Management and Outcomes of Traumatic Cerebral Venous Sinus Injury Among Patients With Traumatic Brain Injury and Concomitant Intracranial Hemorrhage.

BACKGROUND AND OBJECTIVES: Traumatic cerebral venous sinus injury (tCVSI) is a rare but serious complication of traumatic brain injury. Although similar radiographically to spontaneous cerebral venous sinus thrombosis (CVST), tCVSI treatment approaches have not been fully investigated. We characterized complications, clinical outcomes, and recanalization in tCVSI and evaluated treatment approaches. METHODS: In this retrospective review of patients with radiographically confirmed tCVSI across 3 Level 1 trauma centers from 2018 to 2024, we collected patient characteristics, injury patterns, tre

[Subdural hematoma without subarachnoid hemorrhage caused by ruptured traumatic distal aneurysm of anterior cerebral artery].

OBJECTIVE: To describe a rare complication of non-penetrating traumatic brain injury. MATERIAL AND METHODS: We present a 16-year-old patient with multiple skeletal lesions and mild traumatic brain injury (TBI). Two weeks after injury, clinical status suddenly worsened due to large subdural hematoma without signs of subarachnoid hemorrhage despite normal neuroimaging data at baseline. RESULTS: Examination revealed pericallosal artery aneurysm. Microsurgical intervention with aneurysm excision was performed. Histological examination revealed thrombotic masses containing fibrin and no vascular

Systemic tolerance of intravenous milrinone administration for cerebral vasospasm secondary to non-traumatic subarachnoid hemorrhage.

PURPOSE: Delayed cerebral ischemia (DCI) is a severe subarachnoid hemorrhage (SAH) complication, closely related to cerebral vasospasm (CVS). CVS treatment frequently comprises intravenous milrinone, an inotropic and vasodilatory drug. Our objective is to describe milrinone's hemodynamic, respiratory and renal effects when administrated as treatment for CVS. METHODS: Retrospective single-center observational study of patients receiving intravenous milrinone for CVS with systemic hemodynamics, oxygenation, renal disorders monitoring. We described these parameters' evolution before and after mi

Report of cerebral vasospasm as a complication of intracranial subarachnoid hemorrhage following traumatic lumbar puncture.

BACKGROUND: This case report is the first documented and illustrated case of the identification and treatment of intracranial vasospasm as a sequalae of traumatic lumbar puncture (LP). LP is a routine procedure performed for both diagnostic and therapeutic purposes. Although rare, this procedure has risks and complications that should be considered before performing. CASE DESCRIPTION: A 58-year-old male was found to have intracranial subarachnoid hemorrhage (SAH) 2 days after a traumatic LP which occurred in the setting of subtherapeutic international normalized ratio. During his hospitalizat

Digital subtraction cerebral angiography after negative computed tomography angiography findings in non-traumatic subarachnoid hemorrhage.

BACKGROUND: CT angiography (CTA) is widely used for the detection of vascular lesions in patients with non-traumatic subarachnoid hemorrhage (ntSAH); however, digital subtraction angiography (DSA) remains the gold standard for diagnosis. Our aim was to analyze the diagnostic yield of DSA after negative high-resolution CTA findings. METHODS: Records of patients with a CTA-negative ntSAH at a single institution from 2014 to 2018 were retrospectively analyzed. ntSAH patterns were categorized as cortical, perimesencephalic, or diffuse. Subsequent DSA findings were compared across the three cohort

Effectiveness of intrathecal nicardipine on cerebral vasospasm in non-traumatic subarachnoid hemorrhage: a systematic review.

OBJECTIVE: The objective of this review was to determine the effectiveness of intrathecal nicardipine compared to usual care on cerebral vasospasm and its impact on the following outcome measures: mean flow velocities, angiographic and/or clinical vasospasm, and infection rates. INTRODUCTION: The results of non-traumatic (aneurysmal) subarachnoid hemorrhage can have devastating effects on patients in terms of functional outcomes. Although other medications have been and continue to be used, Nimodipine is the only Food and Drug Administration-approved medication for treating and improving outc

Survival with good outcome after cerebral herniation and Duret hemorrhage caused by traumatic brain injury.

