Continuous renal replacement therapy: individualization of the prescription.

PURPOSE OF REVIEW: Continuous renal replacement therapy (CRRT) is now the mainstay of renal organ support in the critically ill. As our understanding of CRRT delivery and its impact on patient outcomes improves there is a focus on researching the potential benefits of tailored, patient-specific treatments to meet dynamic needs. RECENT FINDINGS: The most up-to-date studies investigating aspects of CRRT prescription that can be individualized: CRRT dose, timing, fluid management, membrane selection, anticoagulation and vascular access are reviewed. The use of different doses of CRRT lack conven

Clarifications on Continuous Renal Replacement Therapy and Hemodynamics.

OBJECTIVE: Continuous renal replacement therapy (CRRT) is a continuous process of bedside blood purification which is widely used in the treatment of acute kidney injury (AKI) and for fluid management. However, since AKI and fluid overload are often found to be associated with hemodynamic abnormalities, determining the relationship between CRRT and hemodynamics remains a challenge in the treatment of critically ill patients. The aim of this review was to summarize key points in the relationship between CRRT and hemodynamics and to understand and monitor renal hemodynamics in critically ill pat

Continuous renal replacement therapy in dogs and cats.

Continuous renal replacement therapy (CRRT) is a relatively new extracorporeal blood purification modality that is rapidly gaining acceptance for the treatment of acute kidney injury in cats and dogs. The author has used CRRT for the treatment of leptospirosis, tumor lysis syndrome, heatstroke, pre- and postsurgical support of ureteral obstructions, as well as aminoglycoside and melamine toxicities. As the name implies, CRRT is a slow gradual process, and once treatment begins, patients continue therapy until their renal function returns, they are transitioned to intermittent hemodialysis (IHD

Continuous renal replacement therapy in acute liver failure.

Acute liver failure (ALF) is a devastating condition that primarily affects young adults. This often-lethal condition involves a rapid loss of hepatic function, that then leads to multiple organ failure. The accumulation of numerous toxins, especially ammonia, causes cerebral oedema and intracranial hypertension. Continuous renal replacement therapy (CRRT) is increasingly recognized as having a key role in ammonia removal in ALF and current evidence suggesting that timing of initiation, dose, and duration of therapy may influence survival. In addition to this important role of toxin clearance,

The impact of continuous renal replacement therapy on drug therapy.

Critically ill patients with multisystem organ failure often require daily administration of large volumes of fluid to provide electrolyte and nutrition support, medications, and blood products. This often results in fluid overload, which has historically been managed with intermittent hemodialysis (IHD). Unfortunately, IHD entails a high rate of fluid and solute removal that often exacerbates hemodynamic instability. Accordingly, continuous renal replacement therapy (CRRT), involving slow and continuous removal of water and solutes from the plasma, is currently preferred for managing these pa

[Anticoagulation in continuous renal replacement therapies (CRRT)].

Continuous renal replacement therapies (CRRT) are commonly used in the majority of critically ill patients who need dialysis. Treatment success depends on an efficient anticoagulation protocol devised to maintain the dialysis circuit unclotted, with minimal complications such as bleeding due to excessive anticoagulation. Several features can contribute to dialysis circuit thrombosis, such as the speed of pump blood flow, dialysis catheter, type of dialyzer membrane and also, the type of technique prescribed. Unfractioned heparin (UFH) is the anticoagulant most used in CRRT. Recently, low-molec

Slow continuous renal replacement therapies: an update.

Continuous renal replacement therapies (CRRT) are now being used by nephrologists, intensivists, and anesthesiologists. The various CRRT modalities differ in the kind of vascular access, the application of diffusive or convective clearances (or a combination of both), and in the location where the replacement fluid enters the circuit. CRRTs have certainly made the management of critically ill patients with acute renal failure (ARF) combined with cardiovascular instability, severe fluid overload, hypercatabolism, cerebral edema, adult respiratory distress syndrome, lactic acidosis, sepsis or ot

Slow continuous ultrafiltration and prolonged intermittent renal replacement therapy: Tailoring renal replacement therapy in intensive care unit.

