"A Drop in the Ocean": A Reflection on the Application of Cognitive Analytic Therapy Within a Prison Setting.

This article reflects on the application of cognitive analytic therapy (CAT) with prisoners referred to a mental health in-reach team within a U.K. prison based on the author's experience as a clinical psychologist. The extent of mental health problems in the prison population and the provision of mental health services within prisons are summarized to highlight the extent of mental health need within this setting. The central concepts and tools of CAT that provided the framework within which this author's work was undertaken within the prison are detailed, namely, reciprocal roles and recipro

Do cognitive complaints either predict future cognitive decline or reflect past cognitive decline? A longitudinal study of an elderly community sample.

Data from a two-wave longitudinal study of an elderly community sample were used to assess whether cognitive complaints either predict subsequent cognitive decline or reflect past cognitive decline. Cognitive complaints and cognitive functioning were assessed on two occasions three and a half years apart. Cognitive complaints at Wave 1 were found not to predict future cognitive change on the Mini-Mental State Examination, an episodic memory test or a test of mental speed. Similarly, cognitive complaints at Wave 2 were unrelated to past cognitive changes on these tests after statistically contr

Cognition beyond relapses: cognitive progression independent of relapse activity reflects early smoldering neurodegeneration in multiple sclerosis.

BACKGROUND: Progression independent of relapse activity (PIRA) is frequent in relapsing-remitting MS patients (RRMS). While often studied in terms of physical disability, cognitive decline occurring without relapses, termed "cognitive PIRA", has also been reported. Cortical/deep gray matter atrophy reflect both clinical PIRA and cognition. This study investigated whether early global/region-specific brain atrophy predicts long-term cognitive decline due to PIRA in RRMS. METHODS: A retrospective cohort study was conducted on RRMS patients followed over time at the Verona MS Centre to evaluate

Multivariate environmental exposures are reflected in whole-brain functional connectivity and cognition in youth.

Each individual's complex, multidimensional environment, known as their "exposome", plays an essential role in shaping cognitive neurodevelopment. Understanding the mechanisms whereby children's exposome influences their development is crucial to facilitate the design of interventions to foster positive developmental trajectories for all youth. Recent work has identified a general exposome factor associated with socio-economic inequality that is strongly related to cognition and individual differences in the spatial organization of functional brain networks in youth. Building on these findings

Serum neurofilament light reflects cognitive dysfunctions in children with obstructive sleep apnea.

BACKGROUND: In children, obstructive sleep apnea (OSA) can cause cognitive dysfunctions. Amyloid-beta and tau are elevated in OSA. Neurofilament light (NfL) is a marker of neuro-axonal damage, but there are no reports of NfL for OSA. The objective was to investigate the serum levels of NfL and tau in children with or without OSA and explore their relationship with cognitive dysfunctions caused by OSA. METHODS: This retrospective case-control study included children diagnosed with adenoid tonsil hypertrophy from July 2017 to September 2019 at the Second Affiliated Hospital of Xi'an Jiaotong Un

N2b Reflects the Cognitive Changes in Executive Functioning After Concussion: A Scoping Review.

Objectives: The N2b is an event-related potential (ERP) component thought to index higher-order executive function. While the impact of concussion on executive functioning is frequently discussed in the literature, limited research has been done on the role of N2b in evaluating executive functioning in patients with concussion. The aims of this review are to consolidate an understanding of the cognitive functions reflected by the N2b and to account for discrepancies in literature findings regarding the N2b and concussion. Methods: A scoping review was conducted on studies that used the N2b to

Cortical Cholinergic Pathway Degeneration Reflects Cognitive Impairment and Dementia in Parkinson's Disease.

BACKGROUND: Cognitive impairment is a prevalent and debilitating feature of Parkinson's disease (PD). Increasing evidence from postmortem and in vivo positron emission tomography imaging studies supports cholinergic dysfunction in PD. There is, however, little evidence regarding the influence of the structural integrity of cortical cholinergic pathways that emanate from the nucleus basalis of Meynert (NBM). OBJECTIVES: To investigate the association between cognition and the structural integrity of medial and lateral cortical cholinergic projection pathways in PD. METHODS: Neuropsychological

Beyond Principles: A Reflective-Cognitive Framework for Ethical Decision-Making in Anorexia Nervosa.

Anorexia nervosa is a clinically complex and ethically challenging psychiatric disorder. Clinicians are frequently confronted with ethical dilemmas arising from conflicts between core ethical principles in everyday clinical practice. Professional codes of ethics and legal frameworks often fail to provide a stable basis for resolving these dilemmas due to the fluctuating medical risk and the ego-syntonic nature of anorexia nervosa. Under conditions of heightened responsibility and medical risk, clinicians' cognitive and emotional responses may be activated and may mediate ethical decision-makin

Modulations of cognitive flexibility in obsessive compulsive disorder reflect dysfunctions of perceptual categorization.

