Obesity and clinical restenosis after coronary stent placement.

BACKGROUND: Obesity has been associated with improved clinical outcomes after percutaneous coronary intervention--"obesity paradox." METHODS: We studied 6186 patients pooled from 6 major coronary stent clinical trials. Clinical restenosis was defined as target lesion revascularization (TLR) at 1 year. Body mass index was classified according to World Health Organization criteria as underweight (< 18.5 kg/m2), normal weight (18.5-24.9 kg/m2), overweight (25-29.9 kg/m2), obesity class I (30-34.9 kg/m2), and obesity class II/III (> or = 35 kg/m2). Multivariable logistic regression analysis was u

Hostility predicts restenosis after percutaneous transluminal coronary angioplasty.

OBJECTIVE: To investigate the "toxic" total (potential for) hostility component of the type A behavior pattern (assessed by means of the structured interview) as it relates to prediction of restenosis after percutaneous transluminal coronary angioplasty (PTCA). DESIGN: Patients with single vessel or multivessel coronary artery disease in whom PTCA had been scheduled or done were administered the structured interview by one trained interviewer prospectively or retrospectively (blinded to angiographic endpoints). MATERIAL AND METHODS: A total of 41 patients underwent 53 initial balloon dilatio

Effect of prior coronary restenosis on the risk of subsequent restenosis after stent placement or directional atherectomy.

Lesions that have developed restenosis after a prior intervention may be more likely to develop restenosis after subsequent percutaneous interventions. To determine if this is an independent effect, the clinical characteristics and immediate angiographic outcomes of 179 prior restenosis lesions were compared with those of 254 primary lesions after stenting or directional atherectomy. Six-month angiographic follow-up was obtained for 79% of successfully treated lesions. Univariable and multivariable logistic regression was used to determine how binary restenosis (defined as > or = 50% diameter

Restenosis of the Coronary Arteries: Past, Present, Future Directions.

Restenosis is a pathologic response to vascular injury, characterized by neointimal hyperplasia and progressive narrowing of a stented vessel segment. Although advances in stent design have led to a dramatic reduction in the incidence of restenosis, it continues to represent the most common cause of target lesion failure following percutaneous coronary intervention. Efforts to maximize restenosis prevention, through careful consideration of modifiable risk factors and an individualized approach, are critical, as restenosis, once established, can be particularly difficult to treat. Novel approa

[Endoprosthetic repair with stent grafts in coronary artery restenosis].

Timely performed reconstructive operations on carotid arteries considerably decrease the incidence rate of ischaemic-type acute impairments of cerebral circulation. Nevertheless, restenoses developing in the zone of reconstruction of carotid arteries level the positive result of the primary operation in remote terms of follow up. In such cases it is more preferable to perform stenting of the damaged portions. The article deals with cases of successful use of self-expandable stent grafts in restenoses of carotid arteries after carotid endarterectomy and operations of prosthetic repair of caroti

Proximal coronary artery intervention: stent thrombosis, restenosis and death.

BACKGROUND: Percutaneous coronary intervention (PCI) of lesions in the proximal left anterior descending coronary artery (LAD) may confer a worse prognosis compared with the proximal right coronary artery (RCA) and left circumflex coronary artery (LCX). METHODS: From May 2005, to May 2011 we identified all PCIs for proximal, one-vessel coronary artery disease in the Swedish Coronary Angiography and Angioplasty Registry (SCAAR). We evaluated restenosis, stent thrombosis (ST) and mortality in the LAD as compared to the RCA and LCX according to stent type, bare metal (BMS) or drug-eluting stents

Percutaneous interventions in radiation-associated coronary in-stent restenosis.

This study was performed to evaluate the outcome of percutaneous revascularization in "edge restenoses" developing after radioactive stent implantation in de novo and in-stent lesions. Twenty-one consecutive patients undergoing target lesion revascularization (TLR) at any follow-up after phosphorus-32 radioactive stent implantation were included in this study. We assessed the incidence of death, myocardial infarction, repeated TLR and recurrent angina over the following 18 months. After 6 months, TLR rate was 28.6%, and no stent thromboses, deaths or Q-wave myocardial infarctions occurred. Amo

Prevention of restenosis by bezafibrate after successful coronary angioplasty.

BACKGROUND: To study the role of bezafibrate in prevention of restenosis after successful percutaneous transluminal coronary angioplasty (PTCA), we evaluated the incidence of restenosis and its correlation with serum lipid levels and effects on the coagulation-fibrinolytic system. METHODS: Subjects who had undergone successful elective PTCA were classified into three groups based on their triglyceride level and whether or not bezafibrate was administered. Fifty-two patients who had a triglyceride level < 150 mg/dl were classified as group A. Those with a triglyceride level +/- 150 mg/dl were

Can coronary flow parameters after stent placement predict restenosis?

Intracoronary stent was used to treat a dissection resulting from balloon angioplasty. Coronary flow parameters measured with Doppler Flowire pre- and post-stenting suggested abnormal coronary flow even after stenting. The patient returned 4 months later with an instent restenosis. This case suggests the intriguing possibility of coronary flow parameters obtained immediately after new device angioplasty in predicting restenosis.

