An Assessment of the Presence of Clostridium tetani in the Soil and on Other Surfaces.

INTRODUCTION: Standard emergency medicine practice includes tetanus vaccine administration as part of wound care management for patients who are not fully immunized. Since there have been no available studies in the United States reaffirming the prevalence of Clostridium tetani (C tetani) since 1926, we sought to identify its prevalence in a major urban county in the US. METHODS: We sampled soil, rusted metal, concrete, and dog feces to determine the prevalence of C tetani in a single metropolitan county in the United States. Soil samples and swabs were collected from four locations: the soil

A next-generation sequencing based method for determining genetic stability in Clostridium tetani vaccine strains.

Production of tetanus and other clostridial vaccines highly depends on the stable and reproducible production of high toxin levels. This creates a need to ensure the genetic stability of seed strains. We developed a two-stage method for improved assessment of the genetic stability of Clostridium seed strains. This method is based on next-generation sequencing (NGS) of strain DNA and mapping the sequence reads to a reference sequence. The output allows analysis of global genome consistency followed, if necessary, by detailed expert judgement of potential deviations at the gene level. The limit

An unusual case of Clostridium tetani bacteraemia likely arising from a skin and soft tissue infection.

Clostridium tetani, the causative organism of tetanus, is unusual in developed countries, including Singapore, and to our knowledge, only three known cases of C. tetani bacteraemia have been reported worldwide. This case report describes a case of C. tetani bacteraemia in a Singaporean patient without clinical manifestations of tetanus, likely arising from a sacral sore, highlighting the need for early recognition and targeted antimicrobial treatment for the condition and its complications.

Multiple Independent Origins of Tn916-Mediated Tetracycline Resistance in Clostridium tetani from a Confined Geographic Area.

Background/Objectives: Tetracycline resistance in Gram-positive bacteria has increased globally through horizontal gene transfer. Antimicrobial resistance genes in Clostridium tetani, the causative agent of tetanus, are rarely reported. Previously, we identified tetracycline resistance gene tet(M)-positive C. tetani strains in Japan, but their evolutionary origin and acquisition mechanism remain unclear. This study aimed to elucidate the evolutionary origin of tet(M)-positive C. tetani and clarify the mechanism of horizontal resistance gene acquisition. Methods: Complete genome sequences of si

Comparative Analysis of Toxigenic and Megaplasmid-Free Clostridium tetani Strains Reveals Physiological Transitions Linked to Tetanus Toxin Production.

Understanding Clostridium tetani metabolism and physiology is essential for identifying factors controlling toxin synthesis. We compared a toxigenic C. tetani strain with a derivative strain lacking the toxin-encoding megaplasmid carrying tetR and tetX genes involved in toxin regulation and production. Both strains were cultured under identical strict anaerobic conditions in complex media. The toxigenic strain exhibited four successive fermentation phases, including rapid growth, toxin accumulation-associated slower growth, cell lysis, and toxin maturation. Cultivation yielded 0.45 g/L of biom

[Production of recombinant fragments of the Clostridium tetani neurotoxin for the development of new immune-prophylaxis preparations against tetanus].

Tetanus belongs to dangerous infection diseases, whose effective prevention can be ensured by vaccines. The acting substance of tetanus vaccines, presently in use, is a partially purified and deprived-of-lethal-action Clostridium tetani neurotoxin. The construction of a subunit preparation on the basis of toxin fragments obtained through gene engineering could be a method aimed at promoting the quality of the used tetanus vaccines. With this goal in mind, we built, within the present case study, the expressing genetic constructions and obtained, in the pure form, an extensive tetanus-vaccine c

BotR/A and TetR are alternative RNA polymerase sigma factors controlling the expression of the neurotoxin and associated protein genes in Clostridium botulinum type A and Clostridium tetani.

Clostridium botulinum and Clostridium tetani, respectively, produce potent toxins, botulinum neurotoxin (BoNT) and tetanus neurotoxin (TeTx), which are responsible for severe diseases, botulism and tetanus. Neurotoxin synthesis is a regulated process in Clostridium. The genes botR/A in C. botulinum A and tetR in C. tetani positively regulate expression of BoNT/A and associated non-toxic proteins (ANTPs), as well as TeTx respectively. The botR/A gene lies in close vicinity of the two operons which contain bont/A and antps genes in C. botulinum A, and tetR immediately precedes the tetX gene in C

Detection of Clostridium tetani Neurotoxins Inhibited In Vivo by Botulinum Antitoxin B: Potential for Misleading Mouse Test Results in Food Controls.

