Bilateral superficial cervical plexus block for parathyroidectomy during pregnancy: A case report.

BACKGROUND: Primary hyperparathyroidism (PHPT) is the most common cause of pregnancy-related hypercalcemia. PHPT can cause maternal and fetal complications in pregnant women. General anesthesia for non-obstetric surgery in pregnant women is associated with maternal hazards and concerns regarding long-term neonatal neurocognitive effects. Surgical removal of the lesion in mid-pregnancy is currently the primary treatment option for pregnant patients with PHPT. However, the blood calcium concentration at which surgery should be considered remains under discussion due to the risk of miscarriage.

Efficacy Comparison Between Interscalene Block with and Without Superficial Cervical Plexus Block for Anesthesia in Clavicle Surgery.

BACKGROUND: Clavicle fractures account for over one-third of shoulder injuries and up to 3.3% of all fractures in adults. While the majority of these fractures can be managed non-surgically, there are instances where surgical intervention is performed. Regional anesthesia (RA) can be a preferred alternative to general anesthesia (GA) to avoid complications and high costs in this surgery. Moreover, the identification of the most optimal approach for RA remains challenging. OBJECTIVES: This study aimed to compare the efficacy of interscalene block (ISB) with and without superficial cervical ple

Demonstration of the spread of injectate with deep cervical plexus block: a case series.

BACKGROUND AND OBJECTIVES: The authors conducted a scanographic study in order to characterize the local anesthetic spread of injectate resulting from a single-injection technique of deep cervical plexus block. METHODS: Six consecutive American Society of Anesthesiologists II and III patients scheduled for elective carotid endarterectomy under regional anesthesia were enrolled. Deep cervical plexus block was placed via an anterolateral approach using a nerve stimulator to guide the injection on contact with the levator scapulae nerve. With specific contractions evoked at a stimulating current

[Block of the cervical plexus for the unipolar exclusion of a gastroplasty after bronchial fistula].

A 46 year old man was operated on for an oesophageal carcinoma. Total oesophagectomy and gastroplasty were carried out. Postoperatively, a fistula developed between the gastroplasty and the right main bronchus. This led to respiratory impairment, with pulmonary infection, pleural effusion and hypoxia; the patient could not remain supine. A cervical oesophagostomy was decided on; to carry it out, a cervical plexus block was preferred to a cervical epidural anaesthesia because of the lesser respiratory and haemodynamic effects of the former technique. The procedure was well supported by the pati

Superficial cervical plexus block for vocal cord surgery in an awake pediatric patient.

UNLABELLED: Medialization thyroplasty is a surgical procedure that decreases the incidence of dysphonia and dysphagia in patients who have vocal cord paralysis. We report a case of a pediatric patient who underwent this procedure with minimal sedation and bilateral superficial cervical plexus blockade. The use of a regional technique provided analgesia while allowing the patient to phonate at the request of the surgeon. IMPLICATIONS: Medialization thyroplasty is a surgical procedure that decreases the incidence of dysphagia and dysphonia in patients with vocal cord paralysis. This procedure i

Recurrent laryngeal nerve blockade in patients undergoing carotid endarterectomy under cervical plexus block.

We report two cases of recurrent laryngeal nerve blockade arising during carotid endarterectomy under cervical plexus anaesthesia. These nerve blocks were thought to be due to the instillation of local anaesthetic. The nerve block in one patient was responsible for a paroxysm of coughing which caused the formation of a large neck haematoma. We believe this to be the first report of local anaesthetic induced recurrent laryngeal nerve blockade leading to such a complication.

Cervical plexus block enhanced pain control for unilateral thermal ablation of thyroid nodules.

