Recent advancements and future applications of intrauterine drug delivery systems.

INTRODUCTION: Intrauterine devices (IUDs) are drug-device combination products that are placed inside the uterus above the endometrium. IUDs contain a therapeutic agent used for contraception and have been shown to have other therapeutic benefits. Over the past century, intrauterine drug delivery systems (IUDDS) have revolutionized women's health, and they hold immense growth potential in the future for new developments in the therapeutic areas beyond contraception. AREAS COVERED: The aim of this review is to offer a comprehensive description of the history, development, and current state of

[Male contraception-scientific foundations and contemporary progress].

Despite decades of assiduous inquiry and burgeoning societal demand, contraceptive options for men remain circumscribed. Women have benefitted from highly efficacious hormonal modalities, but the burden of contraceptive responsibility continues to rest almost exclusively upon them. In light of the potential sequelae of female hormonal agents and the inviolable right to bodily autonomy, the quest for safe, reversible male methods is assuming urgency.Hormonal contraception for men relies on suppression of spermatogenesis through exogenous testosterone combined with progestins. Multicenter trials

Nicotine and estrogen synergistically exacerbate cerebral ischemic injury.

The greater incidence of myocardial infarction, cardiac arrest, and ischemic stroke among women who smoke and use oral contraception (OC) compared to women who do not smoke and who do or do not use OC may be due in part to how nicotine influences endocrine function in women. For example, we recently demonstrated that chronic exposure to nicotine, the addictive agent in tobacco smoke responsible for the elevated risk of cardiac arrest, abolishes the endogenous or exogenous 17β-estradiol-conferred protection of the hippocampus against global cerebral ischemia (a potential outcome of cardiac arre

Oral contraceptives contraindicated for nursing mother.

F.S. Schroeder (Eureka, Montana) asked: Is it wise or advisable for a nursing mother to take oral contraceptives? Answer (by W.N. Spellacy): It is neither wise nor advisable to prescribe oral contraceptives to nursing mothers. This is because the steroid components, as is the case with most drugs, are excreted into the milk. Their effects on the neonate are variable and dose-dependent. For example, an androgen-type progestagen may masculinize a female infant, and steroid metabolites may contribute to neonatal jaundice. Accordingly, most pharmaceutical companies include a Food and Drug Administ

"Antibiotics Can Work as a Contraceptive:" Contraceptive Knowledge and Use Among University Students in Calabar, Nigeria.

Introduction: Many sexually active youths who wish to delay pregnancy are not using any form of modern contraceptives. In sub-Saharan Africa, less than 1 in 5 sexually active youth do not use contraceptives. In Nigeria, 48.4% of all sexually active unmarried women have an unmet need for contraception. Although the literature is replete with information on structural barriers to modern contraceptives, there is limited scholarship on contextual factors that may inhibit modern contraceptive use among Nigerian youth. This study uses a qualitative research approach to assess knowledge and use of na

Influence of progestin-only hormonal use on hepatocellular adenomas: A retrospective cohort study.

OBJECTIVES: Exogenous estrogen is associated with growth of hepatocellular adenomas (HCAs), although the influence of progestin-only agents is unknown. We therefore evaluated the association of progestin-only agents on HCA progression compared to no hormone exposure and compared to estrogen exposure in female patients. STUDY DESIGN: In this single-center, retrospective cohort study of reproductive-aged female patients (ages 16-45) with diagnosed HCAs between 2003 and 2021, we evaluated radiographic HCA growth during discrete periods of well-defined exogenous hormone exposures. RESULTS: A tot

Pregnancy Screening and Contraceptive Counseling in Young Women with Hodgkin Lymphoma: A Single Center Retrospective Review.

Purpose: Chemotherapeutic agents used to treat Hodgkin lymphoma are teratogenic. Pregnancy screening before the start of chemotherapy is supported by clinical guidelines. There are limited data on pregnancy screening before the start of Hodgkin therapy but previous studies suggest that it is not consistently completed. The objective of this study is to evaluate the completion of pregnancy screening before the start of chemotherapy in females with Hodgkin lymphoma. Methods: A retrospective chart review was performed for all female patients, regardless of age, with newly diagnosed Hodgkin lympho

Hormonal therapies for menstrual migraine.

Menstrual-related migraine (MRM) affects the majority of female migraineurs, with menstrual-associated attacks reported to be more disabling, longer lasting, and less responsive to traditional treatments than nonmenstrual attacks. Emerging evidence suggests that minimizing or eliminating monthly declines in estrogen concentration may be effective in preventing MRM. This article gives a practical overview of current hormonal options, both contraceptive and noncontraceptive. Our intent is to help the reader better understand the differences in currently available formulations and how some of the

Pharmacology update in adolescents: contraception and human papillomavirus vaccination.

The ideal contraceptive agent remains elusive for the adolescent population. Contraceptive failure is often caused by inappropriate or inconsistent use, and discontinuation within the first year is not uncommon. Various methods have been explored within the adolescent population to increase efficacy rates, minimize side effects, and prevent unwanted pregnancies. The use of intrauterine devices and continuous use of combined oral contraceptives may lead to greater efficacy because of the ease of use and reduction in menstrual symptoms. Recent literature supports the continued use of medroxyprog

Oral contraceptive and hormone replacement therapy in women with cerebral aneurysms.

