Cholelithiasis is being recognized more frequently in the pediatric population. Although hemolytic diseases have an identifiable etiology, the origin of gallstones in approximately 80% of pediatric patients remains unknown. Infants with cholelithiasis can be managed expectantly unless symptoms develop because many of these stones will resolve spontaneously. The 2- to 12-year age group is more likely to present with chronic cholecystitis and will require cholecystectomy at some point, since there is little chance of spontaneous resolution of these gallstones. Teenagers tend to present in a simi

Traditional cholecystectomy has been the standard surgical treatment of the gallstone disease for more than 100 years. The technical development led to a new surgical procedure and its rapid acceptance. This is laparoscopic cholecystectomy. Its application is becoming widespread in therapy too. But most of the surgeons are lack of technical experiences in this field. Currently it restricts the indications those are anyway the same of standard cholecystectomy. Besides its many advantages, laparoscopic cholecystectomy has its own disadvantages and being an invasive procedure, there are possibili

The experience with "open" biliary tract surgery is documented in this report in an effort to provide a basis for comparison with the more recent experience with laparoscopic cholecystectomy. During the years 1932 through 1984, 14,232 patients were surgically treated for nonmalignant biliary tract disease at The New York Hospital-Cornell Medical Center. Among these patients, the mortality rate was 1.7% (237 postoperative deaths). Cholecystectomy was performed in 10,749 patients, and the mortality rate was 0.6% (60 postoperative deaths). Of this group of patients, chronic cholecystitis was pres

A consecutive series of 220 laparoscopic cholecystectomies (June 1990 to May 1991) is presented. These were the author's initial experience of the technique. Procedures were elective (205) and acute (15), including 3 gangrenous cholecystitis and 4 empyema. There were 166 females and 34 males, 12-75 years, weighing 44-115 kg. Forty-eight patients had prior abdominal surgery. Two hundred and eleven patients had successful laparoscopic cholecystectomies, 6 open cholecystectomies and 3 mini-laparotomies to remove split stones. None of the last 120 cases were opened. Operating time ranged from 20 m

Laparoscopic cholecystectomy is a newly developed technique for removing the gallbladder. Its future is very promising and this operation will probably become the preferred method of cholecystectomy for most patients. However, the limitations of laparoscopic cholecystectomy should be realized and great care must be taken to avoid technical complications. If laparoscopic cholecystectomy is associated with a much higher incidence of injuries to the bile duct than is traditional open cholecystectomy, its promise of decreasing pain, disability, and costs to patients undergoing cholecystectomy will

A new method for the surgical correction of biliary tract disease has been developed using a laparoscopic technique rather than a standard open cholecystectomy. Twelve consecutive laparoscopic cholecystectomies and pertaining clinical data are presented. In comparison with other treatment forms of gallbladder disease, including lithotripsy, stone dissolution, and mini-lap technique, it appears that laparoscopic cholecystectomy may safely reduce morbidity.

This article describes the first 50 laparoscopic cholecystectomies emphasizing the two-handed technique that is currently used at Marietta Surgical Center, Marietta, Georgia. This procedure has become primarily an outpatient procedure with resumption of normal activities in 7 days (including athletics). Two patients in this series had complications. One patients was converted to an open procedure because of a common duct injury. The other patient developed a biloma 2 weeks following surgery and recovered uneventfully following paracentesis on an outpatient basis.

Injury to the gallbladder following blunt abdominal trauma is an unusual finding at laparotomy, with a reported incidence of less than 2%. Avulsion from the liver and detachment from both the cystic duct and artery is an extremely rare finding. The condition of the patient and the extent of injuries will dictate the procedure performed. In the case presented, total avulsion of the gallbladder was successfully treated with identification and ligation of the cystic duct, and coagulation of small bleeding points, followed by placement of a Penrose drain.

The crux of the matter is to avoid being forced into cholecystectomy when the patient is acutely ill and has an increased risk of operative mortality. The rationale for performing surgery on an elective basis in patients with chronic gallbladder disease and for early medical management in most patiens with acute disease is discussed, along with the specifics of diagnosis and special considerations in the handling of problem cases.