Brainstem hemorrhage can occur as a primary or secondary event in traumatic brain injury (TBI). Secondary brainstem hemorrhage that evolves from raised intracranial pressure and transtentorial herniation is referred to as Duret hemorrhage. Duret hemorrhage following TBI has been considered an irreversible and terminal event. The authors report on the case of a young adult patient with TBI who presented with a low Glasgow Coma Scale score and advanced signs of cerebral herniation. She underwent an urgent decompressive hemicraniectomy for evacuation of an acute epidural hematoma and developed a

Relationship between mental disorders and non-traumatic cerebral hemorrhage: cross-sectional analysis and mendelian randomization.

BACKGROUND: The causality between mental disorders and spontaneous non-traumatic cerebral hemorrhage (NCH) has not been sufficiently explored. This study aims to investigate the associations of a broad spectrum of 16 mental disorders with NCH through Mendelian randomization (MR) analysis and to assess the association in a real-world study. METHODS: Univariate analysis and mediational MR analysis were employed to investigate the potential association between mental disorders and NCH. Genetic instruments associated with mental disorders were identified through a genome-wide association study (G

Acute Subdural Hematoma Without Subarachnoid Hemorrhage Due to Traumatic Aneurysm of the Posterior Cerebral Artery.

Acute subdural hematoma (ASDH) typically results from high-energy head trauma, such as motor vehicle accidents or severe falls. It can also be caused by minor head injuries in elderly individuals due to damage to bridging veins or small arteries, but it is considerably rarer in younger patients. A previously healthy 39-year-old man presented with progressive headaches and consciousness disturbance. The emergent computed tomography (CT) revealed ASDH covering most of the left cerebral hemisphere without subarachnoid hemorrhage (SAH). CT angiography (CTA) showed an irregularly shaped aneurysm at

Decreasing the Cerebral Edema Associated with Traumatic Intracerebral Hemorrhages: Use of a Minimally Invasive Technique.

Traumatic brain injury (TBI) is a major public health problem worldwide that affects all age groups. In the United States alone, there are approximately 50,000 deaths from severe traumatic brain injuries each year. In most studies, about 40 % of severe TBI have associated traumatic intracerebral hemorrhages (tICHs). The surgical treatment of tICH is debated largely because of its invasive nature, particularly in reaching deep tICHs. tICHs have a clear contribution to mass effect and exacerbate cerebral edema and ICP because of the break-down products of hemorrhage. We introduce a modification

Timing and safety of anticoagulation in patients with a traumatic cerebral venous sinus thrombosis and intracranial hemorrhage.

BACKGROUND: In traumatic cerebral venous sinus thrombosis (tCVST) with concurrent intracranial hemorrhage (tICH), clinicians must balance thrombus control against the risk of hemorrhagic expansion. Available evidence is heterogeneous, with variable outcomes and inconsistent recommendations. We aimed to synthesize the literature to clarify the timing and safety of therapeutic anticoagulation (AC). METHODS: We conducted a PRISMA guided systematic review of adults with tCVST and tICH from inception through October 2025. Eligible studies included adults with concurrent tCVST and tICH and at least

The Anticoagulation Dilemma: Concurrent Traumatic Brain Hemorrhage and Cerebral Venous Thrombosis in a Blast Injury Patient.

Blast injuries produce complex multi-system trauma through primary barotrauma, secondary penetrating projectiles, tertiary displacement forces, and quaternary mechanisms. Among the most challenging complications is the concurrent presentation of traumatic intracranial hemorrhage and cerebral venous sinus thrombosis (CVST), which creates a profound therapeutic dilemma regarding anticoagulation management. We present a 26-year-old male who sustained injuries from a high-order explosive device that resulted in 19 fatalities. He presented 16 h post-injury with a Glasgow Coma Scale score of 10/15 a

Uncommon Presentation of Craniopharyngioma: A Case Report of Post-traumatic Intratumoral Hemorrhage Leading to Cerebral Infarct.

