Abdominal Aortic Junctional Tourniquets: Clinically Important Increases in Pressure in Aortic Zone 1 and Zone 3 in a Cadaveric Study Directly Relevant to Combat Medics Treating Non-Compressible Torso Hemorrhage.

BACKGROUND: "Non-compressible" torso hemorrhage (NCTH) is the leading cause of preventable battlefield death, requiring rapid surgical or radiological intervention, which is essentially precluded close to the point of injury. UK Joint Theatre Trauma Registry (JTTR) analysis 2002-2012 showed 85.5% NCTH mortality. JTTR vascular injury data 2003-2008 revealed 100% mortality in named truncal vessel injuries. Gas insufflation and hyper-pressure intraperitoneal fluid animal studies have demonstrated significant reductions in blood loss in splanchnic injuries. We hypothesized that the noninvasive Abd

The National Disaster Medical System and military combat readiness: A scoping review.

A scoping review was conducted to describe the history of the National Disaster Medical System (NDMS) in the context of US military medical preparedness for a large-scale overseas military conflict. National Disaster Medical System civilian hospitals would serve as backups to military treatment facilities if both US Department of Defense and US Department of Veterans Affairs hospitals reached capacity during such a conflict. Systematic searches were used to identify published works discussing the NDMS in the scientific and gray literature. Results were limited to publicly available unclassifie

Adaptability of Medical Surge Preparedness Practices to Large-Scale Combat Operations.

Since its inception, the National Disaster Medical System (NDMS) has been activated in the context of acute-onset and time-limited disasters; however, its original intended purpose was for providing care for casualties from a large-scale combat operation (LSCO). Consequently, determining the utility of specific medical surge practices for an LSCO is necessary to optimally prepare the NDMS for such an event. Lessons learned from prior medical surge events can inform LSCO preparations despite the contextual differences between the 2 scenarios. This case study builds on earlier analyses by the US

'Golden day' is a myth: rethinking medical timelines and risk in large scale combat operations.

The evolving landscape of battlefield medicine forces medical planners to prepare for large-scale combat operations (LSCO) against peer adversaries, requiring reassessment of recent medical strategies. Despite lacking medical backing, the term 'golden day' has been used by senior military leaders to link the resuscitative benefits of the 'golden hour' to prolonged medical care through similar nomenclature. Pseudomedical terminology can easily enter the lexicon of commanders as attractive soundbites. However, articulating the evidence-based factors influencing mortality on the battlefield is cr

Diagnostic and therapeutic strategies in combating implanted medical device-associated bacterial biofilm infections.

Bacterial biofilms exhibit remarkable resistance against conventional antibiotics and are capable of evading the humoral immune response. They account for nearly 80% of chronic infections in humans. Development of bacterial biofilms on medical implants results in their malfunctioning and subsequently leads to high mortality rates worldwide. Therefore, early and precise diagnosis of bacterial biofilms on implanted medical devices is essential to prevent their failure and associated complications. Culture-based methods are time consuming, more prone to contamination and often exhibit low sensiti

Improving combat medic learning using a personal computer-based virtual training simulator.

The new military occupational specialty for combat medics, the 91W, requires that all medics successfully pass the National Registry of Emergency Medical Technicians examination. The objective of this study was to supplement standard emergency medical technician training with a three-dimensional, computer-based, virtual training simulator and to assess whether scores and pass rates could be increased. Combat medics (N=167) were selected for training through the usual noncommissioned officer unit selection process and were randomized by cohort to the training simulator. Results showed no signif
