Critical Care Functional Rehabilitation Outcome Measure: developing a validated measure.

INTRODUCTION: In 2009, the National Institute for Health and Clinical Excellence (NICE) developed guidelines, which recognized that rehabilitation plays a vital role in recovery from illness and in returning patients to their previous activity levels. NICE recommended the use of valid and reliable "outcome measures" as an evaluation tool. AIM: To develop and evaluate the validity and reliability of a new Critical Care Functional Rehabilitation Outcome Measure (CcFROM). METHOD: The CcFROM was developed by physiotherapists working within the authors' institution and was based on an evaluation

Tracheostomy Outcomes in Muslim Intensive Care Unit Patients with Acute Respiratory Failure: Highlighting a Critical Palliative Care Gap.

BACKGROUND: Tracheostomy often marks the onset of prolonged critical illness with high mortality, yet outcome data from Muslim-majority settings are unavailable. METHODS: We conducted a retrospective cohort study (2019-2023) of Muslim intensive care unit (ICU) patients undergoing tracheostomy for acute respiratory failure. The primary outcome was in-hospital mortality; secondary outcomes included ICU mortality, code-status transitions, and one-year survival. RESULTS: Among 411 patients (median age 68 years; 55% male), 68% were hospice-eligible at admission, while 38% were bedridden. In-hospi

The Association Between Extravascular Lung Water and Critical Care Outcomes Following Bilateral Lung Transplantation.

UNLABELLED: Primary graft dysfunction (PGD) is a form of acute respiratory failure that complicates 30% of bilateral lung transplants. Higher grades of PGD correlate with higher severity of respiratory failure and unfavorable outcomes. Immediate PGD determination posttransplant' however, is not always predictive of PGD over subsequent days or intensive care unit outcomes. We aimed to evaluate whether extravascular lung water index (ELWI) measured immediately post bilateral lung transplant was associated with higher severity of PGD at 72 h and duration of mechanical ventilation. METHODS: We co

Nutritional Risk at intensive care unit admission and outcomes in survivors of critical illness.

BACKGROUND AND AIMS: Risk factors for poor outcomes after critical illness are incompletely understood. While nutritional risk is associated with mortality in critically ill patients, its association with disability, cognitive, and health-related quality of life is unclear in survivors of critical illness. This study's objective was to determine whether greater nutritional risk at ICU admission is associated with greater disability, worse cognition, and worse HRQOL at 3 and 12-month follow-up. METHODS: We enrolled adults (≥18 years of age) with respiratory failure or shock treated in medical

The association between prior statin use and long-term outcomes after critical care admission.

BACKGROUND: Statins may have immunomodulatory effects that benefit critically ill patients. Therefore, we retrospectively examined the association between survival and the prescription of statins prior to admission to an intensive care unit (ICU), or high dependency unit (HDU), as a result of major elective surgery or as an emergency with a presumed diagnosis of sepsis. METHODS: We retrospectively studied critical care patients (ICU or HDU) from a tertiary referral UK teaching hospital. Nottingham University Hospitals have more than 2200 beds, of which 39 are critical care beds. Over a 5-year

Post-operative critical care and outcomes of limb replantation: experience in a developing country.

Replantation is the treatment of choice for traumatic amputation. Its success rates vary, reaching 80% in world's best centres. This study analyses management practices of replantation in a regional centre in a developing country. Out of six replantations, four were successful. The median warm ischaemia time of the severed limb was 4.5h (range 1-13.5) and the median duration of general anaesthesia required for initial surgery was 6.25h (range 4.7-8.0). All patients needed intensive care following replantation for a median of 7 days (range 5-15). Pulse oximetry values were observed to be the sa

Can Model-Informed Precision Dosing of Beta-Lactams in Critical Care Improve Clinical Outcomes?

<p>Background: Critically ill patients exhibit acute physiological changes leading to marked antibiotic exposure variability and an increased probability of either subtherapeutic or potentially toxic exposures. Beta-lactam antibiotics are often affected by these physiological changes, leading to variable and often subtherapeutic exposures. Therefore, methods to increase the probability of a therapeutic exposure may be beneficial for critically ill patients. Model Informed Precision Dosing (MIPD) is an emerging strategy using advanced computer models to individualise dosing in this vulnerable p

Critical care outcomes of hemophagocytic lymphohistiocytosis in children: a single centre retrospective observational study.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a challenging and potentially life-threatening condition characterized by an uncontrolled inflammatory response resulting from hyperactivation of T lymphocytes and macrophages, leading to excessive production of inflammatory cytokines. HLH can lead to multiple organ failure and death in a significant proportion of cases. It is challenging to differentiate HLH from sepsis. This study aims to describe the clinical characteristics, risk factors, and outcomes of a cohort of children admitted to a pediatric intensive care unit (PICU) with prim

Pre-pubertal testicular torsion: A critical analysis of outcomes from a tertiary care center.

INTRODUCTION: Pre-pubertal testicular torsion has typically been associated with high rates of orchiectomy, but there are scarce data on postoperative atrophy in salvaged testicles. OBJECTIVE: To study testicular salvage rates in pre-pubertal boys undergoing surgery for acute testicular torsion in a large, tertiary care academic center to assess true 'testicle loss rate'. STUDY DESIGN: We performed a retrospective review of all pre-pubertal (aged 3 months-10 years) patients treated for acute testicular torsion over a 9-year study period (2015-2023). Patients underwent surgery by one of seven

A Bayesian genomic-enhanced precision critical care early warning model for septic shock outcomes.

BACKGROUND: Septic shock remains a significant contributor to mortality, with high patient variability in treatment response. Precision medicine offers promise in addressing this complexity. This study aimed to develop a Bayesian genomic-enhanced precision critical care clinical decision/probability model to predict outcomes in septic shock patients. METHODS: Utilizing Bayesian modeling, we integrated clinical scoring systems (e.g., the Miami sepsis score and point of care lactate) with genomic data. Sensitivity and specificity from prior studies were incorporated into the model. The effectiv

Bridging Haemato-Oncology and Critical Care: Survival and Functional Outcomes From a Portuguese Central Hospital.

Background Despite substantial therapeutic progress in haemato-oncology (HO), referring exclusively to patients with haematological malignancies, ICU admission decisions for these patients are often hindered by the perception of poor prognosis. This study aimed to evaluate the one-year survival and functional outcomes of critically ill HO patients following hospital discharge. Methods A retrospective, single-centre observational study was conducted in a Portuguese tertiary care centre, including 54 consecutive non-elective ICU admissions of adult HO patients between January 2022 and October 20

Trauma as a critical condition in the very old: assessment, care pathways and outcome differences.

Trauma in very old adults represents a growing and distinct form of critical illness, reflecting demographic aging and increasing levels of frailty, multimorbidity, and dependency. Unlike younger populations, where trauma is typically driven by high-energy mechanisms, injury in older adults most commonly results from low-energy falls. Despite this, the consequences are often severe, with high rates of hemorrhage, traumatic brain injury and thoracic trauma, and subsequent multi-organ dysfunction requiring critical care. Age-related physiological changes, combined with multimorbidity and polypha
