[Near heterophoria with a description of the heterometer].

The examination of near heterophoria with a heterometer using a large series of test plates presented "free-sighted" (i.e., not hindered by optical or mechanical means) enables a more comprehensive analysis to be made than has so far been achieved with other instruments. The principal components of the instruments are an angle mirror and various test panels. The selection of test panels currently available should be developed further. However, comparison with other instruments for examining near heterophoria justifies the conclusion that heterophoria or nearsightedness due to errors of refract

[Accommodation-convergence coupling using the stereomicroscope].

The term "instrument myopia" means an accommodative effort caused by the view into the microscope. This accommodation may be connected with the convergence position of the eye. Heterophoria should be developed if accommodation-induced convergence of the eyes does not harmonize with convergence of the ocular axes of a stereomicroscope. In 16 probands, an accommodation of 1.1 +/- 0.72 dpt and esophoria of +11.1 +/- 8.5 pdpt were measured at parallel ocular axes. The accommodation increased by 0.12 dpt/degree convergence with increasing convergence angle of the ocular axes. When the visual field

The etiology, diagnostics, and treatment of the spasm of the near reflex - a narrative review.

Physiological adaptation of the eye to the visual perception of near objects consists of the "near triad": convergence, accommodation, and pupil miosis. Normally, these tend to revert when one stops fixating on a near object. Spasm of the near reflex (SNR) is a pathological phenomenon, which manifests itself by the persistence of the above-mentioned adjustments, which prevents the eye from returning to its relaxed state. In this narrative review, we aim to summarize the etiology, diagnostics, treatment, and prevention of SNR. The literature review was performed by searching online databases. T

Diurnal Variation in Accommodation, Binocular Vergence, and Pupil Size.

SIGNIFICANCE: Our results show significant diurnal variations in accommodative function and the magnitude of the phoria. Therefore, when comparing visual measures in clinical or laboratory settings, performing the visual examination at the same time of day (±1 hour) is encouraged. PURPOSE: The aim of this study was to evaluate the accommodation, binocular vergence, and pupil behavior on three different times during the day. METHODS: Twenty collegiate students (22.8 ± 2.1 years) participated in this study. Participants visited the laboratory on three different days at 2-hourly intervals (morn

An investigation of prism adaptation latency.

PURPOSE: The latency of adaptation of the human vergence system to a change in convergence or divergence forced upon it by a prism was investigated in this pilot study. METHODS: Adaptation was stimulated by a 5-s period of binocular vision through a prism of 0 delta, -8 delta, or 8 delta. Immediately thereafter and for a further period of 45 s, lateral heterophoria was monitored subjectively by an automated version of Duane's screen and parallax test. Adaptation was calculated from the phoria 40 s after the end of binocular vision. Tests were performed at 0.4 m. In the first test session, the

Characteristics of binocular vision and oculomotor function among sports-concussed athletes.

PURPOSE: To compare the binocular vision and oculomotor function between sports-concussed athletes and aged-matched controls. METHODS: Thirty mild concussed athletes were recruited and compared with aged-matched controls. All the participants underwent a comprehensive ocular assessment followed by an oculomotor assessment which included tests for accommodation, vergence, eye movements, and reading parameters. RESULTS: Three categories of oculomotor-based deficits were found: convergence insufficiency (40%), accommodative insufficiency (25%), and oculomotor-based reading dysfunctions (20%). A

IMI Accommodation and Binocular Vision in Myopia Development and Progression.

The role of accommodation in myopia development and progression has been debated for decades. More recently, the understanding of the mechanisms involved in accommodation and the consequent alterations in ocular parameters has expanded. This International Myopia Institute white paper reviews the variations in ocular parameters that occur with accommodation and the mechanisms involved in accommodation and myopia development and progression. Convergence is synergistically linked with accommodation and the impact of this on myopia has also been critiqued. Specific topics reviewed included accommo

Convergence insufficiency: Review of clinical diagnostic signs.

Convergence insufficiency (CI) is a common binocular vision (BV) disorder characterized by difficulty in maintaining motor fusion at near, which affects approximately 7.5 percent of the population. Diagnostic criteria for the disorder are inconsistent, ranging from one to many clinical signs. Methodology for clinical tests is inconsistent in measurement technique, visual targets, required repetitions, and normative values. This manuscript demonstrates the inconsistencies amongst published studies, and highlights the importance of consistent clinical diagnostic signs, measurement techniques, vi

Lateral short-term prism adaptation in clinical evaluation.

