[Complex regional pain syndrome, CRPS].

Complex regional pain syndrome, CRPS, occurs with severe disabling pain, usually in the leg or hand, coupled with changes in pain perception, hyperesthesia and allodynia. There is as well, edema, changes in the color of the skin, trophic changes, and dystonia. The pain syndrome is often triggered by minor trauma. The pain perception is severe and out of context with the initial trauma. The syndrome is rare, occurring in a population-based study in the United States, with an incidence of only 5.5 per hundred thousand people per year. The incidence in Iceland, from the National Register of Disea

Outcome of the complex regional pain syndrome.

OBJECTIVES: The outcome of complex regional pain syndrome (CRPS) is relatively unknown. High disease resolution rates have been reported, but also long-lasting impairments in many patients. This study aims to assess CRPS outcome in a population-based cohort of CRPS patients. METHODS: CRPS patients were retrospectively identified (1996 to 2005) in a Dutch general practitioners database, the integrated primary care information project, and included if at onset (ie, in the past) they had complied with the International Association for the Study of Pain (IASP) diagnostic criteria. The disease sta

Pediatric complex regional pain syndrome.

Complex regional pain syndrome (CRPS) is a relatively new diagnostic entity in pediatrics. There is debate as to what constitutes the most effective treatment for pediatric CRPS. This study presents the patient characteristics, clinical course, and treatment outcome of 20 children diagnosed with CRPS at a major children's hospital during a 4-year period. The results showed that pediatric CRPS occurs predominantly in girls (90%) in later childhood and adolescence (mean age, 11.8 [range, 8-16 years]). It affects mainly the lower limbs (85%), with a predilection for the foot (75% of all cases), a

Complex Regional Pain Syndrome: An update.

Complex regional pain syndrome (CRPS) is a perplexing painful syndrome of the extremities usually following a harmful event. It is distinguished in two types, mainly depending on the presence of nerve injury. Although its prevalence may vary depending on social and ethnic factors, middle-aged women seem to suffer most often and the upper limb is the most commonly affected extremity. Apart from pain, which is the dominating feature, the clinical picture unfolds across several domains: sensory, motor, autonomic and trophic. This syndrome develops in two phases, the acute (warm) phase, with the c

Treatment of complex regional pain syndrome.

Complex Regional Pain Syndrome (CRPS) is a multifactorial and disabling disorder with complex etiology and pathogenesis. Goals of therapy in CRPS should be pain relief, functional restoration, and psychological stabilization, but early interventions are needed in order to achieve these objectives. Several drugs have been used to reduce pain and to improve functional status in CRPS, despite the lack of scientific evidence supporting their use in this scenario. They include anti-inflammatory drugs, analgesics, anesthetics, anticonvulsants, antidepressants, oral muscle relaxants, corticosteroids,

Prednisolone in complex regional pain syndrome.

BACKGROUND: Although there are several studies of systemic corticosteroid therapies in various doses and various durations in complex regional pain syndrome (CRPS), the outcome measurement parameters are limited to the range of motion measurements, edema, and symptoms of CRPS. OBJECTIVE: To investigate the effects of prednisolone on clinical symptoms, pain, hand grip strength, range of motion, as well as on functional ability and quality of life in patients who developed CRPS after traumatic upper extremity injury. STUDY DESIGN: Retrospective evaluation. METHODS: Forty-five patients who use

[Complex regional pain syndrome in children].

While complex regional pain syndrome (CRPS) is a well-established entity in adults, its occurrence in children was doubted for a long time. However, in the last few years several case reports and some comparative studies have described CRPS in children and adolescents. In contrast to adults most of the involved children are female, suffering from CRPS after an initial event that is typically a minor trauma. Furthermore, CRPS occurs more frequently in the lower extremity than in the upper extremity when compared to adults. Since neither radiological findings nor laboratory parameters are able t

Complex regional pain syndrome underdiagnosed.

Complex regional pain syndrome (CRPS) type 1 may be diagnosed by history and physical exam with no further testing. Several different diagnostic criteria have undergone validity testing: the 1993 IASP criteria, Bruehl's criteria, and Veldman's criteria; there is no compelling reason to recommend 1 set of criteria over the others. Some cases of CRPS type 1 may be preventable. Some cases of CRPS type 1 in post-stroke upper extremity hemiplegia (also known as shoulder-hand syndrome) may be prevented by early inpatient rehabilitation and avoidance of shoulder trauma to the affected arm. Some cases

Complex regional pain syndrome and chiropractic.

