Nutritional management in chyle leaks and chylous effusions.

Chyle leaks occur when there is interruption to the lymphatic ducts that transport chyle around the body. The loss of this protein-rich, calorie-rich fluid can cause serious complications including dehydration, malnutrition and immunosuppression. Treatment of chyle leaks depends on the underlying cause, which may be surgical, secondary to malignant invasion or the result of a medical condition. Nutritional support is vital and leads to spontaneous leak closure in many cases. Nutritional management options include total bowel rest with parenteral nutrition, enteral feeding with specialized form

Incidence and management of chyle leakage after esophagectomy.

BACKGROUND: Postoperative chyle leakage is a rare but well-recognized complication after esophageal surgery. The aim of this study was to identify its incidence and potentially predisposing factors and to assess the consequences and management. METHODS: A consecutive series of 536 patients who underwent esophagectomy for malignant disease of the esophagus or gastroesophageal junction was reviewed. RESULTS: There were 20 patients (3.7%) with chyle leakage. After transthoracic esophagectomy the risk for the development of chyle leakage was higher than after transhiatal resection (p = 0.006). C

Postoperative chyle leak after pancreatic surgery - Is treatment effective?

PURPOSE: With increasing soft tissue clearance in pancreatic cancer surgery, postoperative chyle leak (CL) has become a more commonly observed complication. Recently, a new consensus definition was established by the International study group of pancreatic surgery (ISGPS). The aim of the present analysis was to evaluate risk factors and treatment options of patients with CL after pancreatic surgery. METHODS: Two hundred and twenty-eight patients with serous or chylous drainage after pancreatic surgery were included in this analysis of a prospectively collected database between 01/2014 and 12/

Chyle and gout causing acute monoarthritis: some unusual findings.

A 67-year-old man presented with acute monoarthritis of the right knee. Aspiration was carried out under aseptic precautions. This yielded a milky appearing fluid. Polarized microscopic examination confirmed uric acid crystals. The patient had a history of trauma to the joint. Acute monoarthritis of the knee can occur due to gout. Chylous effusions in the knee are rare. This combination of findings in the same joint has not been described in the literature. We emphasise the importance of history of trauma in acute monoarthritis. We discuss the unusual features of this case.

Chyle Leak after Oral Cancer Surgery - A Case Report.

RATIONALE: Surgery for oral cancer necessitates neck dissection, which is a common and safe procedure. However, it can be associated with few complications. Chyle leak (CL) is one such rare complication of neck dissection surgery, which is a potentially life-threatening complication. PATIENT CONCERNS: We report a case of left-sided CL following neck dissection surgery in a case of cancer of the tongue. DIAGNOSIS: Postoperatively, excessive drain output collection and biochemical analysis of fluid confirmed CL. TREATMENT: Immediate conservative measures were taken. Ligation of the thoracic d

Dual Chyle Leak: A Case of Chylothorax With Chylous Ascites.

Chylothorax is a rare and often insidious disorder, characterised by the accumulation of chyle in the pleural cavity. It results in substantial diagnostic and therapeutic challenges. We report the case of a 56-year-old male with a two-year history of chronic liver disease on treatment, who presented with complaints of breathlessness and abdominal distension for 10 days. Evaluation demonstrated chylothorax, chylous ascites, and a multi-thrombotic state. The underlying etiology was multifactorial, including a thoracic duct leak at the T4-T5 vertebral level. Thoracic duct embolisation was perform

Chyle fistula as complication of thyroid surgery in malignant pathology.

INTRODUCTION: Cervical chyle fistula (CF) is exceptional, and there is no agreement on its treatment. The study objective was to assess the efficacy of conservative and surgical treatment in this condition. METHOD: Study population: Patients undergoing thyroid surgery who experience CF after the procedure. Study groups: 1) Group with conservative treatment: CF with flow rates less than 300cm3/day, late or collected in the neck; and 2) Group with surgical treatment: early CF with flow rates greater than 300 cm3/day. STUDY VARIABLES: Cure rates and rate of conversion to surgery were analyzed.

Managing Chyle Leakage Following Right Retroperitoneoscopic Adrenalectomy: A Case Study.

BACKGROUND Surgery involving the right retroperitoneum can result in lymphatic (chylous) leakage from the cisterna chyli located anterior to the L1 and L2 vertebra or from lymph node dissection. This report describes a 46-year-old woman with retroperitoneal lymphatic (chylous) leak following right adrenalectomy for a nonfunctional adrenal adenoma. CASE REPORT A 46-year-old woman presented with a medical history of hypertension. An adrenal tumor (3.2×2.0 cm) was identified by computed tomography (CT). She was admitted for right retroperitoneoscopic adrenalectomy. The drainage volume of the drai

Radiological Management of Chyle Leaks Following Oesophagectomy: A Systematic Review.