BACKGROUND: Despite cognitive inflexibility is trait like in obsessive compulsive disorder (OCD) patients and underlies clinical symptomatology, it is elusive at what stage of information processing deficits, leading to cognitive inflexibility, emerges. We hypothesize that inhibitory control mechanisms during early stimulus categorization and integration into a knowledge system underlie these deficits. METHODS: We examined N = 25 adolescent OCD patients and matched healthy controls (HC) in a paradigm manipulating the importance of the knowledge system to perform task switching. This was done

Prefrontal neurons transmit signals to parietal neurons that reflect executive control of cognition.

Prefrontal cortex influences behavior largely through its connections with other association cortices; however, the nature of the information conveyed by prefrontal output signals and what effect these signals have on computations performed by target structures is largely unknown. To address these questions, we simultaneously recorded the activity of neurons in prefrontal and posterior parietal cortices of monkeys performing a rule-based spatial categorization task. Parietal cortex receives direct prefrontal input, and parietal neurons, like their prefrontal counterparts, exhibit signals that

Brooding and reflective rumination among suicide attempters: cognitive vulnerability to suicidal ideation.

Previous research suggests that the brooding subtype of rumination is associated with increased suicidal ideation, but findings are inconsistent with respect to reflection, considered to be the more adaptive form of rumination. This study investigated the circumstances under which reflective rumination might be associated with increased suicidal ideation by examining whether a suicide attempt history moderated the relationship between the ruminative subtypes and current suicidal ideation. Thirty-seven young adults who reported a past suicide attempt and fifty-nine young adults without a suicid

Implementing a Cognitively Grounded Artificial Moral Advisor: A Multi-LLM Multi-Agent Approach Based on the Cognitive-Reflective Equilibration Model.

Large language model (LLM)-based artificial intelligence is increasingly used in ethically consequential human decision-making, yet fully autonomous machine ethics remains unrealistic, motivating architectures that support rather than replace human ethical judgment. This study introduces the Cognitive-Reflective Equilibration Architecture (CREA), a cognitively grounded artificial moral advisor that operationalizes the Cognitive-Reflective Equilibration Model (CREM), in which reflective reasoning guides ethical judgment from intuitive cognition toward a more advanced equilibrium among competing

Early Gut Microbiome Alterations in Mild Cognitive Impairment Reflect Changes in Alzheimer Disease.

INTRODUCTION: Alterations in the gut-brain axis have been increasingly linked to neurodegenerative diseases, including Alzheimer disease (AD). It remains unclear whether these microbiome changes are already present during early cognitive decline. We examined whether gut microbiome alterations characteristic of AD are detectable in mild cognitive impairment (MCI) and whether these changes follow a similar pattern across the cognitive continuum. METHODS: This case-control study included 78 participants: 37 cognitively healthy controls, 20 individuals with MCI, and 21 individuals with prodromal

Cognitive development in preterm infants: multifaceted deficits reflect vulnerability of rigorous neurodevelopmental pathways.

Prematurity remains the major cause of neonatal morbidity and mortality, with 15 million preterm births occurring worldwide in 2010. Infants born less than 37 weeks gestation are at high risk of abnormal neurodevelopmental outcomes, given that the central nervous system is extremely sensitive to an abnormal intra- and extra-uterine environment. Children born preterm have multiple neurodevelopmental sequelae involving dynamic and complex cognitive deficits. Former preterm infants have difficulty with each domain of cognition, including executive function, language, learning and memory, complex

Assessing the association between homocysteine and cognition: reflections on Bradford Hill, meta-analyses, and causality.

Hyperhomocysteinemia is a recognized risk factor for cognitive decline and incident dementia in older adults. Two recent reports addressed the cumulative epidemiological evidence for this association but expressed conflicting opinions. Here, the evidence is reviewed in relation to Sir Austin Bradford Hill's criteria for assessing "causality," and the latest meta-analysis of the effects of homocysteine-lowering on cognitive function is critically examined. The meta-analysis included 11 trials, collectively assessing 22,000 individuals, that examined the effects of B vitamin supplements (folic a

Prefrontal activity in Huntington's disease reflects cognitive and neuropsychiatric disturbances: the IMAGE-HD study.

Functional integrity of prefrontal cortico-striatal circuits underlying executive functioning may be compromised by basal ganglia degeneration during Huntington's disease (HD). This study investigated challenged inhibitory attentional control with a shifting response-set (SRS) task whilst assessing neural response via functional magnetic resonance imaging (fMRI) in 35 healthy controls, 35 matched pre-symptomatic (pre-HD) and 30 symptomatic (symp-HD) participants. A ≥70% performance accuracy threshold allowed confident identification of neural activity associated with SRS performance in a sub-s
