The posterior fontanelle: a neglected acoustic window.

The additional information, obtained when using the posterior fontanelle routinely as the second acoustic window, is illustrated in four infants. Three of them are full-term infants with hypoxic ischaemic encephalopathy. In newborn infants, who are still too unstable to be transported to the magnetic resonance unit, extensive damage in the occipital subcortical white matter and/or cortex can be visualised by performing cranial ultrasound through the posterior fontanelle.

Wormian bones in simple sagittal synostosis: incidence and correlations.

PURPOSE: Wormian bones are accessory cranial bones of interest to neurosurgeons, radiologists, and anthropologists. Its origin is attributed to alteration in deformational forces. Sagittal synostosis alters normal cranial development and is a suitable model to explore the role of mechanical forces in wormian bone development. We aim to study the incidence and clinical correlations of wormian bones in patients with isolated sagittal synostosis operated on at Hospital La Fe in the past 10 years. METHODS: We retrospectively reviewed clinical and radiological data from patients with isolated sagi

A new view of the neonatal brain: clinical utility of supplemental neurologic US imaging windows.

Anterior fontanelle imaging has traditionally been the mainstay of neonatal cranial ultrasonography (US). However, this traditional approach has limited diagnostic accuracy, which has led to increasing use of two alternate neurologic US imaging techniques: posterior fontanelle imaging and mastoid fontanelle imaging. These alternate techniques can help detect pathologic conditions and structural malformations in the neonatal brain. Posterior fontanelle imaging allows improved detection of intraventricular hemorrhage. This technique better demonstrates subtle differences in echogenicity between

Ultrasonographic Measurement of Anterior Fontanelle Size in Infants with Deformational Plagiocephaly.

Background/Objectives: We aimed to investigate the relationship between deformational plagiocephaly (DP) severity and anterior fontanelle size and to explore the connection between fontanelle size and developmental delay. Methods: We enrolled 189 (122 boys and 67 girls; mean corrected age, 119.79 days) of the 256 infants who visited our clinic for plagiocephaly between March 2022 and June 2023. This study analyzed the correlation between cranial vault asymmetry (CVA) and anterior fontanelle size as measured using skull anteroposterior (AP) radiography and ultrasonography. The severity of DP wa

Cerebrospinal fluid external leak after penetrating trauma in a neurologic intact infant patient: a case report.

Cranial cerebrospinal fluid (CSF) leak is an extremely rare complication of blunt head trauma causing skull fractures, especially fractures involving the skull base. We present the case of a 10-month-old male who received glass fragments on the midline and posterior tier of his anterior fontanelle producing a cranial cerebrospinal fluid leak without any skull fracture or symptoms. Neurologic exam was completely normal and a superficial stitch wound repair was performed. He was observed for 24 h, had no antibiotic, and left with a 1-week outpatient neurosurgical follow-up. The patient had no ne

[Cleidocranial dysplasia: a case report].

A 3-month-old female infant who presented with patent sagittal suture and loss of weight is described. Physical examination revealed a large sagittal and metopic suture showing delayed closure, a high-arched palate, saddle nose, hypertelorism and nonpalpable edges of the bilateral clavicles. The clavicles also showed undue mobility. Radiological investigations of the cranial skeletal abnormalities revealed enlargement of the sagittal and metopic sutures, and the anterior and posterior fontanelles. A chest radiograph showed a small, bell-shaped thoracic rib cage with partial aplasia of both cla

A rare presentation of cleidocranial dysplasia.

Cleidocranial dysplasia (CCD) is a rare skeletal dysplasia characterised by a defect in ossification. Frequently reported clinical findings are delayed closure of the fontanelles and cranial sutures, hypoplastic clavicles, dental abnormalities and short stature. Our patient suffered from complete absence of ossification of both parietal bones. This is a rare and severe defect that has been reported in only a few patients with CCD.

Ultrasound evaluation of septo-optic dysplasia in the new born. Report of a case.

A newborn female child with hypothermia, tachypnoea and poor Agpar scores was demonstrated to have absent septum pellucidum, partial agenesis of the corpus callosum and misshapen left lateral ventricle on ultrasound examination. Fundoscopic examination noted hypoplastic optic discs. Given the appropriate clinical setting and fundoscopic examination, cranial ultrasound can be the preliminary evaluation for septo-optic dysplasia and related anomalies in infants with open fontanelles.

