User-Centered Redesign of Monitoring Alarms: A Pre-Post Study on Perception, Functionality, and Recognizability Following Real-Life Clinical Implementation.

Background: Auditory alarms in patient monitoring are vital for clinical safety, but their harsh acoustic properties and high frequency contribute to stress, alarm fatigue, and reduced acceptance among healthcare staff. In collaboration with Sen Sound, Philips redesigned its alarm sounds to reduce auditory harshness, particularly for low- and medium-priority alarms, while preserving the salience of high-priority alerts. This study evaluated the impact of these refined alarm sounds in a real-world clinical setting. Objective: The goal was to determine whether anesthesia professionals perceive t

Improving sepsis prediction in intensive care with SepsisAI: A clinical decision support system with a focus on minimizing false alarms.

Prediction of sepsis using machine-learning approaches has recently gained traction. However, the lack of translation of these algorithms into clinical routine remains a major issue. Existing early sepsis detection methods are either based on the older definition of sepsis or do not accurately detect sepsis leading to the high frequency of false-positive alarms. This results in a well-known issue of clinicians' "alarm fatigue", leading to decreased responsiveness and identification, ultimately resulting in delayed clinical intervention. Hence, there is a fundamental, unmet need for a clinical

Efficacy and Safety of High-Dose Vitamin D Supplementation Combined With an Enuresis Alarm vs Alarm Alone in Children With Primary Monosymptomatic Nocturnal Enuresis: A Randomized Clinical Trial.

PURPOSE: This study evaluated the efficacy and safety of high-dose vitamin D supplementation (VDS) combined with enuresis alarm (EA) therapy in children with primary monosymptomatic nocturnal enuresis (PMNE). MATERIALS AND METHODS: A 2-arm randomized clinical trial was conducted at a tertiary center in China between December 2024 and May 2025. Children with PMNE and serum 25-hydroxyvitamin D levels below 30 ng/mL were randomly assigned to receive either 8 weeks of VDS (vitamin D3 soft capsules, 2400 IU/d) plus EA (VDS + EA group) or EA (EA group). The primary outcome was treatment response. S

Using a clinical decision support system to reduce excess driving pressure: the ALARM trial.

BACKGROUND: Patients at need for ventilation often are at risk of acute respiratory distress syndrome (ARDS). Although lung-protective ventilation strategies, including low driving pressure settings, are well known to improve outcomes, clinical practice often diverges from these strategies. A clinical decision support (CDS) system can improve adherence to current guidelines; moreover, the potential of a CDS to enhance adherence can possibly be further increased by combination with a nudge type intervention. METHODS: A prospective cohort trial was conducted in patients at risk of ARDS admitted

Beyond alarm fatigue: nurses' experiences of behavioral nudge fatigue in digital clinical environments.

BACKGROUND: Digital health technologies such as electronic health records, clinical decision support systems, and reminder mechanisms are increasingly integrated into nursing practice to enhance patient safety and support clinical decision-making. While these systems function as behavioral nudges intended to guide clinical actions, sustained exposure to frequent alerts and reminders may generate unintended cognitive and emotional consequences for nurses. Although alarm fatigue has been widely studied, broader experiences related to continuous digital nudging in nursing practice remain underexp

An Overview of the Global Alarming Increase of Multiple Drug Resistant: A Major Challenge in Clinical Diagnosis.

The increased spreading of antibiotic resistance among different infectious agents has been a fast-growing public health challenge worldwide; this is because of the discovery of new resistance mechanisms and the reduction in quality and effective treatments of general pathogenic infections. This has caused unsuccessful microbial responses to standard therapy, which could lead to a higher risk of mortality, prolonged illness, and more expenditures for health care. Most parasites, bacteria, fungi, and viruses can produce a higher degree of multidrug resistance (MDR) with increased mortality and

Evidence of a suffocation alarm system sensitive to clinically-effective treatments with the panicolytics clonazepam and fluoxetine.

Dyspnea, 'hunger for air', and the urge to flee are the cardinal symptoms of respiratory-type panic attacks. Patients also show baseline respiratory abnormalities and a higher rate of comorbid and antecedent respiratory diseases. Panic attacks are also precipitated by both the infusion of 0.5 M sodium lactate and the inhalation of 5-7% carbon dioxide (CO2) in predisposed patients, but not in healthy volunteers nor patients without panic disorder. Further studies show that patients with panic are also hyper-responsive to hypoxia. These and other observations led Klein (1993) to suggest that cli

A knowledge-based alarm system for monitoring cardiac operated patients--assessment of clinical performance.

An intelligent alarm system for the postoperative monitoring of cardiac surgery patients, which did not require any manual data entries, was tested in two phases. A clinician monitored at bedside the patients' recovery and verified clinically abnormal physiological states. After the first test with ten patients, the system's rulebase was upgraded and then tested with an additional 15 patients. The alarm system employed two PC/ATs and was programmed to give notice of four pathological states (hyperdynamic state, hypovolemic state, hypoventilation and left ventricular failure) at two levels of u

Acute Cerebellitis Following COVID-19: Alarming Clinical Presentation Challenged by Normal Paraclinical Findings.

