Right putamen and claustrum infarction mimicking normal pressure hydrocephalus.

A woman in her 70s presented with approximately 2 years of sudden-onset gait and cognitive problems. She had been diagnosed with normal pressure hydrocephalus (NPH) and underwent ventriculoperitoneal shunt (VPS) placement 1 year prior. Before VPS placement, brain imaging showed ventriculomegaly and chronic infarction of the right putamen and claustrum. A lumbar drain trial resulted in modest improvement of gait dysfunction. She underwent VPS placement for suspected NPH, but her symptoms remained unchanged. Examination revealed mild cognitive impairment, left-sided and lower body predominant pa

Multiscale characterization of the human claustrum from histology to MRI.

The claustrum is a thin, bilateral structure embedded deep within the human brain. Its widespread cortical connectivity has motivated perhaps the broadest range of functional hypotheses of any subcortical structure. Yet its complex, sheet-like morphology has hindered investigation in living humans, leaving a small in vivo MRI literature marked by large and often implausible discrepancies. Here, we construct a three-dimensional histological "gold standard" model of the human claustrum and systematically evaluate three ultra-high field 7-Tesla MRI datasets against this reference and its downsamp

Postnatal development of NOS-ir neurons in the rat claustrum.

The morphological features of nitric oxide synthase (NOS)-containing neurons in the rat claustrum (Cl) were studied during the period of four months after birth. Forty-five animals divided into nine groups, according to survival period (P0, P4, P7, P10, P14, P21, P28, P60, P120) were used in the study. The immunocytochemical staining to neuronal NOS was performed and the material was studied both qualitatively and quantitatively using unbiased stereological methods. Our observations indicate that the process of maturation of NOS-immunoreactive (ir) neurons in Cl takes place during the early po

Comparative anatomy of the claustrum in selected species: A morphometric analysis.

The morphology of the claustrum was studied by stereological methods in representatives of five mammalian orders (Insectivora, Rodentia, Lagomorpha, Carnivora and Primates). In each species under study, a dorsal and a ventral part of the nucleus can be distinguished. Based on differences in shape and separation from surrounding structures, five morphological types of the claustrum occur. The claustrum of Insectivora and some rodents represents the least complicated morphological type. The nucleus is very poorly separated from the surrounding structures. The human claustrum is morphologically t

Relationship of corticoclaustral axon terminals to neurons of the claustrum in the cat.

A dark type degeneration of axon terminals was observed in the dorsocaudal part of the claustrum following lesions of the visual cortex. Most of the degenerating axon terminals had synapses mainly with dendritic spines (64.1%) and small dendrites (25.1%). Only 5.8% of degenerating axon terminals formed synaptic contacts with medium-sized dendrites, 1.0% with large dendrites and 0.5%--with other parts of the claustral neurons. 3.5% of degenerating axon terminals were observed to form a contact with more than one postsynaptic structure. Degenerating axon terminals displayed synaptic contacts exc

Parvalbumin in the cat claustrum: ultrastructure, distribution and functional implications.

The presence of the calcium-binding protein (CaBP) parvalbumin (PV) in the neuronal elements of the cat's dorsal claustrum was studied by immunohistochemistry at the light- and electron-microscopic level. PV-immunoreactive neurons and fibers were detected in all parts of the claustrum. The PV-immunoreactive neurons were divided into several subtypes according to their size and shape. Approximately 7% of all PV-immunoreactive neurons were classified as large, while approximately half of the labeled neurons were medium-sized. The small PV-immunoreactive neurons were 45% of the total PV-immunorea

Claustrum hyperintensity: a rare radiological correlate in Niemann-Pick disease.

A 5-year-old male child of consanguineous parentage, without any adverse perinatal history, presented with progressive cognitive regression predominantly in the language and attention domains, for 2 years. He had simultaneous pyramidal and extrapyramidal involvement, frequent generalised tonic-clonic seizures and recurrent respiratory tract infections. Examination was significant for vertical supranuclear gaze palsy, coarse facial features and splenomegaly. Given the clinical features, in the background of consanguinity and mother's history of spontaneous pregnancy losses, inborn errors of met

Aberrant claustrum structure in preterm-born neonates: an MRI study.

The human claustrum is a gray matter structure in the white matter between insula and striatum. Previous analysis found altered claustrum microstructure in very preterm-born adults associated with lower cognitive performance. As the claustrum development is related to hypoxia-ischemia sensitive transient cell populations being at-risk in premature birth, we hypothesized that claustrum structure is already altered in preterm-born neonates. We studied anatomical and diffusion-weighted MRIs of 83 preterm- and 83 term-born neonates at term-equivalent age. Additionally, claustrum development was an

Disruption of the claustrum-ACC pathway contributes to human mind blanking.

