[Vestibular and cochlear manifestations in Fabry's disease].

Anderson-Fabry's disease corresponds to an inherited disorder transmitted by an X-linked recessive gene. The disease is caused by an alpha-galactosidase deficiency leading to an abnormal glycosphingolipid metabolism, resulting in glycosphingolipids deposits all over the body. The disease affects all organs over the body and can be responsible for central nervous system or renal failure, heart attack, which can lead for early death in absence of diagnosis and treatment. In addition to these life-threatening manifestations, other problems which may have a profound impact on quality of life, such

Cochlear dialysis for Menière's disease. An update.

The early results of hyperosmolar perfusion of the scala tympani for advanced Meniere's disease were reported in 1987. Thirty patients were treated in this way between March 1981 and March 1986; follow-up is now 2 to 7 years. The rationale of cochlear dialysis is to reduce the endolymphatic hydrops of the pars inferior by filing the scala tympani with a sterile solution of mannitol in 5% dextrose that has an osmolality of 350 m.osm per liter, which compares with a scala media osmolality of circa 300 m.osm per liter. The technique is a qualitative and quantitative extension of the "salt in the

Cochlear implantation in a patient with Paget's disease.

Paget's disease of the temporal bone can cause hearing loss that eventually develops into profound deafness. However, the treatment options for hearing loss associated with Paget's disease are limited. Thus far, only 3 cases of cochlear implantation in patients with Paget's disease have been reported. In this study, we describe a 62-year-old woman who underwent cochlear implantation for progressive bilateral sensorineural hearing loss. The patient gained useful open-set speech perception over a 5-year postoperative period. Cochlear implantation can be beneficial in patients with profound heari

Cochlear implantation in the chronically diseased ear.

PURPOSE OF REVIEW: The presence of chronic ear disease in patients with profound SNHL presents a challenge to the otologic surgeon and was considered, in the past, a contraindication to cochlear implantation. This review discusses options for cochlear implantation in patients with chronically diseased ears. RECENT FINDINGS: Several management options are available for cochlear implantation in patients with chronic suppurative otitis media (CSOM) and severe to profound sensorineural hearing loss (SNHL). CONCLUSION: Cochlear implantation can be safely achieved in patients with CSOM. The approa

Diagnosis of cochlear Ménière's disease with electrocochleography.

The existence of cochlear Ménière's disease, once considered a variant of classic Ménière's disease but without vertigo, has been questioned due to lack of objective evidence that endolymphatic hydrops is involved with the disease process. Transtympanic electrocochleography (TT ECoG) has emerged as a useful tool for electrophysiologic monitoring of the inner ear, and is especially valuable in assessing endolymphatic hydrops. A retrospective chart review was performed to identify those patients with a diagnosis consistent with cochlear Ménière's disease in order to determine the presence or abs

Cochlear endolymphatic shunt for Meniere's disease.

We have now performed the cochlear endolymphatic shunt operation for Meniere's disease on 102 ears and have follow-up data on ninety cases. In this fourth report of results of the procedure, we show that after follow-up ranging from one month to six years (average 22.3 months), definitive vertigo was relieved in 72% of the cases and that hearing at the time of most recent audiogram (average 12.6 months postoperatively) was worse (AAO criteria) in 45% of the cases.

Patient Preferences for Long-Term Implant Care in Cochlear, Glaucoma and Cardiovascular Diseases.

In the process of developing and implementing innovative implant technologies the consideration of patient preferences can be beneficial for patients, doctors and developers. Nevertheless, in existing literature, there is still scarce knowledge of patients' perspectives on long-term implant care. In this study, three discrete choice experiments (DCEs) were conducted in the context of cochlear implants (CI, n = 92), glaucoma implants (GI, n = 21) and cardiovascular implants (CVI, n = 23), examining the relative importance of attributes of long-term implant care from the patients' perspective. Cochlear implantation in children with sickle cell disease.

Sickle cell disease (SCD) is associated with sensorineural hearing loss (SNHL). Although the hearing loss is usually mild, some develop severe-to-profound hearing loss, in whom cochlear implants (CI) may be an option. We present the cases of two children with SCD who developed bilateral severe-to-profound SNHL and underwent cochlear implantation. One patient became profoundly deaf after an acute episode of dizziness. Imaging indicated bilateral cochlear ossification, making subsequent cochlear implant surgery challenging. The second patient developed bilateral severe-to-profound SNHL following

Cochlear implantation in patients with advanced Ménière's disease.

BACKGROUND/OBJECTIVES: Ménière's disease (MD) that results in bilateral severe to profound sensorineural hearing loss is a rare indication for cochlear implantation; only a few studies exist documenting performance in these patients. The primary objective was to compare the difference in preoperative to 12-month postoperative speech perception scores among subjects with MD and controls. Groupwise comparisons of secondary postoperative outcomes (Tinnitus Handicap Inventory [THI] scores, 36-Item Short Form [SF-36] scores, and postoperative dizziness) were also performed. METHODS: A retrospectiv
