Stenosis of the common bile duct due to Ormond's disease: case report and review of the literature.

In a 46-year-old man endoscopic retrograde cholangiopancreatography and computed tomography scan showed a stenosis of the common bile duct by a hypodense mass highly suggestive of a Klatskin tumor. Histologic examination of the resected tumor revealed only non-specific inflammatory, fibrotic tissue without any evidence of malignancy. Three months later, the patient presented with hydronephrosis of the left kidney. Computed tomography scan showed a retroperitoneal mass with encasement of the left ureter. A percutaneous nephrostomy was performed and immunosuppressive therapy with prednisolone an

[Choledochoduodenostomy in the treatment of benign disease of the common bile duct: evaluation of long-term results].

The Authors report their experience in the treatment of 36 patients managed by side-to-side choledochoduodenostomy for common bile duct disease and followed for a period of five years. The main indication to surgery was a choledochal dilatation more than 1.5 cm. The surgical technique consisted in a single layer side-to-side choledochoduodenostomy with absorbable suture according to Roessner. There was no operative mortality and few post surgical complications (six patients, 15.78%) were registered; 25 patients (69.45%) were completely asymptomatic at five years, while in six cases (16.66%) si

Common bile duct stenosis and pancreatitis in Kawasaki disease: two cases and a review of the literature.

We report 2 children with Kawasaki disease, who developed pancreatitis and common bile duct abnormalities. After reviewing the literature, we conclude that Kawasaki disease may lead to such changes as a result of the underlying vasculitis characterizing this disease, or of direct inflammation of the biliary tree.

Gallstone disease: Epidemiology, pathogenesis, and classification of biliary stones (common bile duct and intrahepatic).

Gallstones are common in Western countries and Japan. Most gallstones are found in the gallbladder, but they sometimes pass through the cystic duct into extrahepatic and/or intrahepatic bile ducts to become bile-duct stones, causing conditions known as choledocholithiasis and hepatolithiasis. Some 10-15% of gallstone patients concomitantly suffer from bile-duct stones. Bile-duct stones can also be formed in the absence of gallbladder stones, and such primary bile-duct stones are more common in East Asian countries than in the Western world. Thus pathogenesis of primary and secondary bile-duct

Beta-glucuronidase-producing bacteria in bile from the common bile duct in patients treated with endoscopic papillotomy for gallstone disease.

This paper reports the occurrence of beta-glucuronidase-producing bacteria in the bile in gallstone patients treated with endoscopic papillotomy (EPT). The study included 36 patients--18 women and 18 men, aged 43-87 years, with a median of 72.5 years. Bile sampling was done with an endoscopic technique. All bacterial strains were tested for beta-glucuronidase activity with a rapid chromogenic tablet test, using 4-nitrophenyl-beta-D-glucuronic acid as substrate. Bacterial growth was found in the bile in 35 patients. Of 103 strains isolated, 30 produced beta-glucuronidase. Twenty-five of the pat

Visibility and Size of the Pancreatic Duct and Common Bile Duct on Routine 2D MRI Sequences in Children Without Pancreaticobiliary Disease.

BACKGROUND. Pancreatic duct (PD) or common bile duct (CBD) dilatation can indicate ductal pathology, but limited data describe normal pediatric duct measurements on routine 2D MRI sequences. OBJECTIVE. The purpose of this study was to characterize the visibility and diameter of the PD and CBD on 2D MR images in children without pancreaticobiliary disease. METHODS. This retrospective study included patients who underwent abdominal MRI using a rapid protocol (composed of noncontrast axial and coronal 2D SSFSE sequences) to assess for suspected appendicitis or ovarian torsion in the emergency dep

High incidence of common bile duct dilatation in autosomal dominant polycystic kidney disease patients.

Two autosomal dominant kidney disease (ADPKD) patients with cholecystitis or communicating extrahepatic dilatation of the common bile duct accompanied by tiny common bile duct stones prompted us to examine the incidence of gall stone disease and dilatation of the common bile duct in ADPKD patients. Computed tomography scans were examined retrospectively in 55 ADPKD patients and 55 age-, gender-, and duration of dialysis-matched non-ADPKD patients. The incidence of calcium-containing gall stones found on tomography scans was the same: eight of the 55 ADPKD patients and nine of the 55 non-ADPKD

Bacteriological studies of common duct bile in patients with gallstone disease and juxta-papillary duodenal diverticula.

Bile samples from the common duct were collected at cholecystectomy and cultivated aerobically and anaerobically. Two groups of patients were studied, patients with juxta-papillary duodenal diverticula and patients without diverticula. All patients had chronic cholecystitis and concrements in the gallbladder, but no biliary obstruction. A significantly higher incidence of positive cultures was found in patients with diverticula than in the control group. The positive samples contained bacterial species belonging to the intestinal flora. We propose an ascending route of infection and assume tha

Disease-Based Risk Stratification of Postendoscopic Retrograde Cholangiopancreatography Pancreatitis for Common Bile Duct Stones.

BACKGROUND: Risk stratification of postendoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP) for common bile duct (CBD) stones is needed for clinicians to adequately explain to patients regarding the risk of PEP in advance of ERCP and to proactively take preventive measures in high-risk patients. AIMS: To stratify the risk of PEP for CBD stones based on CBD-related diseases. METHODS: A total of 1551 patients with naïve papilla who underwent ERCP for CBD stones were divided into three groups: Group A: asymptomatic CBD stones, Group B: obstructive jaundice and elevated liver

Duodenal Crohn's disease complicated by common bile duct obstruction: report of a case and review of the literature.

A 28-year-old woman with Crohn's disease involving the duodenum and terminal ileum presented with biochemical evidence of incomplete, extrahepatic biliary obstruction. Percutaneous transhepatic cholangiography showed partial obstruction of the intramural duodenal portion of the common bile duct. Cholangiography was complicated by cholangitis. Surgical decompression of the biliary system and bypass of the affected duodenal segment were performed after intraoperative confirmation of Crohn's disease involving the ileum, duodenum, and ampulla of Vater. The patient made a complete recovery and all

Extensive primary retroperitoneal fibrosis (Ormond's disease) with common bile duct and ureteral obstruction: A rare case report.

INTRODUCTION: Idiopathic retroperitoneal fibrosis (Ormond's disease) may involve the perirenal tissue, mesentery and biliary system in extremely uncommon situations in addition to classical compression of retroperitoneal structures particularly the ureters. PRESENTATION OF CASE: We report the case of a 60 year's old man with clinical manifestation of obstructive jaundice, due to the common bile duct narrowing caused by a primary retroperitoneal fibrosis. Magnetic resonance cholangiopancreatography (MRCP) showed the presence of intrahepatic bile duct obstruction, suggesting the diagnosis of a

Choledocholithiasis Is a Surgical Disease: The Evolution, Standardization, and Impact of Laparoscopic Common Bile Duct Exploration in the Acute Care Surgery Setting.

BACKGROUND: Acute care surgeons can provide a "surgery first-one stop" approach to choledocholithiasis using laparoscopic common bile duct exploration (LCBDE). We describe the implementation and evolution of a catheter-based LCBDE pathway within an academic acute care surgery (ACS) practice, shifting management of choledocholithiasis back into the hands of surgeons. STUDY DESIGN: This retrospective analysis reviewed patients who underwent LCBDE by our ACS practice between 2019 and 2025. Before 2019, choledocholithiasis was routinely managed with ERCP either before or after cholecystectomy. A