BACKGROUND: Despite being minimally invasive, thermal ablation (TA) of thyroid nodules may still cause significant pain during and shortly afterwards. Conventional analgesia relies on perithyroidal local anesthesia (PLA) with or without sedation. The use of cervical plexus block (CPB) has been extensively studied in thyroidectomy, but never studied in TA of the thyroid gland. This study examined whether adding ultrasound-guided CPB to PLA and sedation could further reduce post-operative pain in unilateral TA of thyroid nodules. METHODS: Consecutive patients aged ≥18 years undergoing unilatera

Comparative efficacy of cervical erector spinae plane block and superficial cervical plexus block for postoperative analgesia in anterior cervical discectomy and fusion: A prospective randomized study.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a commonly performed procedure for cervical disc disease; however, significant postoperative pain may hinder recovery and increase opioid consumption. This study compared the efficacy of cervical erector spinae plane block (cESPB) and superficial cervical plexus block (sCPB) techniques in ACDF. METHODS: This prospective, randomized study included American Society of Anesthesiologists (ASA) class I-II patients aged 18 to 65 years who underwent ACDF surgery for cervical (C)5-C6 or C6-C7 disc disease. Patients were randomly assigned t

Ultrasound-Guided Maxillary Nerve Block and Superficial Cervical Plexus Block During Surgery for Maxillary Malignancy: A Case Report.

We report a case of ultrasound-guided craniocervical nerve blocks performed with ropivacaine for perioperative local/regional anesthesia in a patient who underwent right partial maxillary resection and neck dissection under general anesthesia. The patient was an 85-year-old woman with multiple medical comorbidities in whom analgesia using nonsteroidal anti-inflammatory drugs and opioids was expected to increase the risk of postoperative complications. Bilateral ultrasound-guided maxillary (V2) nerve blocks and a right superficial cervical plexus block were performed, which provided adequate pe

Decompression of Ludwig's angina in a pregnant patient under bilateral superficial cervical plexus block.

Ludwig's angina is defined as a potentially lethal, rapidly spreading cellulitis, involving the sublingual and submandibular spaces, and is manifested by a brawny suprahyoid induration, tender swelling in the floor of the mouth, and elevation and posterior displacement of the tongue. During a life-threatening infection such as Ludwig's angina, the mother and foetus are vulnerable to septicemia and asphyxia. We describe a case of decompression of Ludwig's angina in a 28 weeks pregnant patient under bilateral superficial cervical plexus block. The block, coupled with bilateral mandibular nerve b

Aphonia Following Bilateral Cervical Plexus Blocks for an Awake Hemithyroidectomy: A Case Report.

A 35-year-old female patient was scheduled for a left hemithyroidectomy. We performed bilateral cervical plexus blocks with ultrasound guidance for an awake thyroidectomy. Soon after the blocks, she developed a weak voice, which gradually progressed to complete aphonia. Due to her apprehension, general anesthesia was administered. After an uneventful surgery, the patient spontaneously regained her normal voice in the postoperative period. The case report describes a previously unreported complication of aphonia presumably due to bilateral recurrent laryngeal nerve blocks, which might have occu

Superficial Cervical Plexus Block for Postoperative Pain Management in Occipital Craniotomies: A Narrative Review.

Post-craniotomy pain is common yet often sub-optimally managed because systemic opioids can obscure postoperative neurologic examinations. The superficial cervical plexus block (SCPB) has, therefore, emerged as a targeted regional anesthesia option for occipital craniotomies. The SCPB targets the C2-C4 nerves to anesthetize the occipital scalp region, covering the lesser occipital nerve territory that lies within typical posterior scalp incisions. Clinical evidence shows the block is effective in reducing acute postoperative pain after occipital craniotomy and diminishes opioid requirements. S

A Case Series: Ultrasound-Guided Superficial Cervical Plexus Nerve Block For Musculoskeletal Neck Pain.

BACKGROUND: The ultrasound-guided superficial cervical plexus nerve block (SCPNB) is a well-established technique for analgesia for surgical procedures involving the anterolateral neck and it has been increasingly used in the emergency department (ED) for several indications such as neck abscesses, clavicle fractures, and auricular laceration repair. There is, however, limited evidence for its use in patients presenting to the ED with musculoskeletal neck pain. METHODS: This is a case series of 5 patients who presented to the ED with neck pain and were diagnosed with either musculoskeletal ne
