Microbiological aspects of extended-wear soft contact lenses.

Corneal ulcers are the most devastating complication associated with extended-wear soft contact lenses. The increasing prevalence of microbial keratitis associated with the wear of these lenses is a major concern for the optometrist. Studies of extended-wear and daily-wear contact lens patients strongly suggest that extended-wear patients face a greater risk of ulcerative keratitis than other cosmetic contact lens wearers. This discussion addresses the potential risks of extended-wear, with emphasis on ulcerative keratitis, the likely causes for these increased risks, and what the optometrist

Aphakic extended-wear contact lenses after penetrating keratoplasty.

Fifteen aphakic eyes in 15 selected patients who underwent penetrating keratoplasty were successfully fitted with extended-wear contact lenses three to 42 months postoperatively. After an average follow-up of 17 months, there was no graft edema, no microbial corneal ulcers, and no episode of graft rejection. Three eyes developed minor (1 to 2 mm) graft neovascularization; the neovascularization was stable and did not require discontinuation of the contact lenses. Fourteen of the 15 patients had postoperative visual acuities of 20/70 or better; the one patient whose visual acuity was worse than

Aphakic patient response to extended wear contact lenses.

65 aphakic patients (89 eyes) wore extended wear contact lenses for an average period of 22.8 months. 74% of the patients developed one or more readily detectable ocular changes during the course of the study. These complications, their frequency, cause and management are described and discussed. In spite of the large number of complications, 82% of the patients were still wearing lenses at the termination of the study period. None of the complications were serious enough to produce ocular tissue damage that resulted in any permanent loss of vision. Careful patient management and frequent foll

Complications reported with hydrogel extended wear contact lenses.

We conducted a survey to evaluate the incidence of various complications associated with extended wear (EW) contact lenses. Of the 137 surveys sent out, 44 responded, 15 with data. The analysis indicates: (1) EW contact lens patients receive nonuniform care, evident from the fact that the EW follow-up to EW dispensing ratio varied between 1.6 and 16.75; (2) the incidence of epithelial microcysts, striae, and giant papillary conjunctivitis (GPC) was found to be considerably lower than that reported previously; (3) the incidence of "red-eye" response and corneal ulcers was calculated to be withi

Silsoft extended wear contact lenses in pediatric aphakia.

Fifty pediatric aphakic patients, ranging in age from 7 days to 11 years, were fitted with the Silsoft extended wear contact lens. Compared to other extended wear lenses, the Silsoft lens has several advantages. These include easier handling, a less traumatic fitting procedure for the child, the use of fluorescein for evaluation of the lens-cornea relationship and a low loss rate. This lens appears to be safe, with no permanent adverse effects on the cornea.

Extended-wear soft contact lenses for aphakic correction.

Evaluation of an extended-wear perfilcon A soft contact lens (Permalens) in 299 eyes of 244 patients showed that 266 eyes (89%) were successfully fitted and wore the lenses without important complications (mean follow-up, 23 months; range, four to 48 months). Use of the lens in 33 eyes was discontinued because of transient lens-related problems. "Overcorrected" acuity of 6/12 (20/40) or better (with spectacles plus the contact lens) was achieved in all patients who were without vitreous, retinal, or optic nerve disease. Seventy-six percent of patients could wear the lens for longer than one-mo

Extended-wear contact lenses in 525 aphakic eyes.

Extended-wear contact lenses were fitted to 225 patients with monocular aphakia and 150 with bilateral aphakia. They were observed from three months to three years. Eighty-eight of the 195 eyes fitted with a Softcon lens became red and painful within 14 days during extended wear. The Hydrocurve lens was better tolerated in 55, but many patients had acute congestion, pain, and tearing. Deposits accumulated rapidly on the Sauflon-PW lens. Some 90% of all patients can be fitted successfully with the cellulose acetate butyrate lens or the silicone lens. The silicone coating of the silicone lens is

Corneal ulcers associated with daily-wear and extended-wear contact lenses.