Acute kidney injury complicates the clinical course of nearly half of all critically ill patients admitted to intensive care, conferring a three- to five-fold elevation in mortality risk. The management of these patients has evolved considerably beyond standardized protocols toward individualized therapeutic strategies. Within this paradigm shift, two modalities warrant particular attention: Slow continuous ultrafiltration (SCUF) and prolonged intermittent renal replacement therapy (PIRRT). SCUF operates through isolated fluid extraction at rates between 100 mL and 500 mL per hour, targeting p

Continuous renal replacement therapy for non-renal indications: experience in children.

BACKGROUND: The role of continuous renal replacement therapy in patients with acute renal failure is well recognized. CRRT has also become an important modality of treatment in various acute situations without renal failure. OBJECTIVES: To describe our experience with CRRT in acutely ill infants and children without renal failure. METHODS: We analyzed all infants and children who underwent CRRT during the years 1998-2000 in the pediatric intensive care unit and we focus our report on those who were treated for non-renal indications. RESULTS: Fourteen children underwent 16 sessions of CRRT.

Continuous renal replacement therapy: continuous blood purification in the intensive care unit.

Severe acute renal failure (SARF) occurs when renal dysfunction is such that haemodialysis or haemofiltration becomes necessary to maintain homeostasis. SARF is increasingly seen in association with multiorgan failure and has become a predominantly Intensive Care Unit disorder. Because of this change in epidemiology, the treatment of SARF has evolved from being exclusively nephrologist and intermittent haemodialysis-based to being mostly intensivist and continuous haemofiltration-based, particularly in European countries with a strong ICU tradition and in Australia. Continuous renal replacemen

[Development and Application of Continuous Renal Replacement Therapy Devices].

Continuous renal replacement therapy (CRRT) is one of the most commonly used treatments for patients with acute and chronic renal failure as well as end-stage chronic kidney disease, and is an important modality of renal replacement therapy. CRRT originated in the 1990s, initially focusing on a single dialysis mode. With the advancement of technology, treatment modes have gradually diversified, treatment effectiveness and safety have been optimized, and treatment efficiency has also been significantly improved. This article surveys the development history of CRRT devices and their clinical app

Carnitine deficiency in children receiving continuous renal replacement therapy.

Carnitine deficiency is known to occur in chronic hemodialysis; however, the effect of continuous renal replacement therapy (CRRT) on carnitine homeostasis has not been studied. We hypothesized that children receiving CRRT are at risk for deficiency because of continuous removal, absent intake, decreased production, and comorbidities related to critical illness. Records of patients with acute kidney injury receiving CRRT at Children's National Health System between 2011 and 2014 were reviewed for total carnitine (TC), free carnitine (FC), feeding modality, severity of illness, and survival out

Continuous renal replacement therapy in patients with HIV/AIDS.

BACKGROUND: Acute kidney injury (AKI) is a common complication among human immunodeficiency virus (HIV)-infected patients resulting in increased morbidity and mortality. Continuous renal replacement therapy (CRRT) is a useful method and instrument in critically ill patients with fluid overload and metabolic disarray, especially in those who are unable to tolerate the intermittent hemodialysis. However, the epidemiology, influence factors of CRRT and mortality in patients with HIV/AIDS are still unclear in China. This study aims to study the HIV-infected patients admitted in Intensive Care Unit

Advances in continuous renal replacement therapy: citrate anticoagulation update.

Because of the potential side effects of heparin, methods of regional citrate anticoagulation (RCA) for continuous renal replacement therapy (CRRT) have been gaining wider acceptance with the development of simplified and safer protocols. Advantages of RCA include the avoidance of systemic anticoagulation and heparin-induced thrombocytopenia. The disadvantage is that citrate can add complexity and labor intensity to CRRT. Frequent monitoring of electrolytes, ionized calcium, and acid-base status is required, due to the potential for hypernatremia, metabolic alkalosis, and systemic ionized hypo
