Feasibility of Continuous Actigraphy in Patients in a Medical Intensive Care Unit.

BACKGROUND: Poor sleep and immobility are common in patients in the medical intensive care unit (MICU) and are associated with adverse outcomes. Interventions to promote sleep and mobilization in the MICU are gaining popularity, but feasible instruments to measure their effectiveness are lacking. Actigraphy may be useful for large-scale, continuous measurement of sleep and activity, but its feasibility in MICU patients has not been rigorously evaluated. OBJECTIVE: To evaluate the feasibility of continuous actigraphy measurement in consecutive MICU patients. METHODS: Wrist and ankle actigraph

Continuing care for patients with alcohol use disorders: a systematic review.

BACKGROUND: A chronic care perspective should be adopted in the treatment of patients with alcohol use disorders (AUDs). Initial treatment in a more intense psychiatric care setting should be followed by continuing care. This systematic review aims to identify effective continuing care interventions for patients with AUDs. METHODS: Electronic databases were searched up to February 2013 (MEDLINE, EMBASE, CENTRAL, CINAHL and PsycINFO) to identify RCTs studying continuing care interventions for patients with AUDs. Study selection and quality appraisal was done independently by two reviewers. Dri

[Care continuity for patients with myopathy during transition of childhood to adulthood].

Recently, patients with myopathies who had not previously been able to survive after adolescence required long-term follow-up and treatment. We here discussed medical practice after adolescence in child neurology for these patients with myopathies, such as muscular dystrophies, Pompe disease and juvenile dermatomyositis. Major issues encountered in such practice included inpatient facilities and community medical networks, especially for patients who receive long-term home mechanical ventilation, management for cardiomyopathy and/or gastrogavage. Continuity of care and treatment intensity at the end of life in Swiss cancer patients.

BACKGROUND: Continuity of care (COC) was shown to be associated with fewer hospitalizations. We aimed to evaluate whether COC was associated with intensive intervention(s) at the end of life (IEOL), a preference-sensitive outcome, in cancer patients. METHODS: The study is based on claims data of patients with incident use of anti-neoplastics in Switzerland. COC Index, Usual Provider Continuity score, Sequential Continuity index and Modified Modified Continuity Index were calculated based on consultations with the usual ambulatory care physician. Treatment intensity was evaluated in the last 6

The association between continuity of care and surgery in lumbar disc herniation patients.

Continuity of care is a core dimension of high-quality care in the management of disease. The purpose of this study was to investigate the association between continuity of care and lumbar surgery in patients with moderate disc herniation. The Korean National Sample Cohort was used. The target population consisted of patients who have had disc herniation more than 6 months and didn't get surgery and red flag signs within 6 months from onset. The population was enrolled from 2004 to 2013. The Bice-Boxerman Continuity of Care was used in measuring continuity of care. The marginal structural mode

Hyperkalemia in Real-World Patients Under Continuous Medical Care in Japan.

INTRODUCTION: An abnormal serum potassium (S-K) level is an important electrolyte disturbance. However, its relation to clinical outcomes in real-world patients, particularly hyperkalemia burden, is not extensively studied. METHODS: An observational retrospective cohort study using a Japanese hospital claims database was done (April 2008-September 2017; N = 1,022,087). Associations between index S-K level and 3-year survival were modeled using cubic spline regression. Cox regression model was applied to estimate the time to death according to different S-K levels. Prevalence, patient characte

Critical care patients' experience of the helmet continuous positive airway pressure.

BACKGROUND: Continuous positive airway pressure (CPAP) is a common treatment modality for acute respiratory failure (ARF) in critical care. Historically, a tight-fitting mask is used to provide respiratory support. This however is not without risks to the patient. The helmet CPAP is a new product that provides the same treatment with a different method of delivery. There is minimal evidence to date explaining the patient's experience of the new helmet modality. AIMS AND OBJECTIVES: The aim of this research study is to explore critical care patient's experience of helmet CPAP. DESIGN: A quali

[Care continuity for patients with depression during transition from childhood to adulthood].

The clinical features, course and outcome of mood disorders in childhood and adolescence are discussed. The most specific symptoms of major depressive disorder in childhood and adolescence are irritability, social withdrawal, and comorbidity. Depression is a chronic and recurrent condition. The course of child onset depression resembles those with conduct disorder much more than those with mood disorders, without an increased risk for recurrence of depression into adult life. On the other hand, adolescent onset depression has a high risk of recurrent mood disorder into adulthood.