Over a 21-month period, we treated corneal ulcers in 11 patients using daily-wear contact lenses and 29 patients using extended-wear contact lenses. Since more patients use daily-wear than extended-wear lenses, this suggests that patients using extended-wear lenses are at greater risk for the development of corneal ulcers. Positive bacterial cultures were obtained from the corneal scrapings of nine of 11 patients using daily-wear lenses and 20 of 29 patients using extended-wear lenses. In the daily-wear group, all 11 patients failed to exercise satisfactory care in using and disinfecting their

Extended-wear aphakic soft contact lenses after penetrating keratoplasty.

Seven patients undergoing aphakic keratoplasty or combined keratoplasty and cataract extraction were fitted with a single type of high-water-content, extended-wear, aphakic soft contact lens (Permalens). Six patients were fitted early after keratoplasty, ie, before the removal of the sutures and while they were still receiving corticosteroid drops (1% prednisolone acetate). The patients were followed up at close intervals with high-magnification serial corneal photography. Complications of extended wear included superficial neovascularization, punctate epithelial keratitis, and variable visual

Extended-wear aphakic soft contact lenses and corneal ulcers.

A review of 100 aphakic extended-wear soft contact lenses is presented for the period July 1980 to August 1981. Four previously successfully fitted patients with either American Optical Company's Sofcon or Cooper Laboratories' Permalens for extended wear developed corneal ulcers either directly under the lenses or shortly after removal. Three of the female patients were well controlled diabetics without retinopathy, one of whom sustained severe visual loss and neovascular glaucoma after a pseudomonas ulcer. Another patient, who had developed a Seratia marcescens ulcer 3 months later, developed

Extended wear silicone contact lenses in children with cataracts.

Thirty-twop patients, ranging in age from three weeks to 22 years, were fitted with an extended wear 100% silicone lens. The longest period of time a lens remained in the eye without being removed was 120 days. Most lenses were worn an average of six weeks before cleaning was required. No cases of infection or corneal edema were observed that required discontinuing use of the lens.

Extended wear of CAB contact lenses in aphakic patients.

In order to assess the applicability and safety of cellulose acetate buterate (CAB) contact lenses for extended wear in aphakia, 139 lenses were fitted in 102 aphakic patients. These lenses were removed periodically for cleaning only. There has been an average follow-up of 24.75 months on 99 patients (135 lenses), with a success rate of 80%. No serious complications or corneal vascularization have occurred. The major problems encountered were due to lack of tint, increased tendency to accumulate mucus, and the need for periodic insertion and removal.

Complications associated with extended wear of soft contact lenses.

The major complications with extended wear lenses are due to hypoxia and lens deposits. Certain patients tend to deposit proteins and lipids on the lens surface with daily wear and at a greater frequency during prolonged wear. A build-up of these deposits can be associated with decreased vision, conjunctival irritation, and giant papillary conjunctivitis. Frequent lens cleaning and the frequent use of concomitant topical artificial tears will hopefully reduce the incidence of lens deposits and their complications. Superficial circumlimbal vascularization occurs in a large percentage of patient

Extended wear soft contact lenses induce corneal epithelial changes.

The purpose of this study was to determine the corneal epithelial alterations induced by various types of contact lenses. By employing the specular microscope, the corneal epithelia of 60 patients who had worn contact lenses for more than 1 year were re-examined along with 15 myopic controls. The morphological changes in aphakic patients who changed from extended wear soft to extended wear rigid gas permeable lenses were also studied. The mean cell area of the corneal epithelium was 621.5 (SD 92.5) microns 2 for daily wear rigid gas permeable lenses (n = 15), 645.8 (98.1) microns 2 for daily w

Corneal ulcers associated with extended-wear soft contact lenses.

We treated nine myopic and nine aphakic eyes in patients who developed corneal ulcers while wearing extended-wear soft contact lenses. Bacteria were recovered from the corneal ulcers of 13 of 18 patients with Pseudomonas aeruginosa being the pathogen most frequently isolated. Chemical disinfection was used by seven of the nine patients with myopia and eight of the nine with aphakia. Two of nine patients in the myopic group had visual loss to 20/200 and hand movements; five of nine patients in the aphakic group had visual loss to 20/50 or worse. Invasion of the corneal stroma by bacteria may oc