We report the case of an acute cerebellitis following COVID-19 in 32-year-old man who presented with a life-threatening critical cerebellar syndrome contrasting with normal paraclinical findings. Despite this fulminant critical presentation, the patient fully recovered in 37 days after early treatment with high-dose steroids and intravenous immunoglobulins. This case highlights the need for clinicians to be aware of acute cerebellitis following COVID-19, despite normal laboratory, imaging and electroencephalography findings and the importance to start appropriate treatment as soon as possible.

Traumatic ulcerative granuloma with stromal eosinophilia: a lesion with alarming histopathologic presentation and benign clinical course.

Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is a chronic, benign, self-limiting lesion of the oral mucosa. Clinically, the ulceration is characterized by the presence of indurated elevated borders and may resemble pyogenic granuloma or even squamous cell carcinoma of the mouth. Pathogenesis of the lesion is unclear. Although it had been suggested that TUGSE may represent a CD30+ lymphoproliferative disorder, this theory is currently not supported by evidence. We are presenting a classic example of TUGSE, its clinical course, differential diagnosis, and treatment.

Improved Anticoagulation Guideline Adherence and Clinical Outcome Through Hospital-Wide Atrial Fibrillation Alarming System.

BACKGROUND: Direct oral anticoagulation (DOAC) therapy has been approved for stroke prevention in atrial fibrillation (AF) patients. However, about 30% of patients with AF are not on oral anticoagulants (OACs). About 30% of patients on OACs are on off-label dosages. OBJECTIVE: We used a hospital-wide AF screening system to improve the OAC guideline adherence rate. We also assessed the impact on clinical outcomes. METHODS: All electrocardiograms (ECGs) of each hospitalized patient were screened using an AF screening system, regardless of whether the ECGs were obtained during or before hospita

Improving the Quality of Nursing Care Through Clinical Alarm Management in Intensive Care Units: An Integrative Review.

Background/Objectives: Clinical alarm management in continuous patient monitoring systems remains a major challenge in intensive care units (ICUs). The high frequency of nonactionable alarms contributes to alarm fatigue among healthcare professionals, potentially compromising patient safety, clinical surveillance, and the quality of nursing care. As nurses play a central role in monitoring critically ill patients, evidence-based alarm management strategies are essential to improve the quality of care and patient outcomes. This review aimed to synthesize the scientific evidence on nursing inter

Nurses' Clinical Alarm-Related Behaviors and Their Associated Factors in Malawian Critical Care Units: A Cross-Sectional Study.

BACKGROUND: Understanding nurses' clinical alarm-related behaviors is crucial for enhancing patient safety in critical care settings. These settings operate under significant constraints that profoundly influence how nurses prioritize and respond to alarms. While suboptimal alarm management has been extensively studied in well-resourced settings, research remains scarce in resource-constrained settings. This study aimed to evaluate nurses' clinical alarm-related behaviors and their associated factors in critical care units within resource-limited contexts. METHODS: An analytical cross-section

Male genital dermatophytosis - clinical features and the effects of the misuse of topical steroids and steroid combinations - an alarming problem in India.

Genital dermatophytosis has been considered rare by most Western authorities. However, to the contrary, Indian reports have shown a higher prevalence of genital dermatophytosis due to warm and humid climate, overcrowding and lack of hygiene. A review is presented for 24 cases of male genital dermatophytosis occurring in patients suffering from tinea cruris in India who have been randomly applying various broad-spectrum steroid antifungal and antibacterial creams containing one or more antifungal and antibiotic in addition to potent corticosteroids, mainly clobetasol propionate. This is such a

Alarming increase in HbA1c and near misdiagnosis of diabetes mellitus resulting from a clinical laboratory instrument upgrade and haemoglobin variant.

A 29-year-old woman was referred for new-onset diabetes mellitus after her glycosylated haemoglobin (HbA1c) was found to be 10.2%. Three years earlier, the patient's HbA1c-measured by the same clinical laboratory-had been 5.5%. The newer HbA1c was discordant with fasting glucose levels and a lack of diabetes-associated symptoms. The laboratory reported that their assay methodology remained unchanged and also that no haemoglobin variants were detected. Further investigation, however, revealed, first, that the patient carried a haemoglobin alpha chain mutation (Hb Wayne) that can sometimes cause

Alarming rates of virological failure and drug resistance in patients on long-term antiretroviral treatment in routine HIV clinics in Togo.

Information on efficacy of long-term antiretroviral treatment (ART) exposure in resource-limited countries is still scarce. In 767 patients attending routine HIV centers in Togo and receiving first-line ART for more than four years, 42% had viral load greater than 1000 copies/ml and either were on a completely ineffective ART regime or were with only a single drug active. The actual conditions to ensure lifelong ART in resource-limited countries can have dramatic long-term outcomes.
