The safety of trial of labor after cesarean section (TOLAC) versus elective repeat cesarean section (ERCS): a systematic review and meta-analysis.

OBJECTIVE: Women's choice of birth following a cesarean delivery either includes a trial of elective repeat cesarean section (ERCS) or a trial of labor after cesarean (TOLAC). No comprehensive overview or systematic summary is currently available. METHODS: EMBASE, PubMed, and the Cochrane Library databases were searched from inception to 1 February 2020. Studies reporting the safety of TOLAC and ERCS in pregnant women with prior cesarean delivery were included. Statistical analysis was performed using RevMan 5.3 and Stata 15.0. Odds ratios (ORs) and 95% confidence intervals (CIs) were adopted

Are multiple repeated cesarean sections really as safe?

AIM: Surgical complications were compared between patients with three or less prior cesarean deliveries and four or more prior cesarean deliveries. MATERIALS AND METHODS: Records of 120 patients who had undergone cesarean sections (CSs) in our Department of Obstetrics and Gynecology, between August and November 2015, were retrospectively studied. Cases were reviewed on the basis of age, type of operation, type of anesthesia, number of CSs, time of hospitalization, and intra-operative and post-operative complications. RESULTS: Cesarean sections had been performed on 62 (51.7%) patients whose

Multipara with utero-vesical fistula following repeat cesarean section: a rare iatrogenic complication.

Utero-Vesical fistulas are the rarest of all urogenital fistulas, with most cases occurring after cesarean section. Its prevalence is increasing worldwide because of the increasing indications of cesarean section. Patient usually presents with urine leak, amenorrhea and cyclic hematuria. Herein, we present a case of patient presenting with complain of severe urinary tract infection following cesarean section. Review and update of recent literature regarding the diagnostic imaging of this entity are described.

Effect of type of anesthesia on blood loss at elective repeat cesarean section.

It has recently been reported that the use of halogenated agents during balanced general anesthesia may result in an increase in blood loss associated with cesarean section. This report has been criticized for failure to control for a variety of other factors that may have contributed to the increased blood loss, particularly the indication for and type of cesarean section. The present study was designed in an attempt to resolve this criticism. Blood loss was evaluated in uncomplicated patients undergoing elective repeat cesarean section under either general anesthesia using a halogenated agen

Repeat cesarean section in a COVID-19 positive mother in the United States.

In our report, we present a case of repeat cesarean section in a 29-year-old Ecuadorian mother who contracted COVID-19 and traveled to the United States during her last trimester of pregnancy. We assembled a multidisciplinary team to safely deliver the mother by cesarean section. She received supportive care for her COVID-19 infection. Infection prevention procedures were based on early available data, and the baby was delivered without complications.

Breastfeeding Initiation in Mothers with Repeat Cesarean Section: The Impact of Marital Status.

BACKGROUND: Approximately 90% of mothers with a primary C-section have a subsequent C-section. To date, research has demonstrated that primary C-sections are associated with breastfeeding noninitiation. However, it is unknown if this association persists after the primary C-section. Furthermore, literature has shown a differing relationship between breastfeeding initiations by marital status. Due to the high proportion of women who give birth while unmarried, investigating differences by marital status will add a significant contribution to breastfeeding literature. This study investigates the

Avoiding prematurity in elective repeat cesarean section. A role for amniotic fluid phosphatidylglycerol.

A prospective study was undertaken in 107 elective repeat cesarean deliveries in which the lecithin/sphingomyelin (L/S) ratio was positive. The frequency of neonatal complications in 87 of these women having positive amniotic fluid (AF) phosphatidylglycerol (PG) was compared with the frequency in the remaining 20 patients exhibiting a negative PG. Hyaline membrane disease (HMD) did not occur in either group. However, neonatal complications and related clinical procedures were significantly increased in the PG-negative patients. These neonatal complications included increased frequency of admis

[2 or more cesarean sections--elective repeat cesarean or vaginal delivery?].

Cesarean section rates have been steadily increasing over the past two decades in most countries of the Western world. The review of the literature suggests that a trial of labor in patients with more than one previous cesarean delivery is appropriate, and that these women should be treated no differently from those who have had only one cesarean delivery. Obstetric management should be individualized after thorough patient counseling. If women are carefully selected for a trial of labor and supervised closely, the risk of serious complications can be minimized and a successful outcome achieve

Prevention of iatrogenic neonatal respiratory distress syndrome: elective repeat cesarean section and spontaneous labor.

