Child homicide--the extreme of child abuse.

OBJECTIVE: To review cases of fatal child abuse, looking for any characteristic features of these cases. DESIGN AND SETTING: A case review of all childhood deaths at The Children's Hospital, Camperdown, between January 1976 and December 1990. Cases with a definite diagnosis of child abuse were extracted. Cases where there was only a suspicion of child abuse as the cause of death were not included. SUBJECTS: Seventeen cases of fatal child abuse were reviewed with emphasis on autopsy findings and sociological data. RESULTS: All children who died were less than three years of age; nine were un

Child Abuse and Neglect.

Physical, sexual, and emotional abuse and various forms of neglect of children are associated with substantially increased risk for concurrent and subsequent psychopathology and are among the common problems encountered by clinicians in many clinical settings. Such cases pose additional challenges for clinicians because of the many complex family and system forces that engulf these children and their families. Assessing maltreated children generally requires more time than evaluations of children who have not experienced maltreatment. Young children, who experience the highest rates of maltrea

[Female child sexual abuse].

INTRODUCTION AND AIM: The prevalence of child sexual abuse is 12-13% worldwide (18% by girls, 8% by boys). The exact knowledge of sexual abuse and the spread of the adequate medical diagnosis is an essential medical, social and national requirement. In our present study we examine the medical diagnosis of female child sexual abuse. METHOD: Selective literature research in the available international and domestic databases. RESULTS: Majority of children assessed for suspected sexual abuse have normal genital and anal findings. Contrary to popular belief, the majority of child sexual abuse is

Imaging of child abuse.

Diagnostic imaging plays a fundamental role in the evaluation of suspected physical abuse. Judicious application of modern imaging techniques to cases of suspected abuse aids in early and accurate diagnosis, leading to appropriate measures to protect all family members at risk of serious injury.

Understanding fatal child abuse.

Medical, social service and coroner reports were reviewed for 14 cases of fatal child abuse and neglect identified at a children's hospital from 1988-1992. Twelve cases involved physical abuse and two neglect. The median age was 6.5 months (range 24 days to 3 years). Six families (43%) had prior protective service involvement; however, four of the referrals involved a sibling. Only two of 12 physical abuse victims had a history of a prior suspicious injury. Clinical and postmortem findings are presented. The cause of death in all physically abused patients was blunt impact head injury; one als

[Child abuse by intoxication].

Child abuse by toxic substances is not easy to identify due to both the lack of signs of physical violence and the high incidence of accidental poisonings. Twelve cases of documented abuse, out of 6175 poisonings in children aged 0-13 years, were registered during 1975-1990 at Genova, Torino and Trieste Children Hospitals. In other 139 children the suspicion of abuse couldn't be confirmed. An early suspicion of abuse, is needed to prevent abuse and help the child and his family.

Hyperactivity and child abuse.

The role of a child in his/her physical abuse was reviewed with particular attention to hyperactive children. A retrospective analysis of the records of children referred to a university-based hyperactive children's clinic revealed that children both with and without Attention Deficit Disorder (ADD) were more often physically abused than children in the general population. The period prevalence of physical abuse in children with ADD did not differ significantly from that in children without ADD. This suggests that hyperactivity may either contribute to, or result from, physical abuse.

Child abuse in Nigeria.

Although child abuse occurs in Nigeria, it has received little attention. This is probably due to the emphasis placed on the more prevalent childhood problems of malnutrition and infection. Another possible reason is the general assumption that in every African society the extended family system always provides love, care and protection to all children. Yet there are traditional child rearing practices which adversely affect some children, such as purposeful neglect or abandonment of severely handicapped children, and twins or triplets in some rural areas. With the alteration of society by rap

Child abuse: a study of the child's perspective.

This study ascertained the child's response to various types of abuse and neglect. With interviews and questionnaires for the staff of a child psychiatric unit, the mothers and the children involved, we assessed the severity, frequency and duration of physical abuse, verbal abuse, physical neglect, emotional neglect or sexual abuse of children. The physically abused children accepted blame for mild but not severe mistreatment. Verbally and sexually abused children did not believe it was their fault if the mistreatment was moderately extensive but did if it was mild or severe. The relationship

Phytophotodermatitis mimicking child abuse.

A 13-month-old girl presented with red finger marks on both shoulders thought initially to be secondary to child abuse. The appearance of the marks was not typical of bruising of the stated age and there were no social concerns or other medical features of child abuse. Direct questioning revealed that the mother had been gardening on a sunny day and had picked the child up prior to the appearance of the marks. The marks are thought to represent a phytophotodermatitis. Many paediatric skin conditions can mimic child abuse. Recognition of this further possibility will prevent avoidable errors of

Child abuse by scalding.

Physical abuse of children by burning is a serious crime that leaves the youngest of our children with permanent physical and emotional scars. The victims tends to be less than 2 years of age. Burn-abused children usually suffer from a spectrum of physical, psychologic, and nutritional neglect. Burn abuse is costly in terms of pain, suffering and health care. Up to 15% of acutely injured children seen in emergency departments, admitted or not, have been abused. Burns were involved in up to 22% of physical abuse cases. Up to 26% of pediatric burn admissions were the result of abuse. The most co

Child abuse by drowning.

Drowning as a form of subtle fatal child abuse is difficult to distinguish from accidental immersion or from sudden unexpected natural death when the circumstances of immersion are concealed. Homicidal drownings are unwitnessed, usually occurring in the home, and the victims are young, either infants or toddlers. Accidental drownings are more likely to involve toddlers or older children in public areas such as swimming pools, drainage ditches, lakes, and rivers. This is especially true in rural areas. In cities, bathtubs remain a major site of accidental childhood drownings. Perpetrators of de

Evaluating the risk of child abuse: the Child Abuse Risk Assessment Scale (CARAS).

The present study developed the Child Abuse Risk Assessment Scale (CARAS), an actuarial instrument for the assessment of the risk of physical child abuse. Data of 2,363 Chinese parents (47.7% male) living in Hong Kong were used in the analyses. Participants were individually interviewed with a questionnaire assessing their perpetration of child abuse and some theoretically or empirically tested factors associated with child abuse. Using the split-half validation procedure, the 5-factor, 64-item CARAS was created and validated. When applying to the second half of the split sample, the CARAS had

Childhood history of abuse and child abuse screening.

Although a childhood history of abuse is related to parental child abuse, many parents with a history of abuse are not abusive. To determine the effects of a childhood history of abuse on adult child abuse potential, a modified Conflict Tactics Scale (CTS) and the Child Abuse Potential (CAP) Inventory were administered to matched groups of physically abusive mothers with a childhood history of abuse, nonabusive comparison mothers with a childhood history of abuse, and nonabusive comparison mothers without a childhood history of abuse. The modified CTS asked about childhood events and was used

Changes in child placement after child abuse pediatrics consultation for suspected physical abuse.

BACKGROUND: The association between child abuse pediatric (CAP) assessments and child welfare outcomes is unknown. OBJECTIVE: To determine the association between a CAP determination of the likelihood of physical abuse and change in child placement. We hypothesized that child race would be associated with CAP determination of abuse likelihood and child welfare outcomes. PARTICIPANTS AND SETTING: Children under age ten years with in-person CAP consultation and referral to child protective services for suspected physical abuse at a U.S. pediatric referral center participating in CAPNET, a CAP