Data were obtained from a normal clinical population regarding heterophoria, associated phoria and short-term prism adaptation in an attempt to differentiate between those subjects with symptoms and those without. Correlation between heterophoria, associated phoria, prism adaptation and symptoms was low. A short prism-adaptation test was found to be of little value in diagnosing decompensated heterophoria in the general population. Subjects diagnosed as having decompensated heterophoria did not have an abnormal short-term prism-adaptation system. The clinical implications of these findings are

Convergence insufficiency and vision therapy.

There is no standard meaning of the term "vision therapy", and for this reason it is often a controversial topic between some members of the ophthalmic and optometric community. Most pediatric ophthalmologists avoid using the term because it is nonspecific. Convergence Insufficiency (CI) is a binocular visual problem that causes problems and symptoms with near fixation. There is consensus among eye care professionals that convergence therapy is effective in treating CI. Convergence therapy is not effective in treating learning disabilities, but can sometimes relieve symptoms that might be a ba

Tonic accommodation, tonic vergence and surround propinquity.

Previous studies have typically assessed tonic accommodation (TA) and tonic vergence (TV) by placing subjects in total darkness. However, a recent investigation demonstrated that even under such apparently stimulus-free conditions, subjects' awareness of the proximity of the adjacent surroundings and overall room topography (i.e. surround propinquity) may still influence the accommodative response. Accordingly, the aim of the present study was to measure TA and TV while subjects (n = 34) viewed an illuminated distant target (viewing distance = 6 m) to minimize this propinquity effect. TA was m

The source of disparity vergence innervation determines prism adaptation.

A physiological model of prism adaptation argues that adaptation magnitude and rate are functions of the magnitude of reflex vergence innervation. It has also been shown that there is reduced prism adaptation magnitude for a given disparity stimulus where only peripheral sensory fusion is present (the 'eccentricity effect'). This study attempted to determine whether the eccentricity effect is attributable to reduced reflex vergence innervation in peripheral fusion. Experiments were run in a Maxwellian view haploscope. Convergence and divergence adaptation to 6 delta disparity stimuli were quan

Effects of prolonged forced vergence upon the adaptation system.

In previous studies the ability of the oculomotor system to adapt to prism-induced heterophoria has been shown. The 'phoria that occurs immediately after a prism has been placed before one eye gradually reduces and returns to its original baseline value as the subject is allowed binocular experience while wearing the prism. Little work has been carried out to determine when the adaptation is complete. In this study we report on the results of two experiments designed to determine when adaptation is complete. In the first, the rate of decay of adaptation, i.e. loss of slow fusional vergence, is

Temporal characteristics of proximally-induced accommodation.

This study sought to determine whether proximally-induced accommodation (PIA) is capable of sustaining an accommodative response over time. Accordingly, the steady-state PIA response was measured during the course of a continuous 5 min stimulus period for targets located at distances of 0.2 or 6 m. The vergence and accommodation loops were opened by subjects (n = 8) monocularly viewing the targets through a 0.5 mm pinhole, while accommodation was assessed using an objective, open-field, infrared optometer. The mean PIA response did not change significantly during the fixation period. This find

Effect of manipulating the vergence/accommodation and image size mismatches of the ±2D flipper test on the frequency and precision of accommodative facility.

PURPOSE: The ±2.00 D accommodative facility test presents several limitations, including the lack of objective information and inherent characteristics such as vergence/accommodative conflict, change in apparent size of the image, subjective criteria for judging blur and motor reaction time. By using free-space viewing conditions and an open-field autorefractor to monitor the refractive state, we examined the impact of manipulating these factors on the qualitative and quantitative assessment of accommodative facility. METHODS: Twenty-five healthy young adults (24.5 ± 4.5 years) took part in t

The effect of incipient presbyopia on the correspondence between accommodation and vergence.

PURPOSE: To investigate the accommodation-convergence relationship during the incipient phase of presbyopia. The study aimed to differentiate between the current theories of presbyopia and to explore the mechanisms by which the oculomotor system compensates for the change in the accommodation-convergence relationship contingent on a declining amplitude of accommodation. METHODS: Using a Canon R-1 open-view autorefractor and a haploscope device, measurements were made of the stimulus and response accommodative convergence/accommodation ratios and the convergence accommodation/convergence ratio
