Cranio : the journal of craniomandibular practice1997eng
This study compared the presence of headache and bruxing behavior among 133 craniomandibular disorder patients (CMD) referred to the The Center For the Study Of Craniomandibular Disorders and to the presence of headache and bruxing behavior occurring in 133 controls seeking routine dental care. Both patients and controls were consecutive referrals to the clinic occurring over a three year period. The mean age of the CMD group was 38 years (range 28-42), and the mean age of the controls was 37 years (range 25-44). The information gathered included questionnaire and clinical examination. Anxiety and depression of CMD-patients in a Clinic for Special Dental Care were assessed with the Symptom Checklist-90. The results of the female patients did not differ from the general population; the men showed significantly higher scores. Approximately one third of the patients scored 'high' or 'very high' on anxiety or depression and may need special attention for psychological problems. The SCL-90 may be a useful instrument to assess the presence of clinically significant anxiety or depression. Psychological data should be interpreted with great care and always in combination with data f

Sixty eight consecutive Slovenian patients with craniomandibular disorders (CMD) participated as the study group and another 400 adults from the regional population sample participated as the control group. The aim was to determine the impact of craniomandibular disorders to the self-perceived oral health related quality of life (OHRQoL) and to compare the OHRQoL with a control group. The mean Oral Health Impact Profile (OHIP) summary scores were computed for all patients with the same diagnosis and for the same subgroups of the axis I according to the RDC/TMD exam protocol. The mean OHIP subs

The hyoid bone maintains crucial anatomical and functional relationships with the mandible, pharynx, and cervical spine. Its position is dynamically influenced by surrounding musculature and fascial attachments. The spatial orientation of the hyoid bone plays a major role in muscular and joint imbalances in patients with temporomandibular joint disorders (TMJD). Neuromuscular orthotic therapy in patients with TMJDs can lead to measurable changes in hyoid bone positioning and cervical spine alignment. Using cone-beam computed tomography (CBCT) and Rocabado's hyoid triangle analysis, this case r

BACKGROUND: Within physical therapy, manual therapy is known to be effective for managing temporomandibular disorders (TMDs). However, manual therapy is a broad term including different approaches applied to different body regions.
AIMS: This is the first systematic review that aims to evaluate the effectiveness of manual therapy applied specifically to the craniomandibular structures (Cranio-Mandibular Manual Therapy [CMMT]) on pain and maximum mouth opening in people with TMD.
MATERIAL AND METHODS: This systematic review was developed based on a pre-determined published protocol which was

A group of patients with a craniomandibular disorder of arthrogenous origin demonstrated an age-related variation of condylar asymmetry with age. This may reflect a greater depletion of the mesenchymal cell layer, which is responsible for adaptation of the articular surface as age increases. This would then result in greater deterioration of the articular surfaces and a consequent decrease in condylar asymmetry.

The effect of age on condylar asymmetry in dentate patients has already been investigated and a clear relationship between these two variables was established both for a group with an arthrogenous origin and with myogenous origin of pain. In this study, the mean asymmetry index for an edentulous group of patients with a craniomandibular disorder of myogenous origin was found to be 12.55%, less than that for both dentate groups. There appeared to be no correlation between age and the asymmetry index in this group, suggesting that different mechanisms may be at work in the development of these d

Sixty-seven per cent of the forty-three patients suffering from craniomandibular disorders (CMD) had a vertical condylar asymmetry greater than 3%. After division into two different groups, myogenous and arthrogenous origin of pain, it was found that myogenous patients were younger and showed significantly bigger asymmetry between the vertical heights of the left and right condyles, evaluated on an Orthopantomogram, than the patients with arthrogenous origin of pain (P less than 0.05). The mean vertical condylar asymmetry was 9.10% and 4.83%, respectively. It is suggested that the possible tri

There is some controversial discussion within the therapy of craniomandibular disorders (CMDs) about the mode of action of occlusal splints. Here we present a case report on one CMD-patient measuring cerebral activation changes with functional magnetic resonance imaging (fMRI) before and after therapy with a stabilization splint. Wearing the Michigan splint for 11 nights and partially days resulted in substantial pain relief and changes in occlusal movement performance. Cerebral activation during occlusion was decreased after therapy (PRE-POST) in bilateral sensorimotor regions but also additi