A chyle leak is a rare but potentially life-threatening complication following oesophagectomy. If untreated, it can result in hypovolaemia, malnutrition, electrolyte imbalance, and immunosuppression. Traditionally managed with surgical ligation, radiological interventions have emerged as less-invasive alternatives, though their comparative efficacy remains uncertain. This paper aims to systematically review the literature on the efficacy, safety, and clinical outcomes of radiological interventions for managing chyle leaks following oesophagectomy. An electronic search of PubMed (Medline) was p

Management of chyle leaks following esophageal resection: a systematic review.

BACKGROUND: Chyle leakage is an uncommon but potentially life-threatening complication following esophageal resections. The optimal management strategy is not clear, with a limited evidence base. METHODS: Searches were conducted up to 31 December 2020 on MEDLINE, Embase, and Web of Science for randomized trials or retrospective studies that evaluated the management of chyle leakage following esophageal resection. Two authors independently screened studies, extracted data, and assessed for bias. The protocol was prospectively registered on PROSPERO (CRD: 42021224895) and reported in accordance

[Postoperative chyle leakage after abdominal surgery-Diagnostic and therapeutic strategies].

A postoperative chyle leakage (CL) is caused by intraoperative damage to the main lymphatic vessels or their tributaries. It is characterized by the secretion of a triglyceride-rich fluid, which classically has a characteristic milky appearance in a percutaneous drain. In visceral surgery CLs mostly occur after pancreatic and esophageal surgery but rarely occur after colorectal, liver or gastric surgery. Treatment often consists of a diet rich in medium chain triglycerides (MCT) to reduce the lymphatic flow, while ensuring a sufficient nutrient supply. If the CL does not cease, total parentera

Chyle and Bile Leak Following Open Cholecystectomy: A Rare Complication.

Gallstone disease is extremely common and frequently and safely treated by cholecystectomy. Chyle leak is a rare but significant side effect of many abdominal surgeries with rarely reported post-cholecystectomy. In this case, we report a 78-year-old lady with multiple comorbidities and symptomatic gallstones who underwent open cholecystectomy complicated by bile and chyle leak, which was successfully managed with endoscopic retrograde cholangiopancreatography (ERCP) and stenting for bile leak and conservative management for the chyle leak, which included drainage, low-fat diet, and octreotide.

Intranodal lymphangiography for chyle leakage after esophagectomy: A case report.

Postoperative chylothorax after esophagectomy is a relatively rare complication, but treatment can sometimes be complicated. We report 3 cases of Lipiodol lymphangiography via inguinal lymph node puncture that was effective for chyle leakage occurring after esophagectomy. Case 1: A 67-year-old man with stage IIIA esophageal squamous cell carcinoma underwent radical esophagectomy by video-assisted thoracic surgery (VATS) following neoadjuvant chemotherapy (NAC). After enteral feeding, right pleural effusion drainage increased sharply and changed to white color that was diagnosed as chylothorax.

Incidence and consequences of chyle leak after pancreatectomy for pancreatic cancer.

BACKGROUND: Chyle leak after pancreatic surgery is linked to malnutrition, but its impact on oncological outcomes is unclear. This study assessed if postoperative clinically relevant chyle leak (CR-chyle leak) affects adjuvant chemotherapy receipt rates and overall survival (OS) in pancreatic cancer patients. METHODS: Patients who underwent resection for localized pancreatic ductal adenocarcinoma at the University of Colorado Hospital (2015-2023) were included. Year of surgery was divided in two time periods based on changes in dissection techniques, increased neoadjuvant therapies, and more

Combined antegrade and retrograde approach for embolization of abdominal chyle leak.

Chyle leaks are an uncommon but often morbid complication of surgical resections in the retroperitoneal space. They can lead to multiple/prolonged hospitalizations and significantly diminish patient quality of life. Here we present a case of recalcitrant chylous ascites following resection of a renal mass. Lymphatic embolization was attempted but unsuccessful on two occasions (antegrade followed by retrograde approach) at outside hospitals. We used a combined antegrade and retrograde approach to overcome his challenging anatomy and treat the chyle leak definitively. This case highlights the us

Management of Chyle Leak in Pediatric Surgical Oncology: A Systematic Review.

INTRODUCTION: Chyle leak, a rare complication, arises from damage to primary lymphatic vessels due to congenital factors or medical interventions, leading to conditions such as chylothorax and chylous ascites. Managing chyle leaks is challenging, especially in pediatric surgical oncology, often arising as postoperative complications. Treatment options range from conservative dietary adjustments to surgical interventions, depending on leak severity and patient condition. This systematic review examines the management of chyle leaks in pediatric surgical oncology, emphasizing both conservative a

A protocolized approach to chyle leaks to mitigate prosthetic graft infection.

A 63-year-old man presented to the vascular surgery clinic with an enlarging left subclavian artery aneurysm owing to proximal compression by the anterior scalene muscle. He underwent open repair with a rifampin-soaked Dacron graft and developed a postoperative chyle leak. We describe our management and propose a protocolized deployment of surgical, dietary, pharmacological, and interventional techniques to resolve chyle leaks rapidly and prevent infections in the presence of a prosthetic vascular graft.
