[Interpersonal diagnosis of personality pathology: countertransference as a source of information].

BACKGROUND: One of the defining characteristics of personality pathology is an interpersonal style that is experienced as problematic by the patient and/or others. Patients typically have little awareness of the dysfunctional effects of their interpersonal style. This represents a significant source of psychological suffering for both the patient and their social environment. Systematically recording countertransference helps both patient and psychiatrist to clarify these dysfunctional effects. AIM: To familiarize readers with the interpersonal paradigm that underlies the recording of counter

An interactive view of countertransference: Differentiation of self and client presentation.

Using family systems theory (Kerr & Bowen, 1988), interpersonal theory (Kiesler, 1996; Leary, 1957), and the interactional theory of countertransference (CT; Gelso & Hayes, 2007) as a framework, we investigated the influences of therapist personal factors (differentiation of self in relation to the family of origin; DoS) and client presentation as predictors of CT feelings and behaviors in response to a video analog client. Therapist-participants viewed either an interpersonally hostile-submissive or interpersonally hostile-dominant (HD) client. Overall, therapists lower in DoS reported signif

Countertransference in the treatment of patients with personality disorders: A longitudinal study.

OBJECTIVE: This study examines how therapist emotional response/countertransference (CT) develops during treatment for patients with personality disorders (PDs) and how pre-treatment patient factors (severity of personality pathology, PD category, level of symptom distress) predict CT responses. Secondly, we explored associations between patient clinical outcome and CT. METHOD: A longitudinal, observational study including 1956 patients with personality pathology treated at psychotherapy units within specialist mental health services. Therapists' emotional response was repeatedly assessed by

A long-term case study with sadistically-tinged countertransference elements.

This case report sketches the psychoanalytically-informed supportive psychotherapeutic treatment of a single man through his life challenges over a twenty-year period. The decades-long challenges arise from obsessive compulsive disorder, pathologic self-effacement with insatiable and unusual erotic components, and severe visual impairment leading to blindness. Sadistically-tinged impulses arising in the countertransference are described.

Assisted dying and countertransference: unconscious forces in life-and-death decisions.

Assisted dying debates overlook the powerful unconscious forces that shape end-of-life decision-making. These dynamics influence personal, clinical and societal judgements and may be contributing to the rapid international expansion of assisted dying practices. Strengthening safeguards requires acknowledging these forces and integrating structured psychological assessment, clinician support and reflective practice to reduce unconscious bias and enhance the reliability, transparency and ethical integrity of decisions.

The Meaning of Together: Exploring Transference and Countertransference in Palliative Care Settings.

Establishing an empathic clinical relationship is a cornerstone of high-quality palliative care. More than simply approaching patients with a pleasant affect or "being nice," we propose that skilled clinicians routinely employ distinct psychological elements when creating effective bonds with seriously ill patients and their families. Palliative care involvement has been shown to improve a variety of outcomes for patients with serious cancer, and yet the components of this salutary effect are still becoming known in the literature. Many believe that a successful interpersonal relationship is t

An empirical study of countertransference reactions toward patients with personality disorders.

OBJECTIVE: The study examined to what extent patients with cluster A + B personality disorders (PDs) evoked other countertransference reactions among psychotherapists compared with patients with cluster C PDs as well as the relationship between the different countertransference reactions and outcome. METHODS: A total of 11 therapists at the Department for Personality Psychiatry, Ullevaal University Hospital, Oslo, Norway, filled out the Feeling Word Checklist-58 (FWC-58), 2 weeks after admission and 2 weeks before discharge, for 71 patients admitted to the day treatment program. The patients

Clinician Diagnostic Ratings and Countertransference Reactions Towards Grandiose and Vulnerable Narcissism.

BACKGROUND: Narcissistic personality disorder (NPD) is known to comprise two distinct but connected phenotypes related to 'grandiosity' and 'vulnerability', respectively. While evidence suggests differing countertransference responses to narcissism subtype expression, no study has examined this using a qualitative methodology and explored associations with ratings of personality disorder severity. METHOD: Mental health clinicians (N = 180, 67% female, age = 38.9), completed qualitative clinical reflections and ratings of overall personality disorder severity towards two hypothetical vignettes

Dialogue With Psychosis: Induced Psychotic Countertransference in Psychotherapy With Psychotically Disturbed Patients.

The therapeutic encounter with psychotic patients presents therapists with moments in which they may regress, together with the patient, toward primitive and psychotic areas of experience. Within this shared psychotic world, therapists might feel persecuted, as if the ground is slipping from beneath their feet. The author suggests that the psychotic part of the personality, as argued by Bion, is inherent to all of us and may come alive in the psyche of the therapist in response to patients in psychotic states. The psychotic dialogue that emerges between patient and therapist, which involves pr

Keeping your eye on the process: body image, older women, and countertransference.

Research on body image and older women has grown in the past decade. However, there is a gap in the literature regarding body image, older women, and countertransference. This article provides 7 case examples of racially and ethnically diverse women over 60, drawn from MSW student and agency staff supervision, and participant feedback from a national conference on aging workshop. Themes related to loss and grief, adult daughter and aging mother issues, incest, anger, disability, personality disorders, phobic reactions, and shame are discussed. Recommendations and implications for social work p

Countertransference reactions to adolescents with eating disorders: relationships to clinician and patient factors.

