Advancing Cortical Bone Mapping with global optimization.

Cortical thickness is important in many domains of bone health research. Cortical Bone Mapping (CBM) is a model-based method for measuring cortical thickness in clinical CT, but has not gained traction in the high-resolution peripheral computed tomography (HR-pQCT) community. In this work, we advance CBM by introducing two novel extensions that fit the model globally rather than locally, allowing for spatial regularization of fitted parameters. With two repeat-measures precision HR-pQCT datasets (of the redproximal tibia, distal radius and distal tibia), we show that both the original and nove

The myokine irisin increases cortical bone mass.

It is unclear how physical activity stimulates new bone synthesis. We explored whether irisin, a newly discovered myokine released upon physical activity, displays anabolic actions on the skeleton. Young male mice were injected with vehicle or recombinant irisin (r-irisin) at a low cumulative weekly dose of 100 µg kg(-1). We observed significant increases in cortical bone mass and strength, notably in cortical tissue mineral density, periosteal circumference, polar moment of inertia, and bending strength. This anabolic action was mediated primarily through the stimulation of bone formation, bu

Cortical bone allografting in femoral head necrosis.

Ten femoral heads (six patients) with avascular necrosis were operated on using a fibular allograft. The procedure included core decompression followed by insertion of a cortical bone graft in order to relieve mechanical stresses from the overlying subchondral bone. The presence of the supporting graft should avoid an expected collapse or prevent its worsening if already present. A freeze-dried and processed cortical bone allograft was preferred to an autograft. Weightbearing was normally and fully resumed at the second postoperative month. There were three failures within the first year, four

Repair of microdamage in osteonal cortical bone adjacent to bone screw.

Up to date, little is known about the repair mode of microdamage in osteonal cortical bone resulting from bone screw implantation. In this study, self-tapping titanium cortical bone screws were inserted into the tibial diaphyses of 24 adult male rabbits. The animals were sacrificed at 1 day, 2 weeks, 1 month and 2 months after surgery. Histomorphometric measurement and confocal microscopy were performed on basic fuchsin stained bone sections to examine the morphological characteristics of microdamage, bone resorption activity and spatial relationship between microdamage and bone resorption. Stiffening effects of cortical bone on vertebral cancellous bone in situ.

STUDY DESIGN: This study tested a theory about vertebral cancellous bone stiffness by performing experimental tests and comparing the results with the theoretical predictions. OBJECTIVES: To test experimentally a theoretical prediction that vertebral cancellous bone appears stiffer than would be expected from isolated tests because of the constraining effects of the cortical bone, to measure the magnitude of this strengthening effect and its dependency on tissue composition and density. SUMMARY OF BACKGROUND DATA: Vertebral bodies are composed mainly of cancellous bone surrounded by a thin s

Heat accumulation during sequential cortical bone drilling.

Significant research exists regarding heat production during single-hole bone drilling. No published data exist regarding repetitive sequential drilling. This study elucidates the phenomenon of heat accumulation for sequential drilling with both Kirschner wires (K wires) and standard two-flute twist drills. It was hypothesized that cumulative heat would result in a higher temperature with each subsequent drill pass. Nine holes in a 3 × 3 array were drilled sequentially on moistened cadaveric tibia bone kept at body temperature (about 37 °C). Four thermocouples were placed at the center of four

Bariatric surgery results in cortical bone loss.

BACKGROUND: Bariatric surgery results in bone loss at weight-bearing sites, the mechanism of which is unknown. METHODS: Twenty-two women (mean body mass index 44 kg/m(2); aged 45 years) who underwent Roux-en-Y gastric bypass (n = 14) and restrictive procedures (n = 8) had measurements of areal bone mineral density by dual-energy x-ray absorptiometry at the lumbar spine, total hip (TH), femoral neck (FN), and one third radius and trabecular and cortical volumetric bone mineral density and microstructure at the distal radius and tibia by high-resolution peripheral quantitative computed tomograp

Dynamic short crack growth in cortical bone.

