The Community Paramedicine at Clinic Program: Improving Participant Health while Preserving Healthcare System Resources.

Vulnerable populations such as low-income older adults in social housing suffer from poor quality of life and are impacted by chronic diseases. These populations are also high users of emergency services, which contribute to high healthcare costs. Community-based, patient-centred interventions, such as community paramedicine (CP) programs, can address the healthcare gaps for these underserved populations. Community Paramedicine at Clinic (CP@clinic) is an innovative, evidence-based, chronic disease prevention/management program that improves patient health and quality of life, connects them wi

Community-partnered cluster-randomized comparative effectiveness trial of community engagement and planning or resources for services to address depression disparities.

BACKGROUND: Depression contributes to disability and there are ethnic/racial disparities in access and outcomes of care. Quality improvement (QI) programs for depression in primary care improve outcomes relative to usual care, but health, social and other community-based service sectors also support clients in under-resourced communities. Little is known about effects on client outcomes of strategies to implement depression QI across diverse sectors. OBJECTIVE: To compare the effectiveness of Community Engagement and Planning (CEP) and Resources for Services (RS) to implement depression QI on

Malignant Hyperthermia in a Morbidly Obese Patient Depletes Community Dantrolene Resources: A Case Report.

During resection of a duodenal carcinoid tumor, a 28-year-old morbidly obese woman developed suspected malignant hyperthermia. This hypermetabolic state posed a diagnostic challenge given the similar intraoperative presentation of carcinoid crisis and malignant hyperthermia. The patient's weight posed therapeutic challenges as massive doses and prolonged administration of dantrolene were required that quickly depleted the available supply. Current dantrolene dosing recommendations are based on actual body weight despite a paucity of literature in obese patients. We speculate that the prolonged

Macrophages.com: an on-line community resource for innate immunity research.

Macrophages play a major role in tissue remodelling during development, wound healing and tissue homeostasis, and are central to innate immunity and to the pathology of tissue injury and inflammation. Given this fundamental role in many aspects of biological function, an enormous wealth of information has accumulated on these fascinating cells in the literature and other public repositories. With the escalation of genome-scale data derived from macrophages and related haematopoietic cell types, there is a growing need for an integrated resource that seeks to compile, organise and analyse our c

Examining the referral of patients with elevated blood pressure to health resources in an under-resourced community in South Africa.

BACKGROUND: Low-and-middle income countries face a disproportionate burden of non-communicable diseases (NCDs) that threaten to overwhelm under-resourced health systems. Community health workers (CHWs) can promote NCD prevention, reach patients, and connect them to local community health resources; however, little has been done to examine how referrals to these resources are utilized by community members. The purpose of this study is to examine the use of referrals to community-based health resources and investigate the factors influencing patient utilization of referrals connecting them to ap

A Collaborative Approach to Creating Knowledge-Building Resources for the Early-Stage Parkinson's Disease Community.

INTRODUCTION: Parkinson's disease is a progressive neurodegenerative condition; there are no treatments currently available to slow disease progression, and no cure. Scientists are studying new treatments, such as disease-modifying therapies (DMTs), that may help slow progression of Parkinson's. Here, we discuss our approach to initiating a collaboration between industry and the Parkinson's community that explores the needs of people with early-stage Parkinson's and their care partners. The aim was to examine ways to build knowledge for newly diagnosed patients, align on the potential of DMTs

Modelling community-control strategies to protect hospital resources during an influenza pandemic in Ottawa, Canada.

BACKGROUND: A novel influenza virus has emerged to produce a global pandemic four times in the past one hundred years, resulting in millions of infections, hospitalizations and deaths. There is substantial uncertainty about when, where and how the next influenza pandemic will occur. METHODS: We developed a novel mathematical model to chart the evolution of an influenza pandemic. We estimate the likely burden of future influenza pandemics through health and economic endpoints. An important component of this is the adequacy of existing hospital-resource capacity. Using a simulated population re

Physiotherapists in under-resourced South African communities reflect on practice.

Increasing access to health services and the introduction of primary health care are the cornerstones of transforming healthcare initiatives in South Africa. Sustainability of healthcare provision in rural and under-resourced areas is secured by legislation of a year-long community service (CS) contract with graduating healthcare practitioners. This study explored how the first cohort of physiotherapists experienced their year-long CS and how they felt their undergraduate training prepared them for practice. A survey was conducted at the end of 2003 amongst the first cohort of CS physiotherapi

Resource availability alters breeding strategies in a small mammal community.

Following a resource pulse, animals may finance reproduction by consuming concurrently available resources (income breeding) or by storing resources for future reproduction (capital breeding). Understanding how these reproductive strategies are used is important for determining the ecological mechanisms that structure timing of reproduction and that drive interannual population fluctuations in animals. We gathered a reproductive dataset for five small mammal species over a 12-year period in Northeastern USA during which six masting events of American beech (Fagus grandifolia) and eastern hemlo
