Transarterial Therapy for Liver Tumors.

Primary and secondary liver tumors represent a significant global health challenge and are often diagnosed at stages where curative options are not feasible. Transarterial therapies (TATs) have emerged as key locoregional treatment options for patients with unresectable liver tumors, using the liver's unique dual blood supply to selectively target tumor tissue while sparing healthy parenchyma. These therapies include transarterial embolization, transarterial chemoembolization, and transarterial radioembolization, each with distinct mechanisms, technical considerations, and clinical application

Percutaneous liver-directed therapies of intrahepatic cholangiocarcinoma.

Cholangiocarcinoma is a hepatobiliary malignancy which can manifest anywhere along the biliary tree. Intrahepatic cholangiocarcinoma occurs in the liver within or beyond the second order bile ducts. The prognosis for patients with intrahepatic cholangiocarcinoma is poor, even when successfully resected there is a very high rate of local recurrence. The available systemic therapies are currently limited and have high rates of toxicity. Percutaneous and transarterial liver-directed therapies can be used to treat intrahepatic cholangiocarcinoma with results comparable to current standard of care

[Splenic embolization and hepatic chemoembolization: combined transcatheter treatment of hepatocellular carcinoma in cirrhosis with hypersplenism].

Liver cirrhosis with hypersplenism is often associated with HCC. In many such cases, chemoembolization (TACE) may be very difficult because of the high incidence of hemorrhagic complications due to treatment and/or following portal hypertension, as well as for poor hematologic tolerance to antiblastic drugs in cirrhotic patients. Six patients with nodular HCC and cirrhosis (Child B) with hypersplenism were treated by combined TACE and partial splenic embolization (PSE) to reduce splenic size and to improve hematologic and hepatic function rates. Actual and long-lasting (up to 6 months since TA

Current evidence and the potential role of proton beam therapy for hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) is a leading cause of cancer-related death, and external beam radiation therapy has emerged as a promising approach for managing HCC. Proton beam therapy (PBT) offers dosimetric advantages over X-ray therapy, with superior physical properties known as the Bragg peak. PBT holds promise for reducing hepatotoxicity and allowing safe dose-escalation to the tumor. It has been tried in various clinical conditions and has shown promising local tumor control and survival outcomes. A recent phase III trial demonstrated the non-inferiority of PBT in local tumor control com

[Exploration and application of nanotechnology in precision transarterial chemoembolization for hepatocellular carcinoma].

Transarterial chemoembolization (TACE) is a first-line therapy for intermediate-to-advanced- stage hepatocellular carcinoma, but its clinical efficacy is often limited by incomplete embolization, systemic toxicity of chemotherapeutic drugs, and tumor recurrence and metastasis caused by the postoperative microenvironment. The emergence of nanotechnology provides a novel solution to overcome these bottlenecks. This article aims to review new strategies and clinical translations of nanoparticles as intelligent carriers in enhancing the TACE efficacy. Furthermore, it focuses on the latest advances

Advances in radiation therapy for HCC: Integration with liver-directed treatments.

HCC is the fourth leading cause of cancer-related mortality with increasing incidence worldwide. Historically, treatment for early disease includes liver transplantation, surgical resection, and/or other local therapies, such as thermal ablation. As a result of technical advances and high-quality prospective data, the use of definitive external beam radiotherapy with ablative doses has emerged. Intermediate-stage disease has been generally addressed with arterially directed therapies (eg, chemoembolization or radioembolization) and external beam radiotherapy, while advanced stages have been ad

Image-Guided Percutaneous and Transarterial Therapies for Primary and Metastatic Lung Cancer.

Lung cancer is the leading cause of cancer mortality in the world. A significant proportion of patients with lung cancer are not candidates for surgery and must resort to other treatment alternatives. Rapid technological advancements in fields like interventional radiology have paved the way for valid treatment modalities like image-guided percutaneous and transarterial therapies for treatment of both primary and metastatic lung cancer. The rationale of ablative therapies relies on the fact that focused delivery of energy induces tumor destruction and pathological necrosis. Image-guided percut
