Primary Aldosteronism: Reassessing Underdiagnosis, Overdiagnosis, and Clinical Decision-Making.

Primary Aldosteronism (PA) is postulated to represent one of the most prevalent and significant etiologies of secondary hypertension. A majority of investigations have indicated that the incidence of PA has been escalating, particularly within the cohort of normokalemic individuals. PA generally constitutes an adrenal pathology characterized by excessive production of aldosterone, which may occur bilaterally or unilaterally. The evaluation for Primary Aldosteronism (PA) is alarmingly inadequate, frequently postponed until a substantial duration after the identification of hypertension, general

Patient Preferences on Clinical Decision Making in Multiple Myeloma.

PURPOSE: To better understand the priorities that guide patients with multiple myeloma, we surveyed patients on four different treatment scenarios, each of treatment strategies shown to improve progression-free survival (PFS) but offering similar overall survival (OS) outcomes. METHODS: We conducted a survey using the HealthTree Cure Hub by the HealthTree Foundation, the largest online portal for people with plasma cell dyscrasias. RESULTS: The primary analysis cohort included 466 participants with myeloma, while an additional 297 responses from patients with smoldering myeloma or monoclonal

Restorative dentistry clinical decision-making for hypodontia: complex cases.

Hypodontia is a relatively common condition and patients will be seen by both general dental practitioners and specialist dental colleagues. Although hypodontia can be described as mild, moderate and severe, this does not directly correlate with the complexity of treatment required to provide an acceptable outcome. In addition, the complexity of treatment provided by one colleague in the multidisciplinary team may not be the same as for other colleagues. When treatment planning and delivering dental care for these patients, especially those with severe hypodontia, it is useful to recognise the

Knee adduction moment decomposition: Toward better clinical decision-making.

Knee adduction moment (KAM) is correlated with the progression of medial knee osteoarthritis (OA). Although a generic gait modification can reduce the KAM in some patients, it may have a reverse effect on other patients. We proposed the "decomposed ground reaction vector" (dGRV) model to 1) distinguish between the components of the KAM and their contribution to the first and second peaks and KAM impulse and 2) examine how medial knee OA, gait speed, and a brace influence these components. Using inverse dynamics as the reference, we calculated the KAM of 12 healthy participants and 12 patients

Clinical Decision Making in Patients with Stroke-Related Dysphagia.

This article aims to highlight stroke considerations in the evaluation and management of dysphagia. Although dysphagia was previously thought to occur only following brainstem or bilateral cortical strokes, the development of brain imaging and dynamic swallowing studies has revealed small, unilateral supratentorial strokes can produce dysphagia. In this article, screening, evaluation, and management of dysphagia are outlined, as well as the clinical decision making that occurs when taking into account cognitive and communication deficits that may be present. Narcolepsy: clinical decision making for the primary care physician.

Narcolepsy is a serious neurological condition in which patients are overcome by persistent, excessive feelings of fatigue and drowsiness. In addition to chronic fatigue, patients with narcolepsy often succumb to intermittent, uncontrollable periods where they abruptly fall asleep during waking hours. In addition to episodic bouts of daytime sleeping, narcoleptics also exhibit cataplexy, sleep paralysis, and hypnagogic and hypnopompic hallucinations. Unfortunately, many individuals with narcolepsy remain undiagnosed and therefore, untreated, posing a risk to themselves and those around them. T

Clinical decision making during public health emergencies: ethical considerations.

Recent public health emergencies involving anthrax, the severe acute respiratory syndrome (SARS), and shortages of influenza vaccine have dramatized the need for restrictive public health measures such as quarantine, isolation, and rationing. Front-line physicians will face ethical dilemmas during public health emergencies when patients disagree with these measures. Patients might request interventions that are not recommended or for which they are not eligible, or they might object to intrusive or restrictive measures. The physician's primary responsibility in such emergencies is to the publi

Clinical Immunology for Heart Transplant Nurses: Medication Decision-Making.

