This parameter reviews the current status of reactive attachment disorder with regard to assessment and treatment. Attachment is a central component of social and emotional development in early childhood, and disordered attachment is defined by specific patterns of abnormal social behavior in the context of "pathogenic care." Clinically relevant subtypes include an emotionally withdrawn/inhibited pattern and a socially indiscriminate/disinhibited pattern. Assessment requires direct observation of the child in the context of his/her relationships with primary caregivers. Treatment requires esta

Reviewed in this article is research on children with reactive attachment disorder (RAD) who exhibit specific patterns of socially aberrant behavior resulting from being maltreated or having limited opportunities to form selective attachments. There are no data explaining why 2 different patterns of the disorder, an emotionally withdrawn-inhibited pattern and an indiscriminate-disinhibited pattern, arise from similarly aberrant environments. In this article, we consider whether temperamental differences might contribute to the different manifestations of reactive attachment disorder (RAD) in t

BACKGROUND: The aim of this study is to examine the prevalence of mental disorders in 6- to 12-year-old foster children and assess comorbidity and risk factors.
METHODS: Information on mental health was collected from foster parents and from teachers using Developmental and Well-Being Assessment (DAWBA) Web-based diagnostic interview. Child welfare services provided information about care conditions prior to placement and about the child's placement history.
RESULTS: Diagnostic information was obtained about 279 (70.5%) of 396 eligible foster children. In total, 50.9% of the children met the

Autism spectrum disorder (ASD) occurs in 1 in 68 births, preferentially affecting males. It encompasses a group of neurodevelopmental abnormalities characterized by impaired social interaction and communication, stereotypic behaviors and motor dysfunction. Although recent advances implicate maternal brain-reactive antibodies in a causative role in ASD, a definitive assessment of their pathogenic potential requires cloning of such antibodies. Here, we describe the isolation and characterization of monoclonal brain-reactive antibodies from blood of women with brain-reactive serology and a child

Complex trauma is defined as repeated or prolonged exposure to traumatic events perpetrated within the caregiving relationship during early childhood. Diagnostic decision making is challenging for this vulnerable population, given the widespread impact of these experiences across domains of developmental, social, emotional, and behavioral functioning. One domain, attachment, receives considerable attention for youth within the child welfare system, leading to frequent diagnosis of attachment disorders (i.e., reactive attachment disorder and disinhibited social engagement disorder). This is con

Acquired Brown's syndrome is a disorder of ocular mobility characterized by the inability to elevate the affected eye in full adduction owing to inflammatory tenosynovitis of the superior oblique tendon. We describe a child who developed Brown's syndrome as a complication of poststreptococcal reactive arthritis.

Parent history of psychopathology is an established marker of children's own risk for later disorder and can therefore be used as a means of validating other risks, such as child temperament. While associations between children's temperament and parent psychopathology have been reported, few studies have used observational measures of child temperament or examined trait interactions, particularly between children's affective and regulatory traits such as effortful control (EC). In this bottom-up family study of 968 three-year-olds and their parents, we examined interactions between preschooler

Anxiety disorders become more prevalent in females from middle childhood onwards. Yet it is likely that predictors for developing these disorders occur earlier, with emerging evidence of sex differences. We examined whether early life predictors, including maternal depression, placental cortisol bioavailability, and infant cortisol reactivity, predict sex differences in anxiety disorders at age 8. Data are from 149 mothers recruited in the Mercy Pregnancy and Emotional Wellbeing Study (MPEWS) before 20 weeks' gestation and their children followed to 8 years. Maternal measures included the Stru

PURPOSE: Co-sleeping is common in children with co-morbid conditions. The aim of the study was to analyze the prevalence and determinants of parent-child co-sleeping in children with co-morbid conditions and sleep-disordered breathing and the impact on parental sleep.
METHODS: Parents of consecutive children undergoing a sleep study filled in a questionnaire on co-sleeping.
RESULTS: The parents of 166 children (80 boys, median age 5.7 years (0.5-21) participated in this study. The most common co-morbid conditions of the children were Down syndrome (17%), achondroplasia (11%), and Chiari malf

Reactive Attachment Disorder (RAD) is a childhood disorder characterized by extremely inappropriate social relating across a variety of interactions that must be present by age 5. Although children diagnosed with RAD appear to demonstrate significantly more behavioral problems and psychosocial difficulties than children without RAD, there have been few examinations of empirically informed treatments for this disorder. One avenue that may be particularly promising is the use of treatments that have been successfully used to decrease similar problematic behaviors in children. The present case st

