide a comprehensive, multi-perspective anatomical analysis of the cerebral aqueduct and its surrounding structures, integrating microsurgical dissections with endoscopic anatomical correlations to support safer surgical planning. METHODS: Three

Cerebral aqueduct block attenuates cardio-renal injuries in post-DOCA-NaCl-hypertensive Dahl R rats.

The systemic and/or local effects of the hydrocephalic brain were investigated in DOCA-NaCl-hypertensive Dahl R rats induced by 250 mg kg(-1) DOCA in silicone and 1% saline water. After a 1-week recovery with 0.3% NaCl chow and tap water, one group had the aqueduct of Sylvius blocked with silicone and epoxy materials with a control sham group matching mean blood pressure (BP) and body weight. The 4-week-postsurgery BP on the 0.3% NaCl diet averaged 161±3.2 in the sham group and 146±2.3 mm Hg in the blocked group (P<0.0001). Both groups were then given an 8% NaCl diet and after 4 weeks, the sha

Accuracy of MRI derived cerebral aqueduct flow parameters in the diagnosis of idiopathic normal pressure hydrocephalus.

Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible cause of dementia-like symptoms among the elderly. Current diagnostic guidelines for iNPH rely on clinical manifestations and ventricular morphology, which often lack accuracy. While magnetic resonance imaging (MRI) CSF flowmetry of the cerebral aqueduct provides a noninvasive aid to differential diagnosis, previous studies suffered from small sample sizes. This study compares the accuracy of different CSF flow parameters for iNPH diagnosis in a general patient population. From 2016 to 2018, a total of 216 subjects ove

Endoscopically-assisted resection of a choroid plexus vascular malformation traversing the cerebral aqueduct: technical case report.

OBJECTIVE: We report a case of a choroid plexus vascular malformation of the cerebral aqueduct, third, and fourth ventricles of an adolescent female that was resected with endoscopic assistance. CLINICAL PRESENTATION: A 14-year-old girl presented with a 1-week history of headaches and emesis. A noncontrasted computed tomographic scan of the head demonstrated enlarged lateral and third ventricles. Subsequent magnetic resonance imaging scans with and without contrast revealed an enhancing mass originating in the third ventricle, traversing the cerebral aqueduct, and terminating in the fourth ve

Cerebrospinal fluid velocity amplitudes within the cerebral aqueduct in healthy children and patients with Chiari I malformation.

PURPOSE: To assess the effects of cerebrospinal fluid (CSF) bidirectional motion in Chiari malformation type I (CMI), we monitored CSF velocity amplitudes on phase contrast MRI (PC-MRI) in patients before and after surgery; and in healthy volunteers. MATERIALS AND METHODS: 10 pediatric volunteers and 10 CMI patients participated in this study. CMI patients underwent PC-MRI scans before and approximately 14 months following surgery. Two parameters-amplitude of mean velocity (AMV) and amplitude of peak velocity (APV) of CSF-were derived from the data. Measurements were made at the mid-portion o

Absence of subcommissural organ in the cerebral aqueduct of congenital hydrocephalus spontaneously occurring in MT/HokIdr mice.

The midbrains of pups with congenital hydrocephalus spontaneously occurring in MT/HokIdr mice were histologically examined. The subcommissural organ (SCO) and the posterior commissure were completely absent in the hydrocephalic brain. The cerebral aqueduct in the hydrocephalic brain was never completely stenosed, though it was somewhat narrowed in its middle region as compared with that in the normal brain. A possible interrelationship between an absence of SCO and a cause of congenital hydrocephalus is discussed.

Absence of CSF flow within the cerebral aqueduct in spontaneous intracranial hypotension: a report of two cases.

Spontaneous intracranial hypotension (SIH) is a potentially debilitating condition resulting from a low cerebrospinal fluid (CSF) volume secondary to spinal CSF leakage. Characteristic clinical and radiological imaging findings are helpful in diagnosis. Herein, we present and discuss the magnetic resonance imaging (MRI) and CSF flow study of two patients with SIH and no CSF flow within the cerebral aqueduct, which is extremely rare in the literature.

[Method of unidimensional echolocation of the cerebral aqueduct and upper portions of the 4th ventricle in healthy subjects and in pathology].

The authors suggest a method of unichamber echo location of the aqueduct and upper parts of the fourth ventricle by means of which it is possible to determine displacement of these parts of the c.s.f. system in a plane close to the frontal plane in development of a volumetric focus in the posterior cranial fossa. The value of the method in making the diagnosis of a surgical pathological condition in the posterior cranial fossa is almost the same as the value of unichamber echo diagnosis of supratentorial processes.

CT ventriculographic identification and neuroendoscopic treatment of acquired hydrocephalus due to cerebral aqueduct lesions: a two-case report.

Hydrocephalus secondary to cerebral aqueduct lesions poses significant diagnostic and therapeutic challenges, particularly when causative lesions are occult on conventional neuroimaging. We present two pediatric cases of acquired obstructive hydrocephalus, in which routine laboratory tests were unremarkable except for mildly elevated tumor markers. Both patients exhibited acute symptoms of intracranial hypertension. Magnetic resonance imaging (MRI) demonstrated supratentorial hydrocephalus with periventricular edema but failed to visualize a definitive obstructive lesion. Subsequent cerebral v

Biomechanical effects of hyper-dynamic cerebrospinal fluid flow through the cerebral aqueduct in idiopathic normal pressure hydrocephalus patients.

Normal pressure hydrocephalus (NPH) is an intracranial disease characterized by an abnormal accumulation of cerebrospinal fluid (CSF) in brain ventricles within the normal range of intracranial pressure. Most NPH in aged patients is idiopathic (iNPH) and without any prior history of intracranial diseases. Although an abnormal increase of CSF stroke volume (hyper-dynamic CSF flow) in the aqueduct between the third and fourth ventricles has received much attention as a clinical evaluation index in iNPH patients, the biomechanical effects of this flow on iNPH pathophysiology are poorly understood
