Statistical analysis of case-control studies of communicable diseases.

At the Communicable Disease Surveillance Centre most outbreak investigations are carried out by questioning cases and unaffected 'controls' to look for associations with possible sources of infection. Illness rates are compared in those exposed with those not exposed to a possible risk factor, using statistical techniques appropriate to the survey design. Significance testing to obtain evidence of the source of transmission is made as quickly as possible so that action may be taken. Microbiological corroboration is sought wherever possible. Unlike chronic disease epidemiology the estimation of

Investigating a single case of Legionnaires' disease: guidance for consultants in communicable disease control.

When a single case of legionnaires' disease is reported, it should be investigated to check whether or not it is linked to other cases or part of an outbreak. The investigation includes confirmation of the diagnosis, tracing the patient's movements during the incubation period, and reporting the case to the National Surveillance Scheme for Legionnaires' Disease at the PHLS Communicable Disease Surveillance Centre. If no common factors are identified between the cases and other cases reported previously, no further action is usually required, unless it is suspected that the infection was acquir

Surveillance and control of communicable diseases related to passenger ships in Europe.

This study was conducted within the European SHIPSAN (ship sanitation) project and aims at exploring the legislative framework and current practices related to surveillance of communicable diseases and response aboard passenger ships in Europe. A detailed questionnaire was disseminated and completed by 59 competent authorities in 27 European countries. The majority of competent authorities used national legislation without special provisions for passenger ships. Only 20% had specific provisions for ships regarding quarantine, while a smaller proportion of new Member States (MS) have specific l

An outbreak of syphilis in Alabama prisons: correctional health policy and communicable disease control.

OBJECTIVES: After syphilis outbreaks were reported at 3 Alabama State men's prisons in early 1999, we conducted an investigation to evaluate risk factors for syphilis infection and describe patterns of syphilis transmission. METHODS: We reviewed medical, patient interview, and prison transfer records and documented sexual networks. Presumptive source cases were identified. Odds of exposure to unscreened jail populations and transfer from other prisons were calculated for case patients at 1 prison. RESULTS: Thirty-nine case patients with early syphilis were identified from 3 prisons. Recent j

Communicable diseases in the Eastern Mediterranean Region: prevention and control 2010-2011.

One-third of all morbidities and mortalities in the Eastern Mediterranean Region are attributed to communicable diseases. A continued situation of war and conflict, and growing political unrest in the Region, coupled with factors such as travel and migration, and insufficient infrastructure and inadequate technical and managerial capacity ofthe programmes are the major challenges. Despite these challenges, the Region continued making progress towards the elimination of specific diseases such as lymphatic filariasis, measles, malaria, schistosomiasis and dracunculiasis during 2010-11. Using Social Networking Sites for Communicable Disease Control: Innovative Contact Tracing or Breach of Confidentiality?

Social media applications such as Twitter, YouTube and Facebook have attained huge popularity, with more than three billion people and organizations predicted to have a social networking account by 2015. Social media offers a rapid avenue of communication with the public and has potential benefits for communicable disease control and surveillance. However, its application in everyday public health practice raises a number of important issues around confidentiality and autonomy. We report here a case from local level health protection where the friend of an individual with meningococcal septica

The reporting of communicable diseases: a controlled study of Neisseria meningitidis and Haemophilus influenzae infections.

Surveillance systems for communicable diseases in the United States are primarily passive. We compared the passive reporting system for invasive disease caused by Neisseria meningitidis and Haemophilus influenzae with a concurrent, active laboratory-based system in the four metropolitan counties of Tennessee. The passive reporting system identified approximately 50% of all cases that were identified by the active system and accurately reflected trends in disease occurrence during the study period. Of all reported cases, physicians contributed fewer than 4%. Nearly 40% of all hospitals in the s

Inclusion of older adults in national cancer control and non-communicable disease plans: A global analysis.

BACKGROUND: National cancer control plans and non-communicable disease strategies are key instruments to guide cancer policy and resource allocation. However, the extent to which these frameworks explicitly address older adults remains unclear. METHODS: We conducted a structured content analysis of national cancer control plans and non-communicable disease strategies identified through the International Cancer Control Partnership database. Plans were screened for inclusion of older adult-specific provisions, including explicit mention, age-specific interventions, geriatric assessment, workfor

Comparison of the Hungarian and Scottish communicable disease control systems: lessons for a convergent European Community.

BACKGROUND: European public health systems are converging, particularly in relation to communicable disease control. This process requires mutual learning through comparison; this was undertaken for Scotland (population 5.1 million) and Hungary (population 10.5 million). METHODS: Using the official web- and paper-based publications, the practice of communicable disease control was compared between the two countries in three specific fields: seasonal influenza surveillance; human immunodeficiency virus (HIV) surveillance; and the childhood vaccination system. RESULTS: The organization structu

[Summer 1997 viral meningitis in children Frankfurt/Main--a discussion of the revised communicable disease control regulation].

According to the current law of preventing infectious diseases in Germany, bacterial as well as aseptic meningitis have to be reported to the health authorities, whereas according to the amendment aseptic meningitis will no longer be notifiable. In Summer 1997, 63 children with aseptic meningitis were reported to the local health authorities of Frankfurt compared with 4 or 5 per year during the preceding years. In 40% enterovirus, resp. ECHO-30 virus could be detected. The local health authorities gave detailed information on the medical relevance as well as necessary hygienic measures to the

Importance of intersectoral co-ordination in the control of communicable diseases, with special reference to plague in Tanzania.

Human health, agriculture, including livestock, energy, education, wildlife, construction, forestry and trade sectors are inter-related and their co-ordination is an important pre-requisite for successful control of most communicable diseases including plague. Similar linkage between research, policy, training and extension activities in each sector are essential for any successful control strategy. Inadequate agricultural produce, inaccessibility of people to the available food and ignorance on proper preparation and usage of available food materials are responsible for malnutrition, and maln