Studies suggest that higher parent responsiveness is associated with higher child language abilities. Infants and toddlers later diagnosed with autism spectrum disorder (ASD) often display hyporeactivity to sensory stimuli, which has also been associated with lower child communication abilities and lower parent responsiveness. Yet, whether parent responsiveness mediates the relationship between child hyporeactivity and later communication outcomes remains unexplored. This study is a secondary data analysis which includes children (n = 83; 56 males) identified as at elevated likelihood of later

BACKGROUND: Children with attachment disorder show prosocial behavior problems. Children with a reactive attachment disorder show inhibited and emotionally withdrawn behavior. Consequently, these children typically display prosocial behavior problems. However, the underlying mechanism between reactive attachment disorder and prosocial behavior problems is still unclear and findings in literature are mixed.
METHODS: The current study investigated the role of children's attachment representations in this association. Attachment representations reflect knowledge about a cognitive script regardin

The pandemic produced by COVID-19 has a significant impact on society and has also affected childhood and adolescence, which, in general, has been the most silenced group. This article addresses the contents that affect the mental health of children and adolescents in the heart of the COVID-19 pandemic, it is located in the "fourth wave of care" in category F54 of the ICD-10 ("Behavioral and psychological factors in disorders or diseases classified elsewhere"). The presentation mechanism is: reactive-adaptive to the pandemic; triggering factor, before a previous affective-emotional instability

INTRODUCTION: The eleventh revision of the International Classification of Diseases (ICD-11) is planned to be published in 2018. So called, "beta version" of the chapter of mental and behavioral disorders (ICD-11) is already available and it is considered that there will be no significant deviations in the final version. The DSM-5 was released in 2013. Changes related to mental disorders in child and adolescent psychiatry have been made in both of these classifications. To identify changes in the classifications of mental disorders in childhood and adolescent age in beta version of ICD-11 and

Attention modulation when confronted with emotional stimuli is considered a critical aspect of executive function, yet rarely studied during childhood and adolescence, a developmental period marked with changes in these processes. We employed a novel, and child-friendly fMRI task that used emotional faces to investigate the neural underpinnings of the attention-emotion interaction in a child and adolescent sample (n=23, age M=13.46, SD=2.86, range=8.05-16.93 years). Results implied modulation of activation in the orbitofrontal cortex (OFC) due to emotional distractor valence, which marginally

The sexually abused girls in this study were a sub-sample of a group of girls referred to a Regional Centre for Psychotherapy for the whole of London, North Thames. An inclusion criterion was that they were psychologically symptomatic and so it is likely that they were more problematic cases causing concern in their locality. The control clinical group consisted of referrals to local Child and Family consultation services, were an opt-in matched sample and not a total clinic referral sample. In addition, the reasons for referral covered both child disorder and family problems. It is, therefore

BACKGROUND: Chronic early trauma alters children's stress reactivity and increases the prevalence of anxiety disorders; yet the neuroendocrine and immune mechanisms underpinning this effect are not fully clear. Animal studies indicate that the mother's physiology and behavior mediate offspring stress in a system-specific manner, but few studies tested this external-regulatory maternal role in human children exposed to chronic stress.
METHODS: We followed a unique cohort of children exposed to continuous wartime trauma (N = 177; exposed; N = 101, controls; N = 76). At 10 years, maternal and ch

This article presents a clinical analysis of the first two sessions of interactive guidance therapy conducted with a mother diagnosed with complex post-traumatic stress disorder. Using a clinical case study, it demonstrates how the child's everyday behaviors can reactivate the mother's traumatic experiences and influence mother-child interactions at an early stage. Interactive guidance appears to be a relevant approach for making these dynamics visible in the here and now, supporting parental emotional regulation, and fostering maternal sensitivity. The text also examines the limitations of th

Selective mutism (SM) is an anxiety disorder in which a child fails to speak in some situations (e.g., school) despite the ability to speak in other situations (e.g., home). Some work has conceptualized SM as a variant of social anxiety disorder (SAD) characterized by higher levels of social anxiety. Here, we empirically tested this hypothesis to see whether there were differences in social anxiety (SA) between SM and SAD across behavioral, psychophysiological, self-, parent-, and teacher-report measures. Participants included 158 children (Mage = 8.76 years, SD = 3.23) who were classified int