[Papilledema and echographically detectable retro-orbital dilatation of the sub-arachnoidal space with open fontanelle - six case reports].

This paper analyses the case reports for three children in which a papilledema occurred before the age of one year. Furthermore, an analysis is also given of three further case reports for children aged less than one year in which, in spite of open fontanelle, no papilledema was found, however, a dilatation of the sub-arachnoidal space was demonstrated echographically. Even in children less than one year of age in which an open fontanelle still exists and in whom a neuro-paediatric clarification of internal hydrocepalus is made, in spite of opththalmoscopically inconspicuous findings for the p

Ommaya reservoir placement using ultrasound guidance via anterior fontanelle combined with frameless electromagnetic neuronavigation in patients with mucopolysaccharidosis type 2: Case reports and review of the literature.

Mucopolysaccharidosis type II (MPS II) results from the genetic deficiency of a lysosomal enzyme and is associated with central nervous system (CNS) dysfunction. In Japan, in addition to intravenous enzyme administration, intracerebroventricular enzyme delivery through the Ommaya reservoir has recently gained approval. Nevertheless, the ideal approach for safely implanting the reservoir into the narrow ventricles of infantile MPS II patients remains uncertain. In this report, we present two cases of successful reservoir placement in infantile MPS II patients using ultrasound guidance via the a

Some nonhuman primates have the capacity for head molding at birth.

Cranial sutures have a changing role through ontogeny. They function as passive growth sites during active brain growth; after brain growth they may serve to dissipate biomechanical forces. A third function at the newborn stage allows mobility of these flexible, fibrous joints as a compensatory means to facilitate parturition. This is most discussed in the context of human parturition, and the apparent dilemma of passing a proportionally large fetus through the constraints of the female pelvis. To test the hypothesis that nonhuman primates also have flexible calvarial joints, we examined a sam

Persistent fontanelles in Chihuahuas. Part I. Distribution and clinical relevance.

BACKGROUND: The Chihuahua dog breed is known for frequent occurrence of a bregmatic fontanelle on the dorsal skull. A common conception is that this skull defect is a clinically irrelevant finding. No studies, however, describe its prevalence or whether it is accompanied by other persistent fontanelles (PFs). Although Chihuahuas are predisposed to Chiari-like malformation (CM) and syringomyelia (SM), it is unknown whether PFs occur more commonly in dogs with clinical signs that are caused by CM or SM. HYPOTHESIS/OBJECTIVES: To describe the number and location of PFs at cranial sutures (CSs) a

The foramen magnum: the underutilized acoustic window to the posterior fossa.

Visualization of the posterior fossa during neonatal cranial sonography through the anterior fontanelle can at times be limited. To improve visualization of the posterior fossa, the foramen magnum was used as an acoustic window. Scans through the foramen magnum were obtained in addition to routine scans via the anterior fontanelle in 21 preterm infants. A subjective and retrospective evaluation was performed comparing clarity and definition of posterior fossa structures using the anterior fontanelle and foramen magnum. Images of the fourth ventricle, pons, vermis, cerebellar hemispheres, and c

Cleidocranial dysplasia in a mother and her daughter within the scope of neurosurgery. Report of two cases.

Cleidocranial dysplasia is an autosomal-dominant disorder characterized by late closure or nonclosure of the anterior fontanelle, late ossification of cranial sutures, defective clavicle, and delayed eruption of permanent teeth. In this article, two cases of cleidocranial dysplasia involving a mother and her daughter are reported, and a case management policy is suggested. The 1-year-old daughter was macrocephalic and brachycephalic, and had midface hypoplasia and hypertelorism. Plain radiographs revealed aplasia of the clavicles. Three-dimensional computerized tomography scanning demonstrated

Influence of suture and fontanelle morphological variabilities on infant head injury biomechanics.

Compared to adults, the infant head exhibits significant differences in both material properties and structural composition, yet far fewer studies exist on infant head biomechanics. Sutures and fontanelles, as integral soft tissue structures of the infant skull, allow flexibility and accommodate head growth during development. However, their influence on infant head responses to external forces remains inadequately studied, which hinders the advancement of infant traffic safety measures, pediatric head injury diagnosis, and forensic assessments in cases of suspected abusive head trauma.