Heart transplantation offers life-saving therapy for patients with end-stage heart disease, but outcomes depend on precise management of immune risk and immunosuppression. The transplanted heart is a potent antigenic stimulus that can trigger immune pathways, leading to rejection and graft dysfunction if not adequately controlled. Intensive care unit nurses and advanced practice registered nurses need a working knowledge of transplant immunology because daily decisions about rejection surveillance, immunosuppressive medication management, early recognition of rejection risk, and patient educat

Sociodemographic bias in LLMs' clinical decision-making for dizziness.

ObjectiveAs large language models (LLMs) enter clinical decision support, concerns persist about sociodemographic bias. We assessed whether LLM recommendations for dizziness vary by patient descriptors and clinical detail.MethodsWe conducted a cross-randomized in-silico vignette study. One hundred synthetic emergency department dizziness cases were created using established diagnostic frameworks including the TiTrATE paradigm, SAEM GRACE-3 guidelines, and Bárány Society diagnostic criteria. Each vignette was tested in a neutral form and with 33 sociodemographic descriptor variants (34 total).

Clinical Decision Making in Pediatric Physiotherapy: A Scoping Review.

AIMS: Clinical decision making (CDM) in pediatric physiotherapy is complicated by developmental variability, family-centered care, and age-appropriate child participation. This scoping review mapped the literature to identify frameworks used to study CDM and factors influencing therapists' decisions. METHODS: Following the Arksey and O'Malley framework and PRISMA-ScR reporting checklist, PubMed, Scopus, Web of Science, and the Cochrane Library were searched without date restriction. Empirical qualitative, quantitative, or mixed-methods studies examining CDM in physiotherapy for children and a

Bias in medical AI: Implications for clinical decision-making.

Biases in medical artificial intelligence (AI) arise and compound throughout the AI lifecycle. These biases can have significant clinical consequences, especially in applications that involve clinical decision-making. Left unaddressed, biased medical AI can lead to substandard clinical decisions and the perpetuation and exacerbation of longstanding healthcare disparities. We discuss potential biases that can arise at different stages in the AI development pipeline and how they can affect AI algorithms and clinical decision-making. Bias can occur in data features and labels, model development a

Clinical decision making when cytology indicates a Warthin tumor.

Warthin tumor (WT) is a benign tumor usually affecting the parotid gland. The main diagnostic tool remains ultrasound combined with fine-needle aspiration cytology (FNAC). This study aims to examine how reliably FNAC indicates WT for clinical decision making regarding surgical versus conservative management. We included all patients who underwent FNAC from a parotid gland lesion between 2016 and 2018 at our institution, and whose FNAC revealed WT suspicion. The FNACs were divided into three groups based on the cytology report: certain, likely, and possible WT. The patients were divided into tw

Assessing Shared Decision-Making Clinical Behaviors Among Genetic Counsellors.

Shared decision-making (SDM) is a collaborative approach in which clinicians educate, support, and guide patients as they make informed, value-congruent decisions. SDM improves patients' health-related outcomes through increasing knowledge, reducing decisional conflict, and enhancing experience of care. We measured SDM in genetic counselling appointments with 27 pregnant women who were at increased risk to have a baby with a genetic abnormality. The eight experienced genetic counsellors who participated had no specific SDM training and were unaware that SDM was being assessed. Audio transcript

Unsatisfactory colposcopy: clinical decision-making in conditions of uncertainty.

BACKGROUND: Unsatisfactory colposcopy, where the cells of interest are not visible in women with a positive cervical screening test, is a common area of clinical uncertainty due to the lack of clear evidence and guidance. Colposcopists' opinions and experiences are likely to have a significant influence on service provision and the development of national policy. The aim of this study was to analyse decision-making when applied to women with unsatisfactory colposcopy. METHODS: A multi-centre qualitative study utilizing a series of focus groups in an English healthcare region. Sampling aimed t

Clinical decision-making in functional and hyperkinetic movement disorders.

OBJECTIVE: Functional or psychogenic movement disorders (FMD) present a diagnostic challenge. To diagnose FMD, clinicians must have experience with signs typical of FMD and distinguishing features from other hyperkinetic disorders. The aim of this study was to clarify the decision-making process of expert clinicians while diagnosing FMD, myoclonus, and tics. METHODS: Thirty-nine movement disorders experts rated 60 patients using a standardized web-based survey resembling clinical practice. It provided 5 steps of incremental information: (1) visual first impression of the patient, (2) medical
