Child Welfare System Involvement in the United States: 2016-2023.

BACKGROUND AND OBJECTIVES: Childhood and family contexts and understanding of communicable respiratory diseases shifted during and after the COVID-19 pandemic. Against this backdrop, competing hypotheses have emerged around children's risks of maltreatment and system contact, highlighting a need for greater understanding of trends in child welfare system involvement before, during, and since the COVID-19 pandemic. This study estimates the cumulative prevalence of several stages of child welfare contact for all US children and across racial and ethnic groups between 2016 and 2023. METHODS: We

Childhood abuse history, secondary traumatic stress, and child welfare workers.

Social workers are exposed to trauma vicariously through the trauma of their clients. This phenomenon, called secondary traumatic stress, vicarious traumatization, or compassion fatigue, presents a risk of negative personal psychological consequences. Based on a sample of 166 child welfare workers and using standardized measures, the study findings document the link between a personal history of primary trauma, childhood abuse or neglect, and the heightened risk for secondary traumatic stress in child welfare workers.

Drug Use and Human Milk: Legal and Child Welfare Considerations.

INTRODUCTION: Preventing new parents who use drugs from bonding with and bodyfeeding their babies undermines public health. Because U.S. culture treats substance use as a moral failing rather than a health concern, punitive responses to perinatal substance use continue to dominate. This is particularly true for families of color, who are more likely to be targeted by the family regulation system. These approaches fail to protect families, and cause their own, separate harm. This article will lay out existing evidence surrounding bodyfeeding. Then, it will debunk harmful mythologies about drug

Sexual victimization and intellectual disabilities among child welfare involved youth.

BACKGROUND AND OBJECTIVE: Youth with intellectual disabilities involved in child welfare systems are at greater risk of sexual victimization than youth who have not been investigated for child maltreatment. This study addresses a gap in the literature regarding older youth with intellectual disabilities who are sexually victimized and pushed to engage in transactional sex while they are transitioning from child welfare systems involvement. It does so by examining risk and protective factors at the individual, micro, exo, and macro systems levels. PARTICIPANTS AND SETTING: This study included

Commercial Sexual Exploitation of Children and State Child Welfare Systems.

In several states, commercial sexual exploitation of children (CSEC) is now a reportable child abuse offense. Illinois has taken the lead in tackling the issue and the Illinois experience illuminates valuable lessons. This article delineates the protection, practice, and policy implications that evolve when CSEC falls under a state child welfare system. The specific aims are to (a) discuss CSEC, its victims, risks, harms, and challenges inherent in providing effective care; (b) use Illinois as an exemplar to explicate the consequences and implementation challenges of establishing a state repor

Multilevel correlates of behavioral resilience among children in child welfare.

Resilience, defined as positive adaptation and functioning following exposure to significant adversity, is an important topic of investigation in child welfare. The current study used data from the Ontario Looking After Children (OnLAC) project to estimate the prevalence of behavioral resilience (i.e., lower frequency of conduct and emotional problems, higher frequency of prosocial behavior) in 531 5-9 year olds living in out-of-home care, and to determine how behaviorally-resilient children are functioning in other domains (i.e., peer relationships and academic performance). Furthermore, hier

Family Safety Nets and Child Outcomes of Families Involved with the U.S. Child Welfare System.

A strong safety net, comprised of formal and informal supports, promotes wellbeing among low-income families, and these supports can operate synergistically. Despite the known potential power of safety nets to help families in times of need, research on what safety nets look like among families in the child welfare system (CWS) is lacking. Informed by the family stress model and with a nationally representative dataset of families involved with the CWS (N = 5,489), we used latent profile analysis to examine dimensions of family safety nets at baseline and tested the extent to which profiles we

Child maltreatment and behavioral health outcomes in child welfare: Exploring the roles of severity and polyvictimization.

BACKGROUND: Child maltreatment and polyvictimization are known risk factors for long-term detrimental health and development outcomes, including behavioral health challenges. However, effects from specific types and combinations of maltreatments are unclear. This study examined the association between maltreatment or polyvictimization and behavioral health in a child welfare sample. PARTICIPANTS AND SETTING: Medical records of children with child welfare involvement with at least one behavioral health condition (i.e., mental, behavioral or neurodevelopmental disorder, ICD-10 F01-F99) between

Multiple Family Groups to reduce child disruptive behavior difficulties: moderating effects of child welfare status on child outcomes.

Children who remain at home with their permanent caregivers following a child welfare (CW) involvement (e.g., investigation, out-of-home placement) manifest high rates of behavioral difficulties, which is a risk factor for further maltreatment and out-of-home placement if not treated effectively. A recently tested Multiple Family Group (MFG) service delivery model to treat youth Disruptive Behavior Disorders (DBDs) has demonstrated effectiveness in improving child behavior difficulties among hard-to-engage, socioeconomically disadvantaged families by addressing parenting skills, parent-child r

Rates of psychotropic medication use over time among youth in child welfare/child protective services.