It is well known for almost half a century that bones contain microcracks. Very little is known about the crack growth behaviour of very small cracks, e.g. the stage before they become macroscopically long. The aim of this work was to investigate the dynamic crack growth behaviour of sub-millimetre microcracks in cortical bone. It was found that slow stable crack growth occurs in specimens subjected to static loading conditions. Crack growth direction was dominated by the local fibre orientation of the bones. Crack angles varied between 10 and 36 degrees of the long axis of the bone. Short cra

Microdamage of human cortical bone: incidence and morphology in long bones.

Increased bone fragility and osteoporotic fracture in human bone has been attributed to the accumulation of microdamage. According to Martin and Burr (J Biomech 15:137-139; 1982), microcracks from interstitial bone propagate to the cement line or concentric lamellae and debond or separate the Haversian canal from the surrounding bone which leads to repair of the damaged region. If this is true, we would expect to find a greater incidence of microdamage existing at the cement line than at other locations within the bone microstructure. The incidence of such an occurrence, however, is not establ

Menopause-Related Appendicular Bone Loss is Mainly Cortical and Results in Increased Cortical Porosity.

After menopause, remodeling becomes unbalanced and rapid. Each of the many remodeling transactions deposits less bone than it resorbed, producing microstructural deterioration. Trabecular bone is said to be lost more rapidly than cortical bone. However, because 80% of the skeleton is cortical, we hypothesized that most menopause-related bone loss and changes in bone microstructure are cortical, not trabecular in origin, and are the result of intracortical remodeling. Distal tibial and distal radial microstructure were quantified during 3.1 years (range, 1.5 to 4.5 years) of follow-up using hig

Cortical bone porosity is spatially heterogeneous and VEGF dependent in male bone.

Cortical bone is highly porous and composed of an interconnecting network of vascular canals and osteocyte lacunae. Our understanding of the mechanisms coupling vascular: lacunar spatial organisation in cortical bone is poorly understood. Defining cellular cross-talk mechanisms could be key in identification of reciprocal molecular signals driving increased cortical porosity with age. Driven by the hypothesis that porosity within bone is heterogeneous and influenced by region-specific spatial cues, we utilised synchrotron X-ray computed tomography to characterise intracortical canal and osteoc

Multiscale nanoindentation modelling of newborn piglet cortical bone.

This study presents an integrated experimental-numerical framework for the multiscale mechanical characterization of dehydrated newborn piglet cortical bone. A self-calibrated nanoindentation methodology, coupled with inverse finite element identification based on the Johnson-Cook constitutive model, is introduced to quantify both surface and bulk mechanical properties. Particular attention is given to indentation size effects and preliminary sex-related trends in mechanical response. The proposed framework enables robust estimation of elastoplastic parameters and highlights the presence of a

Ultrasound Characterization of Cortical Bone Using Shannon Entropy.

OBJECTIVE: Ultrasound backscattered signals encompass information on the microstructure of heterogeneous media such as cortical bone, in which pores act as scatterers and result in the scattering and multiple scattering of ultrasound waves. The objective of this study was to investigate whether Shannon entropy can be exploited to characterize cortical porosity. METHODS: In the study described here, to demonstrate proof of concept, Shannon entropy was used as a quantitative ultrasound parameter to experimentally evaluate microstructural changes in samples with controlled scatterer concentratio

Mechanical and metabolic interactions in cortical bone development.

OBJECTIVES: Anthropological studies of cortical bone often aim to reconstruct either habitual activities or health of past populations. During development, mechanical loading and metabolism simultaneously shape cortical bone structure; yet, few studies have investigated how these factors interact. Understanding their relative morphological effects is essential for assessing human behavior from skeletal samples, as previous studies have suggested that interaction effects may influence the interpretation from cortical structure of physical activity or metabolic status.