Stability and compatibility of ceftazidime administered by continuous infusion to intensive care patients.

The stability and compatibility of ceftazidime have been examined in the context of its potential use in concentrated solutions for continuous infusion in patients suffering from severe nosocomial pneumonia and receiving other intravenous medications by the same route. Ceftazidime stability in 4 to 12% solutions was found satisfactory (<10% degradation) for 24 h if kept at a temperature of 25 degrees C (77 degrees F) maximum. Studies mimicking the simultaneous administration of ceftazidime and other drugs as done in clinics showed physical incompatibilities with vancomycin, nicardipine, midazo

Continuous and Non-Invasive Lactate Monitoring Techniques in Critical Care Patients.

Lactate, once merely regarded as an indicator of tissue hypoxia and muscular fatigue, has now gained prominence as a pivotal biomarker across various medical disciplines. Recent research has unveiled its critical role as a high-value prognostic marker in critical care medicine. The current practice of lactate detection involves periodic blood sampling. This approach is invasive and confined to measurements at six-hour intervals, leading to resource expenditure, time consumption, and patient discomfort. This review addresses non-invasive sensors that enable continuous monitoring of lactate in c

Continuous Lidocaine Infusion as Adjunctive Analgesia in Intensive Care Unit Patients.

Despite a paucity of data, the role of intravenous lidocaine (IVLI) as adjunctive analgesia in the intensive care unit (ICU) seems promising due to a low potential to contribute to respiratory depression. A retrospective chart review was conducted to evaluate the safety and effectiveness of IVLI for the treatment of pain in ICU patients with varying degrees of organ dysfunction from March 2014 to March 2016. The primary outcomes included the time to a ≥20% reduction in pain scores after the initiation of IVLI and the difference in opioid requirements as well as pain scores prior to and during

Pharmacists' role in the continued care of patients under opiate substitution treatment.

CONTEXT: In France, over 170 000 persons benefit from opiate substitution treatment. The treatment delivery is most often assured by community pharmacies spread out throughout the territory. The aim of this study is to better know the continued care of individuals treated by methadone or high-dose buprenorphine in community pharmacies in Lille (North of France). METHOD: This was a cross-sectional descriptive study with hand-delivered questionnaires. All pharmacies (n=65) in Lille were included. RESULTS: Fifty-eight of pharmacies (89%) responded to the survey and 55 of them (95%) followed pat

Continuity of care for short-stay neurosurgery patients: a quality improvement initiative.

Decreases in the length of hospital stay for patients undergoing spinal surgery prompted this evaluation of the post-discharge needs of patients and the strategies that patients and their families employ to meet these needs. The nature and extent of post-discharge problems experienced by newly discharged patients was required as a baseline for the evaluation and improvement of discharge planning. Forty patients were interviewed following discharge, 20 patients within the first week of discharge, and 20 different patients between three and four weeks after discharge. Most patients reported that

Clinical trial of continuous infusion of alphaxalone/alphadolone in intensive care patients.

The safety and efficacy of prolonged infusion of alphaxalone/alphadolone (Alfathesin: Glaxo) was assessed in 20 critically ill patients needing sedation during intermittent positive pressure ventilation in a general intensive care unit. The mean dose of Alfathesin infused was 1542 ml (range: 225 to 4820 ml) over 6.9 days (3 to 16 days) at rates between 5 and 15 ml/hour. Significant increases in plasma urea, creatinine, bilirubin, alkaline phosphatase and white cell count occurred during the infusion, but these were expected from each patient's clinical course. Impact of In-Hospital Osteoporosis Treatment on Post-discharge Medication Continuity in Osteoporotic Fracture Patients: Implications for Care Continuity.

BACKGROUND: Despite the well-established benefits of osteoporosis medication in reducing the risk of secondary fractures and mortality, post-fracture treatment rates remain suboptimal. The hospitalization period represents a critical opportunity to initiate therapy, yet the impact of in-hospital osteoporosis treatment on long-term adherence has not been fully evaluated in the Korean population. METHODS: This retrospective cohort study used data from the National Health Insurance Service sample cohort in South Korea. Patients aged ≥ 50 years who were hospitalized for osteoporotic fractures, in