PURPOSE: This study aimed to establish the prevalence of pain in the craniomandibular and cervical spine region in individuals with Temporomandibular Disorders (TMD) and to analyze the effects of these disorders on the bilateral activation of anterior temporalis (AT) and masseter (MA) muscles during the masticatory cycle.
METHODS: The participants were 55 female volunteers aged 18-30 years. The presence of TMD and craniomandibular and cervical spine pain was evaluated by applying the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) questionnaire and using a combination o

Ankylosing spondylitis is a disease that causes inflammatory changes of the involved joints. Although the initial clinical signs are pain and discomfort, synovial changes progressively involve all the axial joints, including the temporomandibular joint (TMJ). Eventually, bony alterations develop (condylar erosions, flattening, sclerosis) that affect the position of the condyle, the superior joint space, and the range of movements. These symptoms correlate with the severity of the disease. Besides physiotherapy and surgery, no dental rehabilitation has been reported for these patients. This rep

INTRODUCTION: Unfortunately, sharp, severe pain in the area of distribution of the fifth cranial nerve is frequently termed trigeminal neuralgia, and no differentiation is made between typical and atypical neuralgia and other types of facial pain disorders. This can lead to inadequate treatment.
CLINICAL MATERIAL AND METHODS: From 1987 to 1993, 577 patients were referred to our clinic with the diagnosis "idiopathic trigeminal neuralgia". Re-examination of these patients was based on a comprehensive history, behavioural and psychosocial assessment, general inspection of the head and neck, eval

Data collected on 406 patients of both sexes, aged between 9 and 78 (mean age 30.47) and suffering from algias and craniomandibular disorders, have been examined, in order to show the more frequent clinical variables and to improve the knowledge of the stomatognatic dysfunction, by characterizing diagnostic subgroups. The observed prevalence of the female sex and young adults is in agreement with the literature. Older subjects seem to accept more easily dysfunctional problems; the developmental age needs an active prevention. The data were similar to those of the previous researches, demonstra

A comparison has been made between clinical and radiographical findings in a study of eighty-nine patients suffering from craniomandibular disorders. In general it was found that condylar changes were more common in these patients than in earlier studies concerning the severe problems of craniomandibular disorders. A statistically significant correlation between clinical and radiographical findings was found for crepitation with osteophyte formation of the condyle (P less than 0.01). After clinical separation of the patients into two groups based upon the origin of pain, myogenous versus arthr

A group of patients with a myogenous origin of craniomandibular disorders was found to show a variation of condylar asymmetry with age. The curve obtained by plotting patient age against their asymmetry index was parabolic in nature. This differed from a group of patients with an arthrogenous origin of pain, where a linear correlation with age, with a negative slope, had previously been found. The mean asymmetry index for this group of patients was found to be 23.77%. This is higher than that reported for a group of patients with a craniomandibular disorder of arthrogenous origin, namely 18.76

Two hundred and twenty-two patients who had been examined and treated for craniomandibular disorders were re-evaluated using a screening protocol. This protocol was developed from previous studies which had elucidated the most reliable signs and symptoms of craniomandibular disorders. Unilateral signs and symptoms were found to be more common than bilateral signs and symptoms. Dynamic and static pain tests were shown to be decisive in determining the preliminary diagnosis. There was a significant difference in vertical condylar height asymmetry between those patients diagnosed as having a myog

BACKGROUND: The term temporomandibular disorder (TMD) includes disorders of the temporomandibular joints (TMJ), masticatory muscles and adjacent tissues. Several studies have examined the effectiveness of manual therapy (MT) for TMDs by evaluating changes in pain and maximum mouth opening (MMO). Nevertheless, the effectiveness of MT exclusively applied to the craniomandibular structures (craniomandibular manual therapy (CMMT)) on pain and TMJ range of motion remains unclear. This review aims to evaluate the effectiveness of CMMT on pain and TMJ range of motion in people with TMDs.

Correlations between craniomandibular disorders (CMD) symptoms and an inherited factor (general joint hypermobility) were studied in 193 adolescents (96 girls and 97 boys). They answered a questionnaire concerning CMD symptoms, oral parafunctions, head and jaw trauma and symptoms from other joints. Joint mobility was assessed by determining the Beighton score (0-9). Twenty-eight percent of the girls and 21% of the boys could perform four or more of the manoeuvres. Twenty-two percent of the girls and 3% of the boys were extremely hypermobile (score greater than or equal to 5). Eighty-nine perce
