[Acute renal failure following ingestion of Cortinarius orellanus in 12 patients. Initial presentation and progress over a period of 13 years].

INTRODUCTION: This study reports the largest series of acute renal failure following collective poisoning by Cortinarius orellanus since 1957. PATIENTS: Twelve men, in whom altered renal function appeared following ingestion of mushroom soup (Cortinarius orellanus) when they were 20 to 23 year-old, were followed up for 13 years. RESULTS: After a period of latency of between 2 to 5 days, the patients complained of asthenia, intense thirst and digestive and neurological disorders. On admission, 4 were anuretic and two exhibited polyuria. Leukocyturia was detected in all patients but without pr

[Acute Kidney Injury Caused by Orellanic Syndrome: Case Report].

Orellanic syndrome is caused by fungi of the Cortinarius orellanus and speciosissimus (Europe) species, Cortinarius fluorescens (South America), and Cortinarius rainierensis (North America). Orellanic syndrome is characterized by initially nonspecific symptoms such as muscle and abdominal pain, and a metallic taste sensation in the mouth. After a few days, more specific symptoms appear, such as intense thirst, headache, chills without fever, and anorexia, followed by a phase of polyuria and then of oligoanuria. Renal failure occurs in 70% of cases and is often irreversible. The clinical case i

Human and experimental toxicology of orellanine.

Orellanine is a nephrotoxic toxin produced by some mushroom species of the Cortinarius genus, typically found in Europe and North America. The nephrotoxicity of Cortinarius orellanus is well known and was first recognized in the 1950s when this mushroom was identified as the cause of a mass poisoning in Poland. Typically, onset of symptoms is delayed for 1-2 weeks after ingestion. Some patients suffer mild gastrointestinal discomfort in the latency period before developing signs of renal impairment due to severe interstitial nephritis, acute focal tubular damage, and interstitial fibrosis. The

[Orellanus syndrome: mushroom poisoning with kidney insufficiency].

The Orellanus syndrome is a rare nephrotoxic disease caused by several fungi of the genus Cortinarius. For a long time the sole report of this syndrome was a mass intoxication in Poland. About 32 cases of Orellanus syndrome caused by Cortinarius orellanus, speciocissimus and splendens have been described in recent years. A few other species are also suspected of being nephrotoxic. The syndrome is characterized by a delayed latency period of 2 days to 3 weeks, and a chronic evolution involving fatigue, anorexia, headache, thirst, pains in the lumbar region and renal insufficiency with oliguria

Renal involvement in mushroom poisoning: The case of Orellanus syndrome.

Although mushroom poisoning is a rare cause of acute renal injury, in some cases it may lead to the development of a severe and irreversible renal failure. Orellanus syndrome is the most important example of organic renal damage related to mushroom consumption. It is caused by the ingestion of orellanine, the main toxin of different types of Cortinarius mushrooms (Cortinarius speciosissimus, C. orellanus, C. orellanoides, etc.), and it is characterized by progressive clinical phases with a predominant kidney involvement, finally requiring renal replacement therapy in about 10% of cases. Renal

A convenient separation strategy for fungal anthraquinones by centrifugal partition chromatography.

As recently shown, some fungal pigments exhibit significant photoactivity turning them into promising agents for the photodynamic treatment of microbial infections or malignant diseases. In the present study, a separation strategy for fungal anthraquinones was developed based on centrifugal partition chromatography. A suitable method was explored employing a methanolic extract of the fruiting bodies of Cortinarius sanguineus (Agaricales, Basidiomycota). An excellent fractionation was achieved using a biphasic solvent system comprising chloroform/ethyl acetate/methanol/water/acetic acid (3:1:3:

Comprehensive Evaluation of the Antioxidant Potential of Coastal Dune Mushroom Species from the Southwest of France.

Numerous mushroom species are used as food and for medicinal purposes; however, many species that may contain bioactive compounds remain underinvestigated. In this study, the antioxidant properties of extracts sequentially isolated with cyclohexane, dichloromethane, and methanol from 25 costal dune mushroom species collected in the southwestern region of France were evaluated based on their radical scavenging capacity, ferric-reducing antioxidant power, oxygen radical absorbance capacity, and Folin-Ciocalteu-determined total phenolic content. Overall, the antioxidant potential of dried mushroo

[Mushroom poisonings: syndromic diagnosis and treatment].