OBJECTIVE: The aim of this study was to examine rates of psychotropic medication use over time among a national probability sample of youths involved with child welfare/child protective services (CW/CPS) in the National Survey of Child and Adolescent Well-Being (NSCAW). METHODS: Growth mixture modeling was used to classify 2,521 youths into groups based on individual medication use trajectories. Determinants associated with groupings were examined using logistic regression. RESULTS: Overall, 22% of youths used medications over 3 years. Three groups were identified: (1) Low medication use (85

Prevalence and Treatment of Maternal Substance Use Disorder in Child Welfare.

IMPORTANCE: Parental substance use is a primary driver of child welfare system involvement, and child welfare services primarily seek to address caregiver challenges, including connecting parents to treatment. Although research highlights that formal child welfare services connect children to health care professionals, less is known about how the system affects caregivers' substance use treatment. OBJECTIVE: To examine the prevalence of maternal substance use disorder (SUD) in the Pennsylvania child welfare system and the association between formal child welfare system response and mothers' t

Social Determinants of Health and Child Welfare Outcomes in NCANDS Data.

Social Determinant of Health (SDoH) Indices, such as the Social Vulnerability Index (SVI) or the Area Deprivation Index (ADI), are increasingly used in child maltreatment research either to explore the degree which SDoH is associated with maltreatment or as a control variable to more accurately understand other associations of interest. To date, however, there have been no published explorations of which SDoH measures are most predictive of child maltreatment outcomes. This paper compares a range of publicly available SDoH measures in terms of their ability to explain variability in rates in N

Profiles and predictors of behavioral resilience among children in child welfare.

Children living in out-of-home care have experienced a multitude of adversities, often resulting in compromised functioning. The current study used Ontario Looking After Children (OnLAC) project data to estimate developmental trajectories of behavioral outcomes (i.e., conduct and emotional problems) over a 4-year period (i.e., ages 6-10 to 9-13) in 313 children living in out-of-home care. Predictors measured at baseline (e.g., sex) and across the subsequent 4-year period (e.g., parenting practices) were also investigated. Findings indicated that 64.2% and 58.6% followed resilient trajectories

Suicide and suicide attempts in children and adolescents in the child welfare system.

BACKGROUND: Few population studies have examined the psychiatric outcomes of children and adolescents in the child welfare system, and no studies have compared outcomes before and after entry into care. Our objective was to assess the relative rate (RR) of suicide, attempted suicide, admission to hospital and visits to physicians' offices among children and adolescents in care compared with those not in care. We also examined these outcomes within the child welfare population before and after entry into care. METHODS: We used population-level data to identify children and adolescents 5 to 17

Providing better opportunities for older children in the child welfare system.

A growing number of children over age 10 reside in and emancipate from foster care every year. Older children face many of the same challenges as younger children, but they also have unique developmental needs. This article discusses older children in the child welfare system and finds: Approximately 47% of children in foster care are over age 11, and in 2001, 20% of children leaving foster care were over age 16. Older children need permanency, stability, and a "forever family." Maintaining connections with siblings and other kin can be a crucial resource for older children as they transition

Validation of the Strengths and Stressors Tracking Device with a child welfare population.

The Strengths and Stressors Tracking Device (SSTD) is a rapid assessment measure of family well-being that assesses the particular strengths and needs of families at intake to help guide case planning and evaluate the effectiveness of treatment. The device assesses families from an ecological perspective in the domains of environmental conditions, social support, caregiver skills, and child well-being, and may be used at multiple points during treatment to assess change. SSTD has high internal consistency in all domains, distinguishes between physical abuse and neglect, and is sensitive to spe

What are the effects of anonymity and secrecy on the welfare of the child in gamete donation?

A perennial concern when using donated gametes in infertility treatment is the effect on the child and his/her family of the traditional anonymity of the donor, and of the secrecy of the procedure. As all involved, from potential parents to carers, wish to maximize the 'welfare of the child' born of gamete donation, conflicting attitudes, sometimes translated into diverging legislations in Europe, are analysed. In the face of the lack of evidence on the consequences of secrecy or openness, it is concluded that future parents are best placed to decide on this matter for the potential children.

Geographic Inequities in Child Welfare Permanency: A Rural-Urban-Mixed Analysis.

Geography has received increasing attention from child welfare researchers. A growing body of research demonstrates that child welfare involvement differs systematically across rural and urban contexts. Beyond initial involvement, this study examines geospatial disparities in permanency outcomes and the moderation effect of race. Using data from AFCARS, we followed a cohort of children who entered foster care in FY 2016 and tracked their permanency outcomes through FY 2019. We conducted two sets of multinomial logistic regression analyses with and without the reunification group, respectively.