Neonatal respiratory distress syndrome (RDS) is an important complication of elective repeat cesarean section. In a review of 68,880 consecutive deliveries, 1,897 women underwent elective cesarean section at a gestation estimated to be greater than or equal to 38 weeks. Seventy-one (4.3%) of the infants of the 1,649 women who were not in labor at the time of delivery developed neonatal RDS. None of the infants of the 248 women who were in spontaneous labor at the time of delivery developed RDS (P less than 0.005). There were no cases of uterine rupture in either group. Of the 71 infants who de

Maternal morbidity following a trial of labor after cesarean section vs elective repeat cesarean delivery: a systematic review with metaanalysis.

This study reviewed maternal morbidity following trial of labor (TOL) after cesarean section, compared with elective repeat cesarean delivery (ERCS). Articles were pooled to compare women planning vaginal birth after cesarean (VBAC) with those undergoing ERCS with regard to maternal morbidity (MM), uterine rupture/dehiscence (UR/D), blood transfusion (BT), and hysterectomy. The former group was subdivided into successful VBAC (S-VBAC) and failed TOL (F-TOL). VBAC was successful in 17,905 of 24,349 patients (73%). MM, BT, and hysterectomy were similar in women planning VBAC or ERCS, whereas UR/

Surprise Finding of Uterine Torsion During a Routine Scheduled Repeat Cesarean Section: A Case Report.

INTRODUCTION: This article describes a case of asymptomatic uterine torsion in the setting of macrosomia and polyhydramnios. While other causes have been reported in the literature, there are no existing cases of uterine torsion associated with fetal macrosomia and polyhydramnios. CASE: This patient had an asymptomatic uterine torsion discovered incidentally during a scheduled repeat cesarean section. A congested vascular lower uterine segment was noticed on entrance into the abdomen. After exteriorization of the uterus, 180° torsion was discovered. If torsion is identified prior to hysteroto

Estimating the prevalence of vaginal birth after cesarean section and repeat cesarean section: An analysis of nationally representative household surveys in 59 low- and middle-income countries.

OBJECTIVE: Little is known about the birthing patterns of patients having experienced a prior cesarean section (CS) in low- and middle-income countries (LMICs). We sought to estimate the prevalence of and characterize trends relating to vaginal birth after cesarean section (VBAC) and repeat CS in LMICs. METHODS: We analyzed 107 nationally representative surveys across 59 LMICs. For countries with population CS rates above 8%, we analyzed repeat CS trends across two or three consecutive surveys and estimated the association between repeat CS and facility level and facility type. RESULTS: The

The effectiveness of preoperative diagnostic methods in predicting intra-abdominal adhesions before repeat cesarean section delivery.

OBJECTIVE: This study aimed to evaluate the effectiveness of skin appearance, striae gravidarum severity, and ultrasonographic "sliding sign" in predicting preoperative adhesions before repeat cesarean section delivery on the same patient and find the most useful one. METHODS: This was a prospective cohort study conducted on pregnant women with a history of cesarean section delivery. Davey's scoring system was used for stria evaluation. The scar was assessed using their visual appearance, and transabdominal ultrasonography was applied to detect sliding sign existence. Surgeons blinded to preo

An evaluation of the relative risks of a trial of labor versus elective repeat cesarean section.

A retrospective study of a trial of labor (TOL) after prior cesarean section was conducted over 18 months in a patient population that is homogeneous with regard to race, socioeconomic class, gestational age, and medical complications. Of 171 women who had undergone previous cesarean sections, 75 were offered a TOL. Thirty-five of these patients had an elective repeat cesarean section, while 40 agreed to a TOL. Thirty-two of the latter patients delivered vaginally (80%). The criteria for offering a TOL and a protocol for TOL are presented. A comparison of the maternal and neonatal morbidity as

Respiratory distress associated with elective repeat cesarean section. A two-year experience in one medical community.

One hundred and forty-five elective repeat cesarean sections performed in one large medical community during a two-year period were studied. Ninety-nine of the patients had no prenatal test for determination of pulmonary or fetal maturity; 18 patients had an amniocentesis for L/S ratio. Only two infants were thought to be delivered prematurely, at 37 weeks gestational age. One of these infants had no respiratory distress. The other infant was delivered before the EDC by menstrual dates because a single ultrasound measurement performed one week before delivery suggested a "term fetus", and this
