Successful kidney transplantation after coccidioidal meningitis.

Coccidioidomycosis is a fungal infection primarily found in residents or visitors to geographic areas where Coccidioides species are endemic, including the southwestern United States, northwestern Mexico, and certain areas of Central and South America. The infection rarely disseminates, but certain populations are at higher risk of dissemination. One population at high risk of disseminated disease is solid organ transplant recipients. At our transplant center in Arizona, patients with proven coccidioidal infection before transplantation undergo thorough counseling about the risks of disseminat

A murine model of coccidioidal meningitis.

Coccidioidal meningitis is lethal in humans. A reproducible murine model was established by lumbar intrathecal injection of Coccidioides immitis arthroconidia. Cerebrospinal fluid (CSF) samples were obtained by cisternal puncture. Lethal infection developed in all mice given 10-60 colony-forming units (cfu). Lethargy, ataxia, or paralysis preceded death. Temporal studies after challenge with 27 cfu revealed positive brain (4/5 mice) and spinal cord (2/5 mice) cultures on day 3; CSF samples contained 688 leukocytes/mm(3) and 33 cfu/mL. The results of histopathologic analysis were unremarkable.

Treatment of coccidioidal meningitis with fluconazole.

Fluconazole was administered at doses of 50-400 mg/d to 18 patients (15 men, three women) with coccidioidal meningitis. After a mean duration of treatment of 9.8 months, 10 (67%) of 15 assessable patients had responded, one (7%) of 15 had partially responded, and four (27%) of 15 had not responded to therapy. Five (63%) of eight assessable patients receiving fluconazole as sole therapy responded or partially responded. Two patients discontinued fluconazole after initially responding to therapy, and both experienced relapse. The toxicity of fluconazole remains minimal at doses to 400 mg/d. The

Coccidioidal meningitis diagnosed by skin biopsy.

We present a patient with coccidioidal meningitis whose diagnosis was not confirmed until a skin biopsy was performed. Because he lived in an area where coccidioidomycosis is not endemic, his meningitis was at first attributed to tuberculosis or sarcoidosis. After a verrucous lesion from the face was biopsied and the diagnosis substantiated, the patient responded well to consolidation therapy consisting of intrathecal amphotericin B and oral ketoconazole.

Coccidioidal meningitis presenting as memory loss.

PURPOSE: This case study is designed to help the nurse practitioner recognize atypical symptoms of coccidioidal meningitis, particularly in high-risk groups that require further diagnostic treatment. DATA SOURCES: Selected research, clinical articles, and case studies. CONCLUSIONS: Coccidioidal meningitis is a potentially lethal infection unless recognized and treated. Unlike other infectious meningitides that present with more acute meningeal symptoms, disseminated coccidioidomycosis can present insidiously. When it presents as cognitive dysfunction, it may be mistaken for early dementia an

Vasculitic complications associated with coccidioidal meningitis.

Vasculitis complicating coccidioidal meningitis is becoming increasingly recognized. At this time, predisposing clinical features have not been elucidated. Histologically, 2 types of vascular inflammation have been described. The first is a transmural inflammatory process of the intracranial blood vessels that occurs early in the course of disease. Encroachment of the vessel lumen may result in thrombosis. The second process occurs with chronic disease and is associated with intimal thickening and luminal occlusion with little inflammation. Numerous substances, including metalloproteinases, cy

MR imaging of acute coccidioidal meningitis.

BACKGROUND AND PURPOSE: Our purpose was to describe the MR imaging findings in patients with acute coccidioidal meningitis. METHODS: Fourteen patients (11 men, three women; 22-78 years old; mean age, 47 years) with coccidioidal meningitis underwent neuroimaging within 2 months of diagnosis. Thirteen patients had MR imaging and one had an initial CT study with a follow-up MR examination 5 months later. Initial and follow-up MR images were evaluated for the presence of ventricular dilatation, signal abnormalities, enhancement characteristics, sites of involvement, and evidence of white matter o

Trapped fourth ventricle in coccidioidal meningitis.