The presence of botulinum neurotoxin-producing Clostridia (BPC) in food sources is a public health concern. In favorable environmental conditions, BPC can produce botulinum neurotoxins (BoNTs) outside or inside the vertebrate host, leading to intoxications or toxico-infectious forms of botulism, respectively. BPC in food are almost invariably detected either by PCR protocols targeted at the known neurotoxin-encoding genes, or by the mouse test to assay for the presence of BoNTs in the supernatants of enrichment broths inoculated with the tested food sample. The sample is considered positive fo

Validation of monoplex assays detecting antibodies against Corynebacterium diphtheriae and Clostridium tetani toxins, rubella virus and parvovirus B19 for incorporation into Multiplex Serology.

Serological assays detecting antibodies in serum or plasma samples are useful and versatile instruments to investigate an individual's infection and vaccination history, e.g. for clinical diagnosis, personal risk evaluation, and seroepidemiological studies. Multiplex Serology is a suspension bead array-based high-throughput methodology for simultaneous measurement of antibodies against multiple pathogens in a single reaction vessel, thus economizing sample volume, measurement time, and costs. We developed and validated bead-based pathogen-specific Monoplex Serology assays, i.e. assays includin

Ileal infarction: a rare cause of tetanus in the elderly.

We report the case of a 70 year-old woman who presented with clinical tetanus following a four day history of abdominal pain and vomiting. The source of Clostridium tetani was an infarcted loop of ileum resulting from pelvic adhesions. The treatment of tetanus and the incidence of endogenous sources of Clostridium tetani are reviewed. Health Department figures for the decade 1973-1982 show that 70% of reported cases of clinical tetanus occur in those aged 45 years or older.

Botulinum and tetanus neurotoxins: structure, function and therapeutic utility.

The toxic products of the anaerobic bacteria Clostridium botulinum, Clostridium butyricum, Clostridium barati and Clostridium tetani are the causative agents of botulism and tetanus. The ability of botulinum neurotoxins to disrupt neurotransmission, often for prolonged periods, has been exploited for use in several medical applications and the toxins, as licensed pharmaceutical products, now represent the therapeutics of choice for the treatment for several neuromuscular conditions. Research into the structures and activities of botulinum and tetanus toxins has revealed features of these prote

Improvement in laboratory diagnosis of wound botulism and tetanus among injecting illicit-drug users by use of real-time PCR assays for neurotoxin gene fragments.

An upsurge in wound infections due to Clostridium botulinum and Clostridium tetani among users of illegal injected drugs (IDUs) occurred in the United Kingdom during 2003 and 2004. A real-time PCR assay was developed to detect a fragment of the neurotoxin gene of C. tetani (TeNT) and was used in conjunction with previously described assays for C. botulinum neurotoxin types A, B, and E (BoNTA, -B, and -E). The assays were sensitive, specific, rapid to perform, and applicable to investigating infections among IDUs using DNA extracted directly from wound tissue, as well as bacteria growing among

Postoperative tetanus after laparoscopic obturator hernia repair for strangulated ileus: report of a case.

This report presents the case of an 84-year-old woman who developed tetanus 3 days after the resection of a gangrenous small intestine caused by obturator hernia incarceration. The diagnosis of tetanus was clinically made after the appearance of generalized spastic contractions with opisthotonus. Clostridium tetani organisms residing in the gastrointestinal tract were presumed to have been endogenously inoculated into the strangulated intestine, where it produced tetanospasmin, causing tetanus. The patient successfully recovered after aggressive intensive care. There have been 16 case reports

Uncommon intraoral source of generalized tetanus: Case report and review of intensive care strategies.

INTRODUCTION: Tetanus is an acute disease caused by the tetanospasmin toxin produced by Clostridium tetani, with a high mortality particularly in developing countries. Although preventable through primary immunization and booster vaccination, cases in adults with uncertain immunization status remain common. Intraoral wounds are uncommonly reported as portals of entry. CASE PRESENTATION: A 54-year-old male developed generalized tetanus after manipulating a tooth with a toothpick, causing bleeding. Initial dental pain and oral numbness progressed to trismus, opisthotonus, and risus sardonicus.

Use of intrathecal and intravenous clonidine in a case of severe tetanus with acute renal failure.

Tetanus is an acute often fatal disease produced by the Gram-positive, obligate anaerobic bacterium Clostridium tetani. Prolonged intensive care is required in severe tetanus, with the associated complications including nosocomial sepsis. Autonomic dysfunction in severe tetanus is difficult to manage and is a significant cause of mortality. We present here, use of clonidine in a case of severe tetanus with acute renal failure who was successfully managed.

Clostridial disease associated with neurologic signs: tetanus, botulism, and enterotoxemia.

Clostridial infections are found worldwide in almost all species of animals and may involve a variety of body systems and present with a diversity of clinical signs. Most damage done through clostridial infections is due to the action of toxins released from the bacteria. Thus, disease caused by Clostridium spp should more correctly be called intoxication. Two prominent clostridial infections are associated with neurologic signs: Clostridium botulinum and C tetani. In both infections, the mechanism that is responsible for causing the problem is similar, despite the remarkable difference in clin
