Cerebral hemorrhage in Fabry's disease.

Fabry's disease is an X-linked lysosomal storage disorder resulting from alpha-galactosidase A deficiency. Although ischemic stroke is recognized as an important manifestation of Fabry's disease, hemorrhagic stroke is considered to be rare. Here, we report our recent clinical experience with three hemizygous male patients with Fabry's disease who developed cerebral hemorrhage. One patient had classic type Fabry's disease with p.Ala37Val mutation and others had cerebrovascular variant with p.Glu66Gln mutation. Degeneration of the cerebral small arteries secondary to deposition of glycosphingoli

[Cerebral hemorrhage and angiopathy amyloid].

BACKGROUND: Cerebral amyloid angiopathy is considered pathogenic in non traumatic cerebral lobar hemorrhages. AIM: To study the frequency of cerebral amyloid angiopathy in brains of patients dying of non traumatic cerebral hemorrhages. MATERIAL AND METHODS: Thirty seven brains from patients, 25 men and aged 65 +/- 10 years old, with cerebral hemorrhages (14 lobar, 18 in basal ganglia and 5 in cerebellum or brainstem) were studied. As controls, the brains of 30 subjects, 14 men and aged 64 +/- 16 years old, dying of non neurological causes were studied. Deep and cortical vessels were stained

[Cerebral hemorrhage in Sneddon syndrome].

INTRODUCTION: Sneddon syndrome is the association of livedo retricularis and cerebrovascular lesions. It is a vasculopathy of vessels of small and medium calibre which affects young people and is of unknown origin. It is often associated with arterial hypertension and repeated ischemic cerebrovascular accidents which lead to intellectual deterioration. CLINICAL CASE: We describe a 49 year old man who, from the age of 23, had had repeated transient or established episodes compatible with ischemic cerebrovascular accidents. These were followed by various neurological sequelae and by intellectua

Cerebral diaschisis following cerebellar hemorrhage.

In 1914, Von Monakow described diaschisis, the recovery of lost cortical function in regions positionally distant from, but linked by neuronal tracts to, the primary site of cortical damage. Cerebellar diaschisis after cortical insult is detailed in the literature; however, cortical diaschisis after cerebellar insult remains a rarely reported occurrence. We describe a 36-year-old woman with rupture of a right-sided cerebellar arteriovenous malformation who developed such expected cerebellar signs as ataxia, dysmetria, and nystagmus. Days later, the patient developed profound impulsivity, disin

[Cerebral hemorrhage associated with phenylpropanolamine].

In recent years, sympathomimetic drugs, including ephedrine, amphetamine and phenylpropanolamine, have been increasingly associated with cerebral haemorrhage and infarction. We report a case of intracranial bleeding in a 37 year-old woman with mononucleosis who took therapeutic doses of phenylpropanolamine. Cerebral CT on admission showed a left-sided intracerebral hematoma. Angiography was normal. Late bleeding occurred on the fourth day. She survived but developed a right-sided cerebral infarction with persistent epilepsy. Her mononucleosis was of moderate severity without bleeding diastesis

Moyamoya disease and cerebral hemorrhage.

A case with the clinical, x-ray, and computed tomographic features characteristic of moyamoya disease and a large intracerebral hemorrhage is presented. The recent literature is reviewed, noting current concepts in diagnosis, treatment, and prognosis of this curious disorder. Although much has been written about moyamoya disease and its treatment, it remains a very unusual disorder, particularly in a middle-aged white adult.

Cerebral vasospasm after subarachnoid hemorrhage.

Chronic cerebral vasospasm remains the most important cause of subsequent morbidity in patients who survive the first 48 to 72 hours after a subarachnoid hemorrhage. Prolonged arterial narrowing compromises cerebral hemodynamics and results in cerebral ischemia. Among patients in whom symptomatic chronic cerebral vasospasm develops, almost half die or have a serious residual neurologic deficit. Present evidence indicates that sustained vessel narrowing results from structural changes within the arterial wall rather than from active contraction of vascular smooth muscle. The mechanism (or mecha

Cerebral hemorrhage following heart surgery.

The authors report a study of the problem of intracranial hemorrhage in 16 children following cardiac surgery, four studied clinically, and 12 by postmortem pathological review. Eleven children sustained subdural hematomas of varying sizes, one had a massive extradural clot, and four had intracerebral clots. The pre-, intra-, and postoperative data of these 16 patients are presented, but the specific factors causing the intracranial hemorrhage remain unexplained. The neurological course was similar to that of patients with an intracranial space-occupying lesion, and fundamental neurosurgical m

Progressive manifestations of reversible cerebral vasoconstriction syndrome presenting with subarachnoid hemorrhage, intracerebral hemorrhage, and cerebral infarction.