A 4-year-old girl with known coccidioidal meningitis developed posterior fossa signs. CT scanning revealed a large fourth ventricle. Injection of contrast medium into the lateral ventricles revealed free flow into the fourth ventricle, and injection into the fourth ventricle revealed no flow into the aqueduct or third ventricle. The posterior fossa signs cleared after shunting of the fourth ventricle. A one-way aqueductal valve resulting from the ventricular inflammation is hypothesized.

Isavuconazole in the Treatment of Coccidioidal Meningitis.

Patients with coccidioidal meningitis require lifelong antifungal therapy. Cumulative toxicity and lack of antifungal efficacy require salvage therapy in the treatment of some patients. In a retrospective review of nine patients with coccidioidal meningitis treated with isavuconazole, successful therapy was seen in three patients and stable disease was confirmed in six patients. Isavuconazole may be a useful addition to the therapeutic choices currently available for coccidioidal meningitis.

Use of radiologic modalities in coccidioidal meningitis.

The diagnostic utility of pentetate indium trisodium in 111 CSF studies, technetium Tc 99m brain scans, and computerized tomographic (CT) scans was evaluated in eight patients in whom coccidioidal meningitis developed following a dust storm in the Central Valley of California. The 111In flow studies and the CT scans demonstrated hydrocephalus in five patients with clinical findings suggesting this complication. Ventriculitis has not previously been diagnosed before death in patients with coccidioidal meningitis; however, it was demonstrated in two patients by the technetium Tc 99m brain scan.

Use of Intrathecal Amphotericin for Coccidioidal Meningitis.

BACKGROUND: Cases of coccidioidomycosis are increasing dramatically in the United States. The utility of intrathecal (IT) amphotericin in treating coccidioidal meningitis (CM) in the era of azole therapy is unknown. We sought to understand how IT therapy is associated with mortality. METHODS: We conducted a retrospective chart review of adult patients with laboratory-proven CM seen at Stanford Healthcare by an infectious diseases physician from 2008 to 2023. RESULTS: Fifty-seven patients met inclusion criteria. All patients received azole therapy. Twenty-seven of those patients (47.4%) addit

Coccidioidal Meningitis in a Patient With HIV.

Coccidioidal meningitis is a rare yet life-threatening complication of disseminated Coccidioides infection, primarily affecting immunocompromised individuals. Coccidioides immitis and Coccidioides posadasii are dimorphic fungi endemic to the southwestern United States, where exposure to inhaled spores can lead to a range of clinical manifestations, including pulmonary and central nervous system (CNS) involvement. This report describes the case of a 27-year-old correctional officer with advanced human immunodeficiency virus (HIV) infection who presented with chronic headaches, altered mental st

Successful treatment of coccidioidal meningitis with voriconazole.

A previously healthy 47-year-old man with coccidioidal meningitis had fluconazole treatment failure and developed severe symptoms, multiple cranial nerve palsies, and brain-stem inflammation visible on magnetic resonance imaging (MRI). High-dose voriconazole therapy resulted in gradual resolution of almost all signs and symptoms, normalization of cerebrospinal fluid, and clearing of brain-stem edema seen on MRI. The patient had photosensitivity after 10 weeks of treatment, but this improved when the voriconazole dose was lowered. Continuous voriconazole therapy kept coccidioidal meningitis in

Treatment of murine coccidioidal meningitis with SCH39304.

The triazole SCH39304 was compared with itraconazole and fluconazole for treatment of murine coccidioidal meningitis. Mice were treated for 30 days with 1, 5, 10, or 30 mg of each drug per kg of body weight. Survival and brain tissue counts were measured. At equivalent doses, SCH39304 was more effective than the other triazoles.

Imidazole therapy of coccidioidal meningitis in children.

The mortality related to coccidioidal meningitis (CM) has been reduced since the introduction of amphotericin B therapy, but children with CM continue to suffer significant morbidity. Some of this is related to the toxicity of the drug. We report nine children with CM treated with orally administered ketoconazole and intraventricularly administered miconazole. Four of them had been treated initially with amphotericin B with resultant failure in one and severe toxicity in all four. The other five children were treated only with imidazoles. All nine children had evidence of ventriculitis at the