OBJECTIVE: Clinical report suggests that therapists have strong and sometimes difficult-to-manage reactions to patients with eating disorders (EDs); however, systematic research is largely absent. The purpose of this study was to explore the emotional responses, or countertransference (CT) reactions, clinicians experience when working with patients with EDs, and to identify clinician, patient, and therapy variables associated with these responses. METHOD: One hundred twenty clinicians reported on multiple variables related to an adolescent female patient they were treating for an ED. RESULTS

Team approach to working through transference and countertransference in a pediatric/psychiatric milieu.

The principles of transference and countertransference are essential to understanding and working through the intense pain and anger that often dominate the emotional field of the pediatric/psychiatric milieu. Using Winnicott's model of the maternal/infant relationship, this milieu can be seen as a holding environment where transferential phenomena can be expressed, interpreted, and worked through. Anger and pain are common elements of the transference/countertransference relationship matrix of the milieu. Staff often have a strong bias regarding anger and hostility. If they are willing to exa

Assessing objective countertransference with a computer-delivered Impact Message Inventory (IMI-C).

The circumplex version of the Impact Message Inventory (IMI-C) is one of the few existing diagnostic devices for the assessment of 'objective' (inter-subjective) countertransference. The IMI-C purports to assess eight categories of countertransference reactions along the circumference of the interpersonal circle, conceptualized as blends of the two basic dimensions of affiliation (AFF) and control (CON): dominant (D), hostile-dominant (HD), hostile (H), hostile-submissive (HS), submissive (H), friendly-submissive (FS), friendly (F) and friendly-dominant (FD). As a measure of objective (or 'int

Countertransference responses mediate the relationship between patients' overall defense functioning and therapists' interventions.

Objective: The aim of this work was to investigate the relationship between therapists' techniques (supportive/expressive intervention level - ESIL), therapists' countertransference (CT) and patients' defense level of functioning (ODF). We hypothesized that CT could play a mediating role in the relationship between patients' ODF and therapists' ESIL. Method: 65 psychotherapy sessions were rated by two different raters each with the Psychodynamic Interventions Rating Scale and the Defense Mechanisms Rating Scale. Clinicians used the Therapist Response Questionnaire to provide information on the

Hot Feelings: Sexual Transference and Countertransference with Male Mid-Adolescents and a Female Psychoanalyst.

The author focuses on the workings of the female analyst-male pair in the consulting room when sexual feelings emerge as part of the adolescent storm. The need for open-bodiedness in relation to the perception of the bodily states of both the analyst and analysand is described and discussed. The author shows how somatic countertransference, reverie, and projective identification are harnessed creatively in the service of transformation. The importance of the third to help provide an analytic space for thought and meaning, rather than enactment and impasse, is discussed. The trajectory from the

Cultural Countertransference of Young Clinicians Facing a State of Possession: A Cultural Case Study.

Possession states are a well-known but a complex clinical aspect of transcultural psychiatry. How do young clinicians react when facing such symptoms? Starting from the case of a Somali patient suffering from possession states, treated in a transcultural consultation group, we will attempt to explain the cultural countertransference experienced by young clinicians who witnessed a trance. Using the Cultural Formulation Interview, we observed an evolution of the countertransference, and therefore an evolution of the symptom's understanding and of the patient's condition. This clinical case thus

Countertransference, alliance, and outcome in the treatment of patients with personality disorder: a longitudinal naturalistic study.

OBJECTIVE: Relational dynamics, including countertransference responses and the therapeutic alliance, are crucial in the treatment of patients with personality disorders (PD). However, few studies on PD treatment focus on the dyadic process of therapy. The present study aims to investigate associations between therapist emotional response/countertransference (CT) and patients' experience of treatment alliance, and CT developments in therapies with treatment completion as outcome. METHOD: A longitudinal, observational study of patients (N = 365) treated at PD treatment units within specialist

Differences and similarities in instant countertransference towards patients with suicidal ideation and personality disorders.

BACKGROUND: Previous findings showed that suicidal patients elicit mostly negative countertransference such as distress, hopelessness, feelings of inadequacy, and apprehension, and that a concurrent personality disorder is associated with more feelings of entrapment and mistreatment, among other adverse reactions. No studies were however conducted on instant countertransference (iCT), i.e., after a single encounter, for example in an emergency setting. We aimed to evaluate the impact of suicidal ideations, self-harm and presence of personality disorders on instant Countertransference (iCT). M

The RFP-C Triangle of Conflict: Listening to Countertransference Helps Listening to Children With Disruptive Behaviors.

Countertransference is universal in the treatment of children with disruptive behaviors. Clinicians must be aware of their countertransference in order to use those feelings as therapeutically as possible. Understanding their countertransference can help clinicians understand and address the defensive aspects of disruptive behaviors that protect children from consciously experiencing painful emotions. Regulation-focused psychotherapy for children (RFP-C), the first manualized psychodynamic treatment for children with disruptive behaviors, uses the RFP-C triangle of conflict as a heuristic to h