Reversible cerebral vasoconstriction syndrome (RCVS) is characterized by sudden-onset headache with focal neurologic deficit and prolonged but reversible multifocal narrowing of the distal cerebral arteries. Stroke, either hemorrhagic or ischemic, is a relatively frequent presentation in RCVS, but progressive manifestations of subarachnoid hemorrhage, intracerebral hemorrhage, cerebral infarction in a patient is seldom described. We report a rare case of a 56-year-old woman with reversible cerebral vasoconstriction syndrome consecutively presenting as cortical subarachnoid hemorrhage, intracer

Delayed Cerebral Ischemia after Subarachnoid Hemorrhage.

Delayed cerebral ischemia (DCI) is the most feared complication of aneurysmal subarachnoid hemorrhage (aSAH). It increases the mortality and morbidity associated with aSAH. Previously, large cerebral artery vasospasm was thought to be the sole major contributing factor associated with increased risk of DCI. Recent literature has challenged this concept. We conducted a literature search using PUBMED as the prime source of articles discussing various other factors which may contribute to the development of DCI both in the presence or absence of large cerebral artery vasospasm. These factors incl

Cerebral Hemorrhage after Cosmetic Facial Injection.

Cosmetic facial injection is a mini-invasive surgical procedure and is widely used at present. Meanwhile, it may also cause severe complications like cerebrovascular infarction occasionally. A 49-year-old female patient presented to a local hospital with dysphoria, left eye and head ache, unconsciousness, orthocolosis, right upper extremity hemiplegic paralysis, and emesia after receiving facial injection of hyaluronic acid into the glabella. Low molecular weight heparin and clopidogrel were used to treat the patient, but there was no relief of symptoms. The patient gradually developed coma an

Cerebral vasospasm following traumatic subarachnoid hemorrhage.

BACKGROUND: Cerebral vasospasm is a preventable cause of death and disability in patients who experience aneurysmal subarachnoid hemorrhage (SAH). The aim of this study is to investigate the incidence of cerebral vasospasm following traumatic SAH and its relationship with different brain injuries and severity of trauma. METHODS: This cross-sectional study was conducted from October 2006 to March 2007 in department of Neurosurgery in Al-Zahra Hospital. Consecutive head-injured patients who had SAH on the basis of an admission CT scan were prospectively evaluated. The severity of the trauma was

[Neurosurgical management of spontaneous cerebral hemorrhage].

Intracerebral hemorrhage (ICH) accounts for 10 to 20% of strokes, but carries the highest rate of morbidity and mortality. Until now, there is no proven benefit in the literature for surgical treatment of ICH, and management of ICH varies greatly in neurosurgical centers. Surgery can be performed through standard craniotomy, or with a stereotactic procedure for deep-seated ICH. Conscious patients with minimal neurological deficit and small ICH are nonsurgical candidates. Patients with a Glasgow Coma Score lower than 4, with large deep-seated ICH are also non surgical candidates. In other situa

[Cerebral computer tomography in subarachnoid hemorrhage].

The evolution of cooperation is a central enigma in evolutionary game theory. Traditionally, the combination of pairwise networks and repeated public goods games (PGGs) with a single state fails to adequately describe realistic group interaction scenarios. On the one hand, pairwise networks lack clear group definitions. On the other hand, a participant's decision affects not only competitors' fitness but also the state of the surrounding environment. To address this problem, we propose a PGG with game transition mechanisms based on uniform random hypergraphs (URH). In our model, game groups fo

[Fibrinolytic treatment of cerebral intraventricular hemorrhage].

INTRODUCTION: Spontaneous cerebral intraventricular hemorrhage (SCIVH) is associated with hydrocephaly which requires ventricular drainage as treatment. Maintenance of adequate debit through the drainage system contributes to reduce morbid mortality. When the drain becomes obstructed, intermittent irrigation is necessary. This causes increased intracranial pressure and risk of infection. We propose the use of recombinant plasminogen tissue activator (r-tPA) in the treatment of SCIVH and associated hydrocephaly. CLINICAL CASES: We gave two patients with SCIVH 4 cc/day of r-tPA intraventricular

[Minimal cerebral hemorrhage in premature infants].

Under the term minimal cerebral haemorrhage we understand subependymal haemorrhages which occur as a consequence of ruptures in the capillaries in the stratum germinativum. They occur exclusively in newborn babies after the 36th week of pregnancy. In premature babies born up to or after 32 weeks we found these haemorrhages to a degree of 45% in cases where there was also more haemorrhage. We did not discover any neurological abnormalities in the development after birth as a result of isolated subependymal haemorrhage. This makes the question of how and in how much detail the parents should be
