[Diet and coronary disease].

The build up of cholesterol in the atheromas is caused by, among other things, a disequilibrium of certain plasmatic lipoproteins. Food components which modify the levels of these lipoproteins can be considered as "atherogenic". Other components influence platelet activity and can be considered "thrombogenic". The saturated fatty acids, C-12, C14 and C-16, are atherogenic because they increase LDL plasmatic levels when consumed in large quantities. Estearic acid would not be included in this group because it is easily transformed into oleic acid in the organism. However, all the above acids ar

Coronary disease in women.

Myocardial infarction is uncommon in younger women in the absence of coronary risk factors; cigarette smoking and diabetes mellitus are major contributors to coronary risk. The presentation of coronary disease in women is more likely to be angina pectoris than myocardial infarction or sudden death. Myocardial infarction in women is characterized by increased morbidity and mortality, an excess of congestive heart failure, and a less favorable response to coronary bypass surgery. Psychosocial complications are greater and return to work is less likely.

Comparison of Coronary Computed Tomography Angiography and Invasive Coronary Angiography in Nonobstructive Coronary Artery Disease: The Brazilian Coronary ARtery Disease (BARD) Study.

BACKGROUND: The prognosis of nonobstructive coronary artery disease (CAD) is not well established. Comparative data between coronary computed tomography angiography (CCTA) and invasive coronary angiography (ICA) are limited. OBJECTIVES: To compare CCTA- and ICA-derived information in nonobstructive CAD regarding detection and clinical outcomes. METHODS: We followed 4,004 adult patients who underwent either ICA (n = 2,355) or CCTA (n = 1,649) for a median of 9 years. The primary endpoint was a composite of all-cause mortality, acute coronary syndrome/acute myocardial infarction, and stroke. P

The Role of Landiolol in Coronary Artery Disease: Insights into Acute Coronary Syndromes, Stable Coronary Artery Disease and Computed Tomography Coronary Angiography.

Coronary artery disease (CAD) constitutes a major contributor to morbidity, mortality and healthcare burden worldwide. Recent innovations in imaging modalities, pharmaceuticals and interventional techniques have revolutionized diagnostic and treatment options, necessitating the reevaluation of established drug protocols or the consideration of newer alternatives. The utilization of beta blockers (BBs) in the setting of acute myocardial infarction (AMI), shifting from the pre-reperfusion to the thrombolytic and finally the primary percutaneous coronary intervention (pPCI) era, has become increa

[Coronary artery anomalies and coronary disease: is there a relationship?].

BACKGROUND: It has been suggested that in coronary anomalies the turbulence of the flow might develop atherosclerosis, but this theory remains controversial. The relationship between coronary anomalies and coronary artery disease have been studied. METHODS: Twenty-five patients [15 males, 10 females (mean age 75+/-1.4 years)] with coronary anomalies at coronary angiography were enrolled in a prospective study. Thirteen patients out of 25 (group 1) had critical coronary artery disease (>50% of reduction in luminal diameter). The anomalous vessels passing intramyocardial or between aorta and pu

Coronary Involvement of Behçet's Disease: Saccular Coronary Artery Aneurysm.

Coronary artery aneurysms (CAAs) are rarely reported in large angiographic series. The majority are atherosclerotic in origin. Other causes are connective tissue disorders, trauma, vasculitis, congenital, mycotic, and idiopathic. We herein present the case of an symptomatic patient with a giant left anterior descending artery aneurysm. The CAA was successfully treated by surgical resection and a mammary artery bypass graft.

Native Coronary Disease Progression Post Coronary Artery Bypass Grafting.

BACKGROUND: It remains unclear if graft type impacts native disease progression in the target coronary artery post coronary artery bypass grafting (CABG). METHODS: Patients who underwent repeat angiograms at least 6 months post CABG with ≥1 arterial graft were included. Pre/post CABG angiograms were examined by 2 experienced readers. Progression was defined as new stenosis of ≥50% in a previously normal coronary, an increase in previous stenosis of ≥20%, or a new occlusion. Primary outcome was the occurrence of native disease progression in bypassed vessels. Secondary outcomes included comple

Selective coronary arteriography: arteriographic patterns in coronary heart disease.