Craniopharyngiomas emanate from squamous cell remnants in the hypophyseal/pharyngeal duct region. This report details the unprecedented case of a 29-year-old male with adamantinomatous craniopharyngioma, who, following a motor vehicle collision (MVC), presented with post-traumatic intratumoral hemorrhage leading to acute basal ganglia infarct. The patient, previously subjected to subtotal resection, exhibited focal neurological deficits attributed to compression of lenticulostriate arteries due to the sudden increase in tumor volume. The patient, ineligible for thrombolysis or thrombectomy, wa

Hidden dense middle cerebral artery sign in a 4-year-old boy with traumatic subarachnoid hemorrhage.

A 4-year-old boy was admitted with acute onset of hemiplegia of the right side that was secondary to a traffic accident. Initial computed tomography revealed a traumatic subarachnoid hemorrhage, and follow-up computed tomography showed a more localized hematoma of the left sylvian cistern. After a few days of conservative treatment, magnetic resonance imaging (MRI) revealed a cerebral infarction of the left lenticulostriate territory, even though magnetic resonance angiography showed preserved middle cerebral artery flow. Thus, we realized that the hematoma of the sylvian cistern was the so-ca

Derivation of a Predictive Score for Hemorrhagic Progression of Cerebral Contusions in Moderate and Severe Traumatic Brain Injury.

BACKGROUNDS: After traumatic brain injury (TBI), hemorrhagic progression of contusions (HPCs) occurs frequently. However, there is no established predictive score to identify high-risk patients for HPC. METHODS: Consecutive patients who were hospitalized (2008-2013) with non-penetrating moderate or severe TBI were studied. The primary outcome was HPC, defined by both a relative increase in contusion volume by ≥30 % and an absolute increase by ≥10 mL on serial imaging. Logistic regression models were created to identify independent risk factors for HPC. The HPC Score was then derived based on

[Ruptured traumatic aneurysms of the peripheral anterior cerebral artery: study of delayed hemorrhage after closed head injury].

We report three cases of ruptured traumatic aneurysms of the peripheral anterior cerebral artery after closed head injury. These cases were all young men with closed head injury due to traffic accidents. Consciousness level on admission was coma in all three cases. Case 1 was a 19-year-old man with interhemispheric hematoma on initial CT, then 7 days later his consciousness cleared. However, 14 days later he suddenly lapsed into a deep coma with a severe frontal hemorrhage. Case 2 was a 13-year-old boy. Plain skull films demonstrated a frontal depressed fracture, but CT scan showed no bleeding

Advanced lung cancer inflammation index is associated with mortality in critically ill patients with non-traumatic cerebral hemorrhage.

Existing prognostic tools for non-traumatic intracerebral hemorrhage (NCH) mainly focus on neurological injury severity but overlook systemic factors that cause secondary deterioration. This study aimed to evaluate the Advanced Lung Cancer Inflammation Index (ALI), a biomarker combining nutritional parameters (BMI, albumin) and inflammatory parameters (neutrophil-to-lymphocyte ratio, NLR), as a new prognostic indicator for NCH. It aimed to fill the gap in quantifying the synergy between inflammation and malnutrition in neurocritical care.In this retrospective cohort study, 1502 critically ill

Similarities in the Electrographic Patterns of Delayed Cerebral Infarction and Brain Death After Aneurysmal and Traumatic Subarachnoid Hemorrhage.

While subarachnoid hemorrhage is the second most common hemorrhagic stroke in epidemiologic studies, the recent DISCHARGE-1 trial has shown that in reality, three-quarters of focal brain damage after subarachnoid hemorrhage is ischemic. Two-fifths of these ischemic infarctions occur early and three-fifths are delayed. The vast majority are cortical infarcts whose pathomorphology corresponds to anemic infarcts. Therefore, we propose in this review that subarachnoid hemorrhage as an ischemic-hemorrhagic stroke is rather a third, separate entity in addition to purely ischemic or hemorrhagic strok
