The high cost of direct-acting antiviral-based regimens raises concerns about the outcome of treatment in uninsured patients with chronic hepatitis C virus (HCV) infection. This study assessed the relationship between health insurance status and sustained virologic response (SVR) rates in a community hospital in South Florida. Sofosbuvir-based therapy was initiated in 82 patients, of which 73% were uninsured and 28 (34%) were HIV coinfection. The overall SVR rate for those tested was 98%. The SVR rates were similar between HCV mono- and HCV/HIV coinfected patients (96% versus 100%, P = .204).

BACKGROUND: The implementation of community-based health insurance in (CBHI) in Rwanda has reduced out of pocket (OOP) spending for the > 79% of citizens who enroll in it but the effect for surgical patients is not well described. For all but the poorest citizens who are completely subsidized, the OOP (out of pocket) payment at time of service is 10%. However, 55.5% of the population is below the international poverty line meaning that even this copay can have a significant impact on a family's financial health. The aim of this study was to estimate the burden of OOP payments for cesarean sect

BACKGROUND: Community-Based Health Insurance (CBHI) schemes are recognized as an important health financing pathway to achieving universal health coverage (UHC). Although previous studies have documented CBHIs in low-income countries, the majority of these have been provider-based. Non-provider based schemes have received comparatively less empirical attention. We sought to describe a novel non-provider based CBHI munno mubulwadde (your friend indeed) comprising informal sector members in rural central Uganda to understand the structure of the scheme, the experiences of scheme members in terms

Community-acquired pneumonia (CAP) is an acute pulmonary parenchymal infection acquired outside the hospital. The utility of blood cultures in inpatients with CAP to reduce mortality and length of hospital stay is controversial. This study aimed to determine the utility of blood cultures on the first day of hospitalization for CAP inpatients and its influence on mortality, length of hospital stay, and antibiotics use. We conducted a fact-finding survey on the implementation of blood culture in inpatients with CAP in Japan. A propensity score (PS)-matched analysis based on the National Database

BACKGROUND: Despite the global target of 80% glycemic control among people diagnosed with diabetes in 2030, diabetes treatment coverage and control rate were still low. Diabetes imposes a substantial economic burden on health systems, patients, and their families. Due to low health insurance coverage in developing countries, the expenses related to diabetes care often result in significant out-of-pocket costs for patients. OBJECTIVE: To assess the level of out–of–pocket expenditure and the effect of community-based health insurance (CBHI) on out-of-pocket (OOP) expenditure among diabetic patie

BACKGROUND: Cardiovascular diseases (CVD) are a leading contributor to the burden of disease in low- and middle-income countries. Guidelines for CVD prevention care in low resource settings have been developed but little information is available on strategies to implement this care. A community health insurance program might be used to improve patients' access to care. The operational research project "QUality Improvement Cardiovascular care Kwara - I (QUICK-I)" aims to assess the feasibility of CVD prevention care in rural Nigeria, according to international guidelines, in the context of a co

Low health insurance coverage has remained a persistent challenge in Nigeria, restricting access to healthcare despite the coexistence of various insurance schemes. While previous research has primarily focused on the National Health Insurance Scheme (NHIS) or community-based health insurance (CBHI), few studies have collectively examined the factors influencing enrolment across different types of insurance-namely, community-based, employer-sponsored, and private schemes. This study addresses this research gap using household data from the 2018 Nigeria Demographic and Health Survey (DHS). Empl

In California, health and community-based organizations serve as vital intermediaries, helping Latino immigrants navigate Medi-Cal (the state's Medicaid program) enrollment and access to care. The 2024 Medi-Cal expansion extended eligibility to undocumented immigrants who meet income criteria. Despite this policy shift, persistent barriers to care remain at the individual, insurance system, and organizational levels. This study examines Latino immigrants' navigation of health insurance and access to care in California. As part of the MIRA Latino study, we interviewed leaders from 32 California

IMPORTANCE: Underserved populations, especially Hispanics along the U.S.-Mexico border, are systematically excluded from early hearing loss detection efforts. Without targeted, culturally informed approaches, existing health disparities will only be exacerbated. This study addresses a critical gap in equitable access to hearing health care for Hispanic individuals in the United States.
OBJECTIVE: The aim of this study was to examine the outcomes of community-based hearing screenings embedded within a mobile health outreach program in El Paso, Texas, and identify demographic predictors of scre