The severity and distribution of coronary arteriographic abnormalities have been reviewed in 88 patients with clinical evidence of coronary heart disease who were studied by Sones' technique. The patients were divided into four groups: myocardial infarction without angina, myocardial infarction with angina, angina with normal resting electrocardiogram, angina with abnormal resting electrocardiogram.Arteriographic abnormalities were generally diffuse throughout the coronary circulation, and at least two vessels were involved in 84 patients. Although the frequency of lesions was similar in the f

Coronary artery ectasia in atherosclerotic coronary artery disease, inflammatory disorders, and sickle cell disease.

Coronary artery ectasia (CAE) or aneurysm is usually defined as dilation ≥1.5-fold the normal vessel diameter. It has an incidence of 1.4-5.3% and is associated with a wide variety of etiologies-mainly congenital, atherosclerotic, and inflammatory ones. CAE is very common in sickle cell disease, and possibly sickle cell trait, with an incidence of 17.7%. It is likely related to the inflammatory process associated with hemoglobin S. Prognosis depends mainly on the underlying etiology. Atherosclerotic CAE does not carry additional risks compared to atherosclerotic coronary artery disease (ACAD)

Coronary artery disease and percutaneous coronary intervention in patients with severe chronic kidney disease.

Coronary artery disease (CAD) is the leading cause of mortality among patients with chronic kidney disease (CKD), presenting unique challenges in diagnosis and management. Advanced CKD patients often present with atypical symptoms, and conventional diagnostic and interventional approaches carry risks, including contrast-induced nephropathy and the potential need for renal replacement therapy. These risks have led to the phenomenon of "renalism," where necessary procedures may be deferred due to concerns over renal injury. Emerging techniques, such as ultra-low contrast angiography (ULCA) and z

Coronary artery disease in adult congenital heart disease: outcome after coronary artery bypass grafting.

BACKGROUND: Atherosclerotic coronary artery disease may be seen during repair of adult congenital heart disease (ACHD). There are few data outlining outcomes of concomitant coronary artery bypass grafting (CABG) in these patients. METHODS: Between February 1972 and August 2009, 122 patients (77 men) underwent concomitant CABG at the time of ACHD repair; median age was 64 years (range 40 to 85 years). Thirty patients (25%) had preoperative angina, 7 patients (6%) had previous myocardial infarction (MI), and 6 patients (5%) had previous percutaneous intervention. Most common primary cardiac dia

When to Consider Coronary Revascularization for Stable Coronary Artery Disease.

Revascularization is an effective adjunct to medical therapy for some patients with chronic coronary disease. Despite numerous randomized trials, there remains significant uncertainty regarding if and how to revascularize many patients. Coronary artery bypass grafting is a class I indication for patients with significant left main stenosis or multivessel disease with ejection fraction ≤ 35%. For other patients, clinicians must carefully consider the potential benefits of symptom improvement and reduction of future myocardial infarction or CV death against the risk and cost of revascularization

Coronary microvascular dysfunction equivalent to left main coronary artery disease.

Coronary microvascular dysfunction, also known as cardiac syndrome X, is a clinical syndrome presenting with typical angina and evidence of myocardial ischemia in the absence of flow-limiting stenosis on coronary angiography. Of patients undergoing coronary angiography due to suspected myocardial ischemia, 50% are found to have normal or near-normal coronary arteries. Described in this case report is a patient who developed hypotension and ST segment depressions during treadmill exercise test. Left main coronary artery or multivessel disease was suspected. Coronary angiography was normal, but

Periodontal and coronary heart disease in patients undergoing coronary angiography.

Periodontal inflammation has been implicated in atherosclerosis and coronary heart disease (CHD). Coronary angiography (CA) is used in the assessment of CHD; only a few studies have evaluated periodontal disease (PD) and angiographic measures of coronary atherosclerosis. The aim of this study was to investigate the association between CHD and PD. In this prospective epidemiologic study, 466 patients underwent CA and were assessed for PD. All patients underwent physical, laboratory, cardiac, and dental examination including dental x-rays. Periodontal disease and coronary angiograms were evaluat
