The hidden consequence of intraventricular hemorrhage: persistent cerebral desaturation after IVH in preterm infants.

BACKGROUND: Previous studies describe a short-term decrease in cerebral oxygen saturation (StO2) after intraventricular hemorrhage (IVH) in premature infants; little is known about long-term implications. METHODS: Infants born <30 weeks gestational age (GA) were included. Clinical characteristics, hemoglobin measurements, the highest grade of IVH, and white matter injury (WMI) were noted. NIRS monitoring occurred daily or every other day for 4 weeks; weekly through 36 weeks GA. Recordings were error-corrected before calculation of mean StO2 and fractional tissue oxygen extraction (FTOE). Mean

Cerebral Oxygenation is Associated with Neurodevelopmental Outcome in Infants Born Preterm with Intraventricular Hemorrhage.

OBJECTIVE: To examine how cerebral oxygenation levels monitored by near-infrared spectroscopy (NIRS) are associated with neurodevelopmental trajectories in neonates with intraventricular hemorrhage (IVH). STUDY DESIGN: In this prospective study, 95 neonates born at <34 weeks of gestation with IVH (63 males, median gestational age: 25+6 weeks) between 2013-2023 were included. Regional cerebral oxygen saturation (rScO2) was monitored from IVH diagnosis until term-equivalent age. Analysis included rScO2 levels, duration outside the normal range (<55%; >85%), fluctuations, and cerebral fractional

Uncomplicated intraventricular hemorrhage is not associated with lower estimated cerebral volume at term age.

BACKGROUND AND AIMS: Cerebral lesions detected using cerebral ultrasound (cUS) in very preterm infants are associated with increased risk for neurodevelopmental problems. However, uncomplicated intraventricular hemorrhage (IVH) has no consistent association with poor outcome. In this study we evaluate the effect of uncomplicated IVH on estimated brain volume at term-equivalent age (TEA), using a model based on measurements made from cUS. METHODS: We studied 2 groups of preterm infants (<32 weeks' gestational age (GA)) up to and at TEA: (1) infants with uncomplicated grades 2 or 3 IVH, (2) inf

Placental Pathology, Cerebral Blood Flow, and Intraventricular Hemorrhage in Preterm Infants: Is There a Link?

BACKGROUND: There is growing evidence to support an association between placental inflammation and neurological sequelae of preterm infants. The goal of this study is to evaluate the relationship between placental pathology, post-natal Doppler cerebral resistive indices (RI's), and intraventricular hemorrhage (IVH) in premature infants. METHODS: In a retrospective cohort study, preterm infants born between 23 0/7 and 32 6/7 weeks' gestation at Parkland Hospital were examined with placental pathology and serial ultrasound Doppler to evaluate for the primary outcome of IVH and death. RESULTS:

[Intraventricular hemorrhage due to the rupture of atherosclerotic dissecting aneurysm of the middle cerebral artery].

INTRODUCTION: We present a case of fusiform intracranial aneurysm where, apart from the unusual site, we draw attention to the form of clinical presentation, namely intraventricular haemorrhage. Clinical case. A 68 year-old-man with a history of smoking, hyperuricemia with seizures of gout treated with colchicine and allopurinol, and hypertension treated with captopril. Nine years previously he had a right capsulothalamic haematoma and presented (as a sequela of this) a left sensomotor deficit, with a good functional level. In December 1998 he was admitted for sudden onset of headache and dete

Subarachnoid hemorrhage in the presence of a cerebral arteriovenous malformation and an intraventricular oligodendroglioma: case report.

The authors report a case of a ventricular oligodendroglioma associated with a cortical arteriovenous malformation. The patient presented with subarachnoid hemorrhage. Computed tomographic scan showed an intraventricular hyperdense lesion, mimicking a hematoma. Angiography revealed a superficial arteriovenous malformation in the right parietal lobe, unrelated to ventricular cavities. Both lesions were treated during the same operation. The diagnostic difficulties and the significance of this uncommon association are discussed.

Cold-Induced Reversible Cerebral Vasoconstriction Syndrome Presenting With Intraventricular Hemorrhage in a Normotensive Patient.

Reversible cerebral vasoconstriction syndrome (RCVS) is a transient neurovascular disorder characterized by sudden, severe headaches and reversible multifocal cerebral artery narrowing. While it is often associated with vasoactive substances, emotional stress, or postpartum status, environmental triggers such as cold exposure are rarely reported. We describe the case of a 61-year-old normotensive female who developed a thunderclap headache and vomiting shortly after brief exposure to cold weather. Initial evaluation revealed hypothermia and metabolic acidosis; she was discharged without neuroi

Assessment of intraventricular hemorrhage risk in preterm infants using mathematically simulated cerebral blood flow.