From 2000 to 2010, Rwanda implemented comprehensive health sector reforms to strengthen the public health system, with the aim of reducing maternal and newborn deaths in line with Millennium Development Goal 5, among many other improvements in national health. Based on a systematic review of the literature, national policy documents and three Demographic & Health Surveys (2000, 2005 and 2010), this paper describes the reforms and the policies they were based on, and provides data on the extent of Rwanda's progress in expanding the coverage of four key women's health services. Progress took pla

BACKGROUND: Out of pocket payments are the predominant method of financing healthcare in many developing countries, which can result in impoverishment and financial catastrophe for those affected. In 2010, WHO estimated that approximately 100 million people are pushed below the poverty line each year by payments for healthcare. Micro health insurance (MHI) has been used in some countries as means of risk pooling and reducing out of pocket health expenditure. A systematic review was conducted to assess the extent to which MHI has contributed to providing financial risk protection to low-income

BACKGROUND: Maternal mortality is a major public health problem worldwide especially in low income countries. Most causes of maternal deaths are due to direct obstetric complications. Maternal mortality ratio remains high in Rukungiri district, western Uganda estimated at 475 per 100,000 live births. The objectives were to identify types of community involvement and examine factors influencing the level of community involvement in the management of obstetric emergencies.
METHODS: We conducted a descriptive study during 2nd to 28th February 2009 in rural Rukungiri district, western Uganda. A t

The Hispanic population in the United States is growing rapidly but this population has many health care needs that are not being met. The findings from recent research on the current health status of Hispanic people who live in the United States are presented. An assessment of how accessible and available medical care services are to Hispanic people is made. Serious gaps exist in the delivery of medical care services to this group. Human service providers, particularly social workers, can help make the current health care system more responsive to the needs of this group by helping Hispanic i

BACKGROUND: The prevalence of vaccine-preventable diseases in adults remains a significant public health issue in the United States. The objectives of this demonstration project were to increase the number of influenza, pneumococcal, pertussis, and herpes zoster immunizations administered to adults by community pharmacists, evaluate the percentage of immunizations documented in the state immunization information system (IIS), and design and pilot a value-based payment model.
METHODS: A one-year prospective, observational demonstration project was conducted in 70 community pharmacies in wester

BackgroundAccess to credible health information is critical for health literacy and informed health behaviors. Although digital platforms are widely used, limited technology access and lower eHealth literacy may constrain online health information seeking in some communities. Black and low-income individuals, in particular, may rely less on internet-based sources and more on in-person channels, including social networks and community-based organizations.AimsThis study examined community-based sources of health information used by Black individuals living in a low-income, urban area.MethodsSemi

1. Through a community based program, nurses were hired by a rural medical center to facilitate care for injured workers in the community. This placed the nurses outside the industrial and insurance arenas. 2. Rapid access to care and expedited return to work were made possible largely through improved communication, facilitated by the nurse case coordinators, among all involved parties (i.e., workers, employers, health care providers, insurance carriers). 3. The program provides access to occupational health nurses, or case managers, to a large number of rural workers--many of whom work for s

Full parity of health insurance benefits for treatment of mental illness, including substance use disorders, is a major achievement. However, the newly-published regulations implementing the legislation strongly endorse aggressive managed care as a way of containing costs for the new equality of coverage. Reductions in "very long episodes of out-patient care," hospitalization, and provider fees, along with increased utilization, are singled out as achievements of managed care. Medical appropriateness as defined by expert medical panels is to be the basis of authorizing care, though clinicians

OBJECTIVE: To describe sources of health care used by homeless and housed poor adults.
DESIGN: In a cross-sectional survey, face-to-face interviews were conducted to assess source of usual care, preferred site of care for specific problems, perceived need for health insurance at different sites of care, and satisfaction with care received. Polychotomous logistic regression analysis was used to identify the factors associated with selecting non-ambulatory-care sites for usual care.
SETTING: Twenty-four community-based sites (i.e., soup kitchens, drop-in centers, and emergency shelters) freque

OBJECTIVE: This study examines whether school-based health center (SBHC) access mitigates insurance-based disparities in preventive care utilization, including well-child visits and influenza (flu) vaccination, among students with primary care visits in a rural region of New York State.
METHODS: Using 2024 electronic health records from a regional healthcare network, we analyzed 15,267 students aged 5-18. Students were categorized by SBHC access and insurance type (Medicaid vs. commercial). Logistic regressions estimated preventive care utilization by SBHC access and insurance type, with marg