BACKGROUND: It is well known that cerebral aneurysms occur more frequently in women, with numerous studies suggesting a role for hormones in aneurysm pathogenesis. Estrogen promotes normal physiologic vascular endothelial function but also fluctuates during the menstrual cycle and drops significantly at menopause. METHODS: A retrospective, case control study was conducted to determine if exogenous estrogen use, which stabilizes estradiol levels, had any association with the presence of cerebral aneurysms. 60 women with intradural cerebral aneurysms were interviewed about their basic medical a

Nutritional effects of oral contraceptive use: a review.

Oral contraceptives agents (OCA) have been in use for more than two decades, and at the present time, 150 to 200 million women are using the preparations. Apart from their gynecologic influence, the hormones have been shown to affect a number of metabolic and nutritional processes, some advantageously and others disadvantageously. Concern over the nutritional status of females consuming OCA prompted this review. Eight vitamins and three minerals were investigated. Contraceptive steroid ingestion was shown to depress the physiologic levels of six nutrients (riboflavin, pyridoxine, folacin, vita

Stroke in users of low-dose oral contraceptives.

BACKGROUND: Previous studies have linked the use of oral contraceptive agents to an increased risk of stroke, but those studies have been limited to oral contraceptives containing more estrogen than is now generally used. METHODS: In a population-based, case-control study, we identified fatal and nonfatal strokes in female members of the California Kaiser Permanente Medical Care Program and who were 15 through 44 years of age. Matched controls were randomly selected from female members who had not had strokes. Information about the use of oral contraceptives (essentially limited to low-estrog

Effect of concurrently coadministered drugs on the pharmacokinetic/pharmacodynamic profile of centchroman, a nonsteroidal oral contraceptive, in rats.

INTRODUCTION: Centchroman (international nonproprietary name: ormeloxifene) is a nonsteroidal selective estrogen receptor modulator, oral contraceptive, anticancer and antiosteoporotic agent that is intended for long-term use by women. In view of the vast clinical applications and interactions of steroidal oral contraceptives with commonly used therapeutic agents, the interaction potential of certain concomitantly administered therapeutic agents was investigated in terms of postcoital contraceptive efficacy (pharmacological) and the pharmacokinetic profile of centchroman in female Sprague-Dawl

Bipolar disorder in women.

This article summarizes research pertinent to the clinical care of women with bipolar disorder. With bipolar disorder, female gender correlates with more depressive symptoms and different comorbidities. There is a high risk of symptom recurrence postpartum and possibly during perimenopause. Women with bipolar disorder have increased risk of sexually transmitted diseases, unplanned pregnancies, excessive weight gain, metabolic syndrome, and cardiovascular disease. Mood stabilizing medications, specific psychotherapies, and lifestyle changes can stabilize mood and improve functioning. Pharmacolo

Advances in male contraception.

Despite significant advances in contraceptive options for women over the last 50 yr, world population continues to grow rapidly. Scientists and activists alike point to the devastating environmental impacts that population pressures have caused, including global warming from the developed world and hunger and disease in less developed areas. Moreover, almost half of all pregnancies are still unwanted or unplanned. Clearly, there is a need for expanded, reversible, contraceptive options. Multicultural surveys demonstrate the willingness of men to participate in contraception and their female pa

Precipitous Delivery Masquerading as Biliary Colic in the Setting of Depo-Provera® Failure.

Precipitous delivery in the emergency department is a high-acuity, low-occurrence event that requires rapid recognition and interdepartment cooperation to prevent fetal and maternal morbidity and mortality. Prompt recognition of the peripartum state can be delayed by reported usage of long-acting contraception and concurrent distracting complaints. In this case report, a young female presented to the emergency department with epigastric abdominal pain in the setting of recent workup for biliary colic and multiple doses of long-acting, depot contraceptive agents. Intra-uterine Candida infection: a report of four infected fetusses from two mothers.

Although Candida albicans is a frequent inhabitant of the female genital tract, chorioamnionitis is rarely caused by this fungal organism. In this report we present two cases with manifest Candida chorioamnionitis. The first case is a twin pregnancy with premature delivery and survival of both twins. The second case is a pregnancy with intra-uterine contraceptive device in situ ending in a midtrimester abortion, followed by the next pregnancy also ending in an abortion in the second trimester. Possible triggers responsible for the increased invasiveness of otherwise benign Candida vaginitis ar

The role of cilostazol, a phosphodiesterase 3 inhibitor, on oocyte maturation and subsequent pregnancy in mice.

It is important to identify effective contraceptive drugs that cause minimal disruption to physiological processes. Phosphodiesterase 3 (PDE3) inhibitors suppress meiosis in oocytes by decreasing the level of cAMP and blocking the extrusion of the first polar body. In this study, we tested the PDE3 inhibitor, cilostazol, as a potential contraceptive agent. The effects of cilostazol treatment in vitro and in vivo on the suppression of oocyte maturation in a mouse model were investigated. The results indicated that treatment with increasing concentrations of cilostazol led to a dose-dependent ar

[Olanzapine and pregnancy. 2 case reports].

We report on two schizophrenic females who both had unexpected first pregnancies in their mid-thirties while being treated with olanzapine. Both patients had histories of psychosis of several years with some relapses and were treated previously with traditional antipsychotics. Since they wished to have children, no contraception had been performed past the age of 30. Both pregnancies occurred within a few months after the drug treatment was switched from traditional agents to olanzapine. Olanzapine was continued throughout the pregnancies, which were without complication. It is well known that