Intraventricular hemorrhage (IVH)4 is one of the most threatening neurological complications associated with preterm birth which can lead to long-term sequela such as cerebral palsy. Early recognition of IVH risk may prevent its occurrence and/or reduce its severity. Using multivariate logistic regression analysis, risk factors significantly associated with IVH were identified and integrated into risk scales. A special aspect of this study was the inclusion of mathematically calculated cerebral blood flow (CBF) as an independent predictive variable in the risk score. Statistical analysis was b

Cerebral oxygenation, extraction, and autoregulation in very preterm infants who develop peri-intraventricular hemorrhage.

OBJECTIVE: To test the hypothesis that near-infrared spectroscopy (NIRS)-determined patterns of regional cerebral oxygen saturation (rScO2), cerebral fractional tissue oxygen extraction (cFTOE), and autoregulatory ability can identify neonates at risk for developing peri-intraventricular hemorrhage (PIVH). STUDY DESIGN: This case-control study is a subanalysis of 30 neonates who developed PIVH >12 hours after admission as part of a lager prospective observational cohort study comprising 650 preterm neonates born at ≤32 weeks' gestational age. PIVH was diagnosed by cranial ultrasound, performe

Acute cerebral ischemia following intraventricular hemorrhage in moyamoya disease: early perfusion computed tomography findings.

The authors present a rare case of an infarction complication 15 days following acute intraventricular bleeding due to moyamoya disease. Before the infarction occurred, perfusion CT imaging disclosed early but reversible ischemic injury on the day of hemorrhage. Dehydration and hypotension are both possibly contributing factors of progressive injury from reversible ischemia due to infarction. Although the patient underwent successful bypass surgery, 1 month after the ictus the neurobehavior evaluation still showed marked executive dysfunction. The authors address that, in hemorrhagic-type moya

A case of pediatric moyamoya disease with severe cerebral vasospasm and delayed cerebral infarction following an intraventricular hemorrhage.

Reports on patients with moyamoya disease presenting cerebral ischemic complications after the onset of intraventricular hemorrhage (IVH) and/or intracerebral hemorrhage (ICH) are limited. Herein, we report a case of a 7-year-old girl with moyamoya disease with severe cerebral vasospasm and delayed cerebral infarction following an IVH. Although the case is rare, the potential for vasospasm-induced cerebral infarction should be carefully considered and thus, intensive treatment should be immediately initiated.

Delayed cerebral ischemia associated with reversible cerebral vasoconstriction in a patient with Moyamoya disease with intraventricular hemorrhage: Case report.

We describe a case of cerebral infarctions caused by transient vasoconstrictions in the posterior circulation 2 weeks after intraventricular hemorrhage without subarachnoid hemorrhage in a 35-year-old patient with Moyamoya disease. To our knowledge, this is the first case report where diffuse segmental vasoconstrictions of the basilar and posterior cerebral arteries were recognized after intraventricular hemorrhage in Moyamoya disease. The patient complained of severe and acute-onset headache 14 days after the intraventricular hemorrhage, which had a different character and severity from the o

Reversal of blood flow in deep cerebral vein in preterm intraventricular hemorrhage: two case reports.

BACKGROUND: Intraventricular hemorrhage during the early stage is a major complication in very low birth weight infants. Elevation of venous pressure is one of the contributing factors. The internal cerebral vein receives most of the venous flow from the subependymal germinal matrix, the most common site of origin of intraventricular hemorrhage. Recently, it has been reported that pulsatile or partially interrupted internal cerebral vein waveforms might also be risk factors for intraventricular hemorrhage in extremely low birth weight infants. Here, we report two cases of partially reversed in

[Intraventricular cerebral hemorrhage in the fetus as a cause of congenital hydrocephalus. A contribution to the origin of congenital hydrocephalus].

Three newborns with posthemorrhagic hydrocephalus after fetal cerebral intraventricular hemorrhage are presented. In all 3 patients hydrocephalus internus was already prenatally diagnosed. Cerebral ultrasound postnatally performed revealed findings of old cerebral intraventricular hemorrhage. Severe factor VIII deficiency was found in one of the patients. The possible connexion of fetal intraventricular hemorrhage with coagulopathy of the fetus has not yet been defined.

Exploring hemoglobin dynamics and scavenging mechanisms in preterm infants and preterm rabbits with cerebral intraventricular hemorrhage.

BACKGROUND: Intraventricular hemorrhage (IVH) is a leading cause of neurodevelopmental impairment in preterm infants, driven by vascular rupture, intraventricular blood accumulation, and secondary injury. Hemolysis releases extracellular hemoglobin (Hb) into the cerebrospinal fluid (CSF), where it promotes oxidative stress, inflammation, and cytotoxicity. The endogenous Hb-scavenging protein haptoglobin (Hp) may mitigate Hb-mediated injury, but its capacity in the immature brain remains unclear. This study aimed to characterize Hb and Hp dynamics in the CSF of preterm infants with severe IVH a