The claustrum is a highly connected structure hypothesized to orchestrate conscious experience, yet its role in humans remains enigmatic. To address this question, we prospectively investigated patients with drug-resistant epilepsy who underwent stereoelectroencephalography (SEEG) implantation driven by clinical indications, with electrode trajectories optimized to target the claustrum. Across the eight participants, the stimulated claustrum was left-sided in six and right-sided in two. Focal stimulation of the left claustrum reproducibly induced a transient arrest of ongoing thought and a red

The Role of the Claustrum in Parkinson's Disease and Vascular Parkinsonism: A Matter of Network?

BACKGROUND: The mechanisms underlying extrapyramidal disorders and their anatomical substrate have been extensively investigated. Recently, the role of the claustrum in Parkinson's disease and other neurodegenerative conditions has been better detailed. The main aim of this review was to summarize the supporting evidence for the role of the claustrum in degenerative and vascular parkinsonism. METHODS: The anatomy, biology, vascular supply, and connections of the claustrum in humans were identified and described, providing the substrate for the vascular involvement of the claustrum in large- a

Division of the human claustrum according to its architectonics and morphometric parameters.

The topography and cytoarchitectonics of the claustrum as well as morphometric parameters of its neurons were studied in 10 human brains obtained from patients without any detectable neuropathological changes. We distinguished four parts of the claustrum: dorsal, orbital, temporal and paraamygdalar. The dorsal and orbital parts contain larger cells, than those of the temporal and paraamygdalar parts, although these differences were statistically non significant. The highest neuronal density was observed in the paraamygdalar part. The nucleus and nucleus@cell body area ratio was significantly s

Influence of claustrum on cortex varies by area, layer, and cell type.

The claustrum is a small subcortical structure with widespread connections to disparate regions of the cortex. However, the impact of the claustrum on cortical activity is not fully understood, particularly beyond frontal areas. Here, using optogenetics and multi-regional Neuropixels recordings from over 15,000 cortical neurons in awake mice, we demonstrate that the effect of claustrum input to the cortex differs depending on brain area, layer, and cell type. Brief claustrum stimulation, producing approximately 1 spike per claustrum neuron, affects many fast spiking (FS; putative inhibitory) b

Topographical distribution of NADPH-diaphorase-positive neurons in the cat's claustrum.

The NADPH-diaphorase histochemical technique was used to visualize the morphological features of the NADPH-diaphorase positive cells and fibres, and their distribution in both parts of the cat's claustrum. Taking into account the size and form of the perikaryon and the dendritic and axonal characteristics, the neurons are grouped in different subclasses: large, medium-sized and small. The present data suggest the occurrence of two populations of NADPH-diaphorase neurons in the claustrum. One population consisting of large and medium-sized positive neurons represents the projection neurons whil

Human claustrum neurons encode uncertainty and prediction errors during aversive learning.

Flexible behavior depends on continuous updating of internal models, yet the neural circuits coordinating this process remain poorly understood. The claustrum, which is reciprocally connected to nearly the entire neocortex, is uniquely positioned to influence cortical processing. Here we report single-neuron recordings from the human claustrum during aversive learning, with anterior cingulate cortex and amygdala recordings for comparison. Claustrum and anterior cingulate neurons displayed structured, task-related responses. Distinct subpopulations encoded stimulus onset and action-contingent o

Claustrum Involvement in New Onset Refractory Status Epilepticus: A Systematic Review.

The claustrum sign is a distinctive neuroimaging finding characterized by bilateral T2/FLAIR hyperintensity of the claustrum, one of the most interconnected regions of the human brain. It was first described in new-onset refractory status epilepticus (NORSE) and febrile infection-related epilepsy syndrome (FIRES). A systematic search was conducted according to PRISMA guidelines from 2018 (formal definition of NORSE) to February 2026 (PROSPERO 2026 CRD420261307306). We included peer-reviewed original studies reporting MRI findings and patient-level data, and compared clinical characteristics, t

Regional and cell-type-specific afferent and efferent projections of the mouse claustrum.

The claustrum (CLA) is a conspicuous subcortical structure interconnected with cortical and subcortical regions. Its regional anatomy and cell-type-specific connections in the mouse remain not fully determined. Using multimodal reference datasets, we confirmed the delineation of the mouse CLA as a single group of neurons embedded in the agranular insular cortex. We quantitatively investigated brain-wide inputs and outputs of CLA using bulk anterograde and retrograde viral tracing data and single neuron tracing data. We found that the prefrontal module has more cell types projecting to the CLA