Pharmacological inhibition of coronary restenosis: systemic and local approaches.

INTRODUCTION: Percutaneous coronary intervention (PCI) with stent implantation has revolutionized the treatment of obstructive coronary artery disease. However, the main limitation of this therapy is stent failure, which is usually caused by in-stent restenosis. AREAS COVERED: The aim of this article is to critically review the literature on the prevention of in-stent restenosis focusing on drug compounds that have reached clinical testing. EXPERT OPINION: The pathophysiological response following PCI includes many possible targets for antirestenosis treatment. Most notable success is seen w

Morphological predictors of restenosis after coronary stenting in humans.

BACKGROUND: Experimental studies suggest that arterial injury and inflammation lead to increased neointimal growth after stenting. Despite the increased use of coronary stents in humans, there are only limited pathological data on the morphological features of in-stent restenosis. METHODS AND RESULTS: Detailed histology was performed on 116 stents, implanted > or =90 days in 87 coronary arteries, from 56 patients (mean age, 59+/-13 years). The mean duration of stent implant was 10 months. In-stent restenosis was defined as a stent area stenosis of >75%. Lumen area increased as stent area incr

Dyslipoproteinemias in atherosclerosis, thrombosis and restenosis after coronary angioplasty.

Lipids play a vital role in normal metabolic function in mammals. However, dyslipoproteinemias have been implicated in the pathophysiologic process of atherogenesis, thrombogenesis and restenosis after interventional procedures. Lipoproteins provide important chemical linkages among these three complex phenomena. Lipoproteins participate in atherogenesis and play a major role in plaque fissuring, the pathophysiologic common denominator of acute ischemic syndromes. Thrombogenesis is majoraly affected by the action of lipids on platelets, coagulation and fibrinolysis. LDL tend to destabilize pla

Restenosis after transluminal coronary angioplasty: a risk factor analysis.

In order to determine the relationship of restenosis following percutaneous transluminal coronary angioplasty (PTCA) to risk factors such as hypercholesterolemia, hyperglycemia, smoking, and weight, we performed a univariate analysis to test the association of these variables with restenosis in 723 patients who had percutaneous transluminal coronary angioplasty and follow-up catheterization. Cholesterol levels were higher in younger and female subjects (less than 0.0001). Initial cholesterol did not predict restenosis, and follow-up cholesterol levels showed an inverse relationship with resten

PCI for in-stent restenosis of an anomalous right coronary artery.

Coronary angioplasty for congenital anomalies of the coronary arteries (ANOCOR) remains a technically demanding procedure due to the difficulties associated with selective cannulation of the coronary ostium and achieving adequate support for the guide catheter. We report a case of in-stent restenosis in a proximal connection anomaly of the right coronary artery arising from the left coronary sinus, which was successfully treated using an innovative telescopic technique. A 63-year-old female patient, presenting with multiple cardiovascular risk factors and having previously undergone angioplast

Research advances in the etiology of in-stent restenosis of coronary arteries.

This article conducts a systematic literature analysis, focusing on exploring the etiological factors of coronary in-stent restenosis(ISR). It is found that the occurrence of ISR is influenced by a variety of factors, including patient-related biological factors (such as diabetes and smoking), lesion anatomical factors (such as calcification and lesion length), and factors related to the surgical procedure (such as incomplete stent expansion and stent malposition). Regarding the prevention of coronary in-stent restenosis, individualized treatment strategies need to be developed by combining pa

A Senolytic-Eluting Coronary Stent for the Prevention of In-Stent Restenosis.

The vast majority of drug-eluting stents (DES) elute either sirolimus or one of its analogues. While limus drugs stymie vascular smooth muscle cell (VSMC) proliferation to prevent in-stent restenosis, their antiproliferative nature is indiscriminate and limits healing of the endothelium in stented vessels, increasing the risk of late-stent thrombosis. Oxidative stress, which is associated with vascular injury from stent implantation, can induce VSMCs to undergo senescence, and senescent VSMCs can produce pro-inflammatory cytokines capable of inducing proliferation of neighboring nonsenescent V

Overview of therapies for prevention of restenosis after coronary interventions.

Coronary artery disease is a leading cause of morbidity and mortality in the United States and across the world. The economic impact of coronary artery disease is staggering and on the rise. Percutaneous transluminal coronary angioplasty is widely used to treat severe, symptomatic coronary stenosis. The Achilles heel of angioplasty is restenosis of those treated arteries. As a result, numerous therapies, including mechanical and pharmacological approaches, to prevent restenosis have been studied. A greater understanding of the pathophysiology of restenosis has enhanced the success of these the

Influence of the methodology of percutaneous transluminal coronary angioplasty on restenosis.

Restenosis after percutaneous transluminal coronary angioplasty (PTCA) remains the principal factor preventing broader application of this form of myocardial revascularization. Several methodologic variables contribute to the quality of the angioplasty result and may directly or indirectly influence restenosis rates after the procedure. PTCA operators attempt to minimize thrombotic activity at the angioplasty site by delaying PTCA when thrombus is present, with heparin and antiplatelet agents and with thrombolytic agents if thrombus is identified during the procedure. Therapy directed at preve
