Diagnostic imaging methods are very important for patients with bilateral sensourinoural hearing loss. Magnetic resonance imaging (MRI) is able to demonstrate the vestibulocochlear nerve and facial nerve in the internal acoustic canal. Also computed tomography can be helpful to determination of the deficiency of the cochlear nerve. Cochlear nerve anomalies are classified into three group according to the magnetic resonance imagings. Patients who have cochlear nerve deficiency may not hear with cochlear amplifications. Auditory brain stem implant (ABI) is most suitable for these cases. In this

Although the parameters that are most important for postoperative speech perception in cochlear implantation have not been identified, it is assumed that the numbers of remaining cochlear neurons and spiral ganglion cells in the implanted deaf ears are critical. In this study, we evaluated the correlation of the maximum diameter of the cochlear and vestibular nerve trunks with the number of spiral ganglion cells in horizontal sections of the temporal bone of 42 patients who were profoundly deaf during life, and in 5 patients with normal hearing. The maximum diameters of the cochlear, vestibula

BACKGROUND AND PURPOSE: High-resolution T2-weighted fast spin-echo MR imaging provides excellent depiction of the cisternal and intracanalicular segments of the vestibulocochlear and facial nerves. Absence or reduction in caliber of the cochlear nerve (deficiency) has been described in association with congenital sensorineural hearing loss (SNHL). Depiction of cochlear nerve integrity may be important for diagnosis and management of SNHL.
METHODS: We retrospectively reviewed high-resolution T2-weighted fast spin-echo MR images of 22 patients examined for SNHL who had deficiency of the cochlea

We present eight cases of FN dysfunction after cochlear implantation. Four mechanisms of injury are proposed. The most common injury occurs from the heat of a bur shaft rotating over the FN in the facial recess. Several suggestions are made to help prevent the type of injuries we have seen. Preoperative imaging and intraoperative FN monitoring may be of general assistance in avoiding injuries to the FN, especially in cases of revision surgery and congenital malformations.

BACKGROUND: Intraoperative monitoring of cochlear nerve action potential (CNAP) has been used in patients with small vestibular schwannoma (<15 mm) to preserve cochlear nerve function. We performed surgery for a larger vestibular schwannoma under CNAP monitoring with the aim of preserving cochlear nerve function, and compared the data with findings from 10 patients with hemifacial spasm who underwent microvascular decompression surgery.
CASE DESCRIPTION: We report the case of a patient with a 26-mm vestibular schwannoma and normal hearing function who underwent neurosurgery under electrophysi

Thirty-eight adult cochlear implants have been performed at the University of Minnesota. Facial nerve stimulation by the implant in response to sound has been noted in four of these cases. Three of the four were patients whose sensorineural hearing loss was caused by cochlear otosclerosis. In each case it was possible to place the electrodes, however multiple leads had to be deprogrammed in order to avoid facial nerve stimulation. In each case characteristic radiographic findings of cochlear otosclerosis could be identified on preoperative temporal bone computed tomography scans. Although faci

Background and Objectives: Many otologists face a dilemma in the decision-making process of surgical management of patients with cochlear nerve (CN) aplasia. The goal of this study is to provide fresh evidence on cochlear implantation (CI) results in patients with CN aplasia. Materials and Methods: We scrutinized functional outcomes in 37 ears of 21 children with bilateral CN aplasia who underwent unilateral or bilateral CI based on cross-sectional and longitudinal assessments. Results: The Categories of Auditory Performance (CAP) scores gradually improved throughout the 3-year follow-up; howe

The outcome of cochlear implantation (CI) in patients with cochlear nerve deficiency (CND) is variable, resulting in a wide range of speech perception performance, from degrees of environmental sound perception to conversation without lip-reading. Twenty-five cochlear implantees with CND were enrolled retrospectively to determine the factors correlated with CI outcome in patients with CND and to develop a predictive model for CI outcome. CI outcome was evaluated using the Categories of Auditory Performance (CAP) score at 2 years after CI. Patients with negative auditory brainstem response (ABR

OBJECTIVE: Cochlear implantation for children with cochlear nerve deficiency remains controversial, as the presence of the cochlear nerve has been central to the success of cochlear implantation. This study sought to investigate whether there is any benefit from cochlear implantation for children with cochlear nerve deficiency.
METHODS: Nine children with cochlear nerve deficiency and bilateral prelingual profound sensorineural hearing loss were included in this study. Inner ear and internal auditory canal structures were evaluated using magnetic resonance imaging and temporal bone computed t

BACKGROUND/PURPOSE: Since otoneurosurgical pathologies are being diagnosed increasingly early, the functional prognosis of surgery is gaining in importance. Hearing and cochlear nerve exploration can be carried out using three principal methods: pure-tone and speech audiometry, evoked otoacoustic emissions (EOAEs), and brainstem auditory evoked potentials (BAEPs). The aim of this paper is to present the current knowledge on auditory functional assessment.
MATERIALS AND METHODS: The medical and scientific literature from the Pubmed-Medline database was reviewed. Recently published related book

Fiber diameters were analyzed in the meatal segment of the cochlear nerve from 7 temporal bones obtained from 7 patients. Two patients had normal hearing for their age. Two had sustained noise exposure and one had presbyacusis of predominantly neural type. The cochleae displayed characteristic degeneration patterns. The other two manifested hearing loss of unspecified type. The fiber diameters ranged from 0.5 to 11 microns. The diameter distribution was unimodal in all seven nerves. The means of the diameters ranged from 4.2 to 5.5 microns. They were significantly different between patients wi

With conventional light and transmission electronmicroscopy we studied 10 cases of acoustic nerve tumour, 3 of which proved to be instances of von Recklinghausen neurofibroma and 7 of schwannoma. Schwannomas were not found to infiltrate the cochlear nerve. Hearing loss, if present in cases of schwannoma, could be related to non-specific lesions of the uninfiltrated cochlear nerve in the vicinity of the vestibular nerve tumour. Only neurofibromas were found to infiltrate the cochlear nerve. Distinction between tumour infiltration and non-specific lesion could be made by electron microscopy.