OBJECTIVE: Complex regional pain syndromes (CRPS) represent curious and difficult syndromes for both patient and clinician. CRPS presents as a triad of signs and symptoms, usually after a seemingly trivial injury to a peripheral joint or appendage. The clinical triad includes severe pain, vasomotor changes in and around the affected area, and trophic changes in the affected limb. Many of the acute symptoms are similar to those seen after many acute injuries, which makes an early diagnosis often times difficult. Current treatment protocols revolve around aggressive physical therapy plus pharmac

[Complex regional pain syndrome versus chronic regional pain syndrome (Hand-Finger Syndrome)].

Dystrophy is a main factor of CRPS. A large number of patients do not develop dystrophy but, instead, they suffer from pain with limitation in movement, possible paraesthesia and/or swelling. This is then a chronic regional pain syndrome or (shoulder-arm-) hand-finger syndrome. These patients should never be confronted with the diagnosis Morbus Sudeck or algodystrophy, which are today also well known among non-professionals, to avoid pushing them into a status of constant severe invalidity. Histories, clinical examination, as well as a good personal understanding of the patient are indispensab

Spatial distribution of pain in complex regional pain syndrome.

Pain often spreads away from the injured site in complex regional pain syndrome (CRPS), but what drives this process is unclear. To explore this, pain drawings were investigated in 136 patients in relation to the type and location of injury, pain intensity and quality, CRPS duration, subtypes, and hyperalgesia to thermal and mechanical stimuli. Areas of pain were quantified using the ImageJ polygon selection tool. The pain area in the affected limb increased in line with pain intensity and with indices of hyperalgesia in the affected limb and ipsilateral forehead ( P 's < 0.01). The pain area

Widespread pain in patients with complex regional pain syndrome.

OBJECTIVES: Our goal was to ascertain the prevalence of widespread pain in our cohort of patients with complex regional pain syndrome (CRPS). METHODS: We conducted a retrospective analysis of clinical letters and notes. We assessed data from consecutive patients diagnosed with CRPS according to the Budapest criteria, after a referral to one consultant at a tertiary Pain Medicine referral center. RESULTS: Between July 2007 and September 2012, 190 patients (149 females) received a diagnosis of CRPS according to the Budapest criteria, and an additional 26 patients received the diagnosis of CRPS

Focal nerve inflammation induces neuronal signs consistent with symptoms of early complex regional pain syndromes.

Early forms of complex regional pain syndromes (CRPS) are characterized by severe pain and autonomic dysfunction in a limb, both of which seem out of proportion to the inciting event. While often caused by obvious nerve injury, the syndromes also occur following relatively trivial trauma. Persistent inflammation has been implicated in the etiology of CRPS. We hypothesized that inflammation of a nerve proximal to the symptoms could lead to neural changes consistent with clinical CRPS. Using a rat model of neuritis, the activity of sensory and autonomic neurons was recorded distal to the inflame

Complex Regional Pain Syndrome: A Comprehensive Review.

Complex regional pain syndrome (CRPS) is a chronic pain condition often involving hyperalgesia and allodynia of the extremities. CRPS is divided into CRPS-I and CRPS-II. Type I occurs when there is no confirmed nerve injury. Type II is when there is known associated nerve injury. Female gender is a risk factor for developing CRPS. Other risk factors include fibromyalgia and rheumatoid arthritis. Unfortunately, the pathogenesis of CRPS is not yet clarified. Some studies have demonstrated different potential pathways. Neuropathic inflammation, specifically activation of peripheral nociceptors of

Complex regional pain syndrome: an evolving perspective.

BACKGROUND: Complex regional pain syndrome (CRPS) is a heterogenous and poorly understood condition that can be provoked by quite minor injuries. The symptoms and signs of CRPS persist, long after the patient has recovered from the inciting event. In some cases, there is a clear association with a peripheral nerve injury. The degree of disability produced by CRPS is often out of proportion to the scale of the original insult and the condition is associated with protracted recovery times and frequent litigation. METHODS: We have performed a PubMed literature search, referenced landmark papers

MicroRNA modulation in complex regional pain syndrome.

BACKGROUND: Aberrant expression of small noncoding RNAs called microRNAs (miRNAs) is a common feature of several human diseases. The objective of the study was to identify miRNA modulation in patients with complex regional pain syndrome (CRPS) a chronic pain condition resulting from dysfunction in the central and/or peripheral nervous systems. Due to a multitude of inciting pathologies, symptoms and treatment conditions, the CRPS patient population is very heterogeneous. Our goal was to identify differentially expressed miRNAs in blood and explore their utility in patient stratification. METH