The major syndromes of mushroom poisoning can be divided by presentation timing: Early syndromes (symptom onset <6 hrs after ingestion) have little probability to cause organ damage. Epigastric pain, nausea, vomiting and diarrhea occur in most cases and treatment includes initial gastrointestinal decontamination with oral activated charcoal and fluid rehydration. In addition, an acute gastrointestinal syndrome can be combined with cholinergic toxicity, epileptiformic response or immuno-hemolytic anemia. Neurotoxic Syndromes may present as dysphoria, delirium, hallucinations or disulfiram-like

Orellanine: From Fungal Origin to a Potential Future Cancer Treatment.

Fungal metabolites represent an underutilized resource in the development of novel anticancer drugs. This review will focus on the promising fungal nephrotoxin orellanine, found in mushrooms including Cortinarius orellanus (Fools webcap). Emphasis will be placed on its historical significance, structural features, and associated toxicomechanics. Chromatographic methods for analysis of the compound and its metabolites, its synthesis, and chemotherapeutic potential are also discussed. Although orellanine's exceptional selectivity for proximal tubular cells is well documented, the mechanics of it

Acute renal failure associated with suspected Amanita smithiana mushroom ingestions: a case series.

Mushroom poisoning leading to acute renal failure is extremely rare in North America. Cortinarius species and Amanita smithiana both can cause acute renal failure and inhabit the Pacific northwest. This article describes 4 patients who presented in acute renal failure and who ingested mushrooms described as resembling A. smithiana. Two patients (a 74-year-old Korean couple) described eating mushrooms with an approximately 6 x 0.5-inch stipe with a white cap, 1.25-1.5 inches in diameter. The other 2 patients (a 55-year-old male and a 30-year-old female) also described a white-capped mushroom. A

Characterization of fungal microbial diversity in healthy and diarrheal Tibetan piglets.

BACKGROUND: Diarrhea is an important ailment limiting the production of the Tibetan pig industry. Dynamic balance of the intestinal microbiota is important for the physiology of the animal. The objective of this work was to study fungal diversity in the feces of early weaning Tibetan piglets in different health conditions. RESULTS: In the present study, we performed high-throughput sequencing to characterize the fungal microbial diversity in healthy, diarrheal and treated Tibetan piglets at the Tibet Autonomous Region of the People's Republic of China. The four alpha diversity indices (Chao1,

Cytotoxic fungi--an overview.

Among fungal toxins causing organ damage in the human body, amatoxins and orellanine remain exceptional. Amatoxins, a group of bicyclic octapeptides occurring in some Amanita, Galerina and Lepiota species, induce deficient protein synthesis resulting in cell death, but might also exert toxicity through inducing apoptosis. Target organs are intestinal mucosa, liver and kidneys. Poisoning will result in dehydration and electrolyte derangement, liver necrosis and possibly kidney damage. In established poisoning the mainstay of treatment is optimum symptomatic and supportive care. No specific trea

Amanita poisonings resulting in acute, reversible renal failure: new cases, new toxic Amanita mushrooms.

BACKGROUND: Renal failure as a consequence of eating mushrooms has been reported repeatedly after ingestion of webcaps of the Cortinarius orellanus group. But mushrooms of the genus Amanita can also cause renal failure: Amanita smithiana (North America) and Amanita proxima (Mediterranean area). Here, we discuss poisonings caused by other white amanitas. A German and--independently--two Portuguese patients reported the ingestion of completely white mushrooms with ring. Similar to intoxications with A. smithiana or A. proxima, the clinical picture was characterized by nausea and vomiting 10-12 h

[A rare case of acute renal failure related to amanita proxima ingestion].

OBJECTIVE: The first cases of acute renal tubulopathy related to Amanita proxima poisoning were described in 1994; the mushroom contains a toxin responsible for the allenic-norleucine syndrome, characterised by a kidney damage that occur earlier than in Cortinarius orellanus poisoning and generally improve with complete resolution within ten days. CASE REPORT: A 45 years old woman was admitted to the hospital because of nausea, vomiting, and heartburn started the day before. Symptoms occurred eight hours after eating a single big wild mushroom; three friends who had the same meal were all asy

Fungi ingestion as an important factor influencing heavy metal intake in roe deer: evidence from faeces.

In nature, animals have to cope with the fluctuating bioavailable metal pool in their habitat, which results in a seasonal variability of heavy metal levels in the animal body. Indeed, a pronounced summer-autumnal peak of heavy metals in roe deer (Capreolus capreolus L.) kidney was recently found in Slovenia. Considering the well-known hyperaccumulative ability of fungi, their ingestion was hypothesised to be one of the main reasons for the peak. Although fungi as a group are known to be a seasonally important food source for roe deer, data on their composition in the nutrition of the species
