Managing children and adolescents on parenteral nutrition: Challenges for the nutritional support team.

Managing infants, children and adolescents, ranging from premature infants to 18-year-old adolescents, on parenteral nutrition (PN) is a challenge. The ability of children to withstand starvation is limited and, unlike adults, children require nutrition for growth. PN in children is often required secondary to a congenital bowel problem rather than because of an acquired condition. Conditions requiring PN include motility disorders, congenital disorders of the intestinal epithelium and short-bowel syndrome (SBS). Intestinal failure may be temporary and children with SBS may be weaned from PN.

Protocol for the nonoperative treatment of obstructing intramural duodenal hematoma during childhood.

A prospective plan for the nonoperative treatment of obstructing intramural duodenal hematoma was developed to (1) promptly establish the diagnosis and rule out transmural leaks by obtaining a contrast roentgenogram on admission and to subsequently study evolution of the obstructing intramural duodenal hematoma with sequential roentgenograms at 5 to 7 day intervals; (2) identify associated pancreatic injury with ultrasonography and serum amylase and lipase determinations; and (3) to determine effectiveness of nasogastric suction and total parenteral nutrition. Twelve children, who ranged in ag

[Hypovitaminosis D, the osteomalacia syndrome and calcium homeokinesis in childhood].

The author pays attention to hypovitaminosis D in childhood. Despite efforts to eliminate it, its estimated incidence in the first and second year of life is 5-20%. This relatively high incidence in our population as well as in other countries makes hypovitaminosis D one of the most frequent disorders of the child organism. In addition to nutrition-conditioned rickets more than 50 different diseases have been described with rachitic and osteomalatic manifestations.

Community-based approaches to address childhood undernutrition and obesity in developing countries.

Community-based approaches have been the mainstay of interventions to address the problem of child malnutrition in developing societies. Many programs have been in operation in several countries for decades and originated largely as social welfare, food security and poverty eradication programs. Increasingly conceptual frameworks to guide this activity have been developed as our understanding of the complex nature of the determinants of undernutrition improves. Alongside this evolution, is the accumulation of evidence on the types of interventions in the community that are effective, practical

Effects of an individualized nutrition intervention on the respiratory quotient of patients with liver failure.

BACKGROUND AND OBJECTIVES: Malnutrition and energy metabolism disorders are characterized by a low respiratory quotient in patients with liver failure and often lead to poor prognosis. Therefore, early nutrition interventions are crucial for patients with liver failure to ameliorate abnormal metabolic status and malnutrition. This study explored the effect of an individualized nutrition intervention on the respiratory quotient of patients with liver failure. METHODS AND STUDY DESIGN: An individualized 2-week nutrition intervention was conducted on patients with nutritional risk caused by live

Child health and nutritional status in Ulaanbaatar, Mongolia: a preliminary assessment.

Over 85% of Mongolia's foreign trade and development aid, which formerly came from the USSR, have abruptly ceased causing shortfalls in almost all sectors. The UNICEF Mongolia Country Program and the East Asia and Pacific Regional Office (UNICEF/EAPRO) realized that Mongolian children are likely to suffer the most as reduced income and food availability aggravate problems associated with malnutrition. Hence, from 16 June-7 July 1992, a team from the Institute of Nutrition at Mahidol University, Thailand, collaborated with local UNICEF personnel and government health officials in designing and

Insights on food and nutrition in the Federated States of Micronesia: a review of the literature.

BACKGROUND: Nutrition-related disorders, including vitamin A deficiency (VAD) and chronic diseases, are serious problems in the Federated States of Micronesia (FSM). Many suggest that these disorders are new problems related to dietary and lifestyle changes. In the past four decades, imported foods, such as white rice, flour, sugar, refined foods and fatty meats, have increasingly replaced local foods in the diet. AIM: A literature review was conducted to understand underlying issues related to dietary change and obtain insights for nutrition research and interventions. METHOD: Published and

[Kwashiorkor after an exclusion diet for eczema].

BACKGROUND: Nutritional disorders observed in children with eczema may be aggravated by inappropriate diet, sometimes with dramatic effects. CASE REPORT: A child aged 3 years 7 months presented a state of severe malnutrition with edema (kwashiorkor). The child had chronic eczema and had been given a highly restrictive diet based on a principle of exclusion without medical advise. Cure was achieved without sequelae with a normal diet. DISCUSSION: This case illustrates the fragility of nutritional status in children with chronic widespread skin disease due to the risk of cutaneous loss of prot

Malnutrition in infants receiving cult diets: a form of child abuse.

Severe nutritional disorders, including kwashiorkor, marasmus, and rickets, were seen in four children and were due to parental food faddism, which should perhaps be regarded as a form of child abuse. All disorders were corrected with more normal diets and vitamin supplements. In view of the potentially serious consequences of restricted diets being fed to children, families at risk should be identified and acceptable nutritional advice given. When children are found to be suffering from undernutrition due to parental food faddism a court order will normally be a necessary step in providing ad

Chronic Non-infectious Osteomyelitis Mimicking Scurvy as the Presenting Sign of Crohn's Disease: Case Report.

Chronic non-infectious osteomyelitis (CNO) is a rare, inflammatory process associated with pediatric inflammatory bowel disease (IBD). Signs and symptoms of CNO parallel scurvy, a nutritional deficiency that can affect children with autism spectrum disorder (ASD). This is the first report of a child initially thought to have scurvy, then subsequently diagnosed with CNO as the presenting manifestation of Crohn's disease. This case enhances the literature elucidating extra-intestinal manifestations of IBD and pediatric nutritional deficiencies.