PURPOSE: Laparoscopic subtotal cholecystectomy (LSC) can be an alternative surgical technique for difficult cholecystectomies. Surgeons performing LSC sometimes leave the posterior wall of the gallbladder (GB) to shorten the operation time and avoid liver injury. However, leaving the inflamed posterior GB wall is a major concern. In this study, we evaluated the clinical outcomes of standard laparoscopic cholecystectomy (SLC), LSC, and LSC removing only anterior wall of the GB (LSCA).
METHODS: We retrospectively reviewed the medical records of laparoscopic cholecystectomies performed between J

Laparoscopic cholecystectomy has traditionally been performed using multiple small sites. Single-incision laparoscopic surgery has emerged as an alternative technique to improve cosmesis and minimize complications associated with multiple incisions. A retrospective study was performed of all patients who underwent laparoscopic cholecystectomy by a single surgeon (DTA) from April 2008 to August 2011. Charts were reviewed for surgical indication, operative technique (multiple vs. single transumbilical incision), operative time, length of stay, and surgical complications. Sixty-three patients und

INTRODUCTION: Duplication of the gallbladder is a rare congenital anomaly. An incidence of 2.5:10,000 has been published in autoptic studies.
METHODS: We carried out an analysis of the published case reports of the last 30 years to examine those cases in which an accessory gallbladder was pre- or intraoperatively detected or missed and to evaluate the implications of the time of diagnosis of the duplication on the surgical therapy.
RESULTS: 28 case reports were analysed. If the duplication of the gallbladder was recognised pre-operatively or during operation, both gallbladders could be remov

BACKGROUND: Conversion to open cholecystectomy is still required in some patients. The aim of this study was to evaluate preoperative factors associated with conversion to open cholecystectomy in elective cholecystectomy and acute cholecystitis.
METHODS: The records of 1,804 patients who underwent cholecystectomy from May 1992 to January 2004 were reviewed retrospectively. The demographics and preoperative data of patients who required conversion to laparotomy were compared to those with successful laparoscopic cholecystectomy.
RESULTS: Conversion to open cholecystectomy was needed in 94 pat

In this retrospective study, we compared the results of 1,283 open cholecystectomies (OCs) performed at our medical center during the pre-laparoscopic era with 1,107 laparoscopic cholecystectomies (LCs) performed from 1990 to 1992. There was no difference in the percentage of cases of acute and chronic cholecystitis in each time period (16.8% in each), nor were there differences in the patient characteristics for each group. The percentage of patients undergoing intraoperative cholangiography was similar for patients with chronic cholecystitis for each period, although the incidence of abnorma

Laparoscopic cholecystectomy is rapidly becoming accepted as the best method for the treatment of symptomatic cholelithiasis. Randomized clinical trials comparing laparoscopic cholecystectomy with open cholecystectomy are unlikely to be performed. In order to compare these two operations, surgeons need an historical control group of patients who have undergone a conventional open cholecystectomy. The aim of this study was to document a control group of patients having an open cholecystectomy and compare them with patients having a laparoscopic cholecystectomy. This was achieved by a retrospect

The explosion in laparoscopic cholecystectomy has posed many questions about its safety compared with the "gold standard" of open cholecystectomy. A statewide database was established in Connecticut to study these issues. Thirty-three (97%) of 34 hospitals in Connecticut participated in the study, which began at the inception of the laparoscopic procedure. Four thousand six hundred forty laparoscopic cholecystectomies were performed between May 1, 1990, and September 30, 1991. The overall conversion rate to open cholecystectomy was 6.9%. Conversions were more frequent with acute cholecystitis,

INTRODUCTION: Patients undergoing elective laparoscopic cholecystectomy (ELLC) represent a heterogeneous group making it challenging to stratify risk. The aim of this paper is to identify pre-operative factors associated with adverse peri- and post-operative outcomes in patients undergoing ELLC. This knowledge will help stratify risk, guide surgical decision making and better inform the consent process.
METHODS: All patients who underwent ELLC between January 2015 and December 2019 were included in the study. Pre-operative data and both peri- and post-operative outcomes were collected retrosp

BACKGROUND: Laparoscopic cholecystectomy becomes the gold standard surgical procedure for treating gallstones. Standard laparoscopic cholecystectomy (SLC) requires proper dissection of Calot's triangle to achieve the critical view of safety. This may be difficult in certain conditions, resulting in higher incidence of bile duct injury and conversion to open. We aimed to compare the outcomes of laparoscopic cholecystectomy by retroinfundibular (RI) approach to that of SLC, in difficult cases.
PATIENTS AND METHODS: This study is prospective cohort study, in which 60 patients were operated by SL

BACKGROUND AND OBJECTIVES: Evidence is increasing that single-port or single-incision laparoscopic cholecystectomy is a safe and feasible alternative for cholecystectomy in children. In this study, we sought to compare the single-port hybrid technique, which we originally reported in 2012, with the conventional 4-port approach, in regards of complications, outcome, operative time and cost.
METHODS: A retrospective, single-center comparison of hybrid single-port versus conventional 4-port laparoscopic cholecystectomy was performed in 98 consecutive pediatric patients between January 2010 and O
